During OJT, a beltpack crew pulling 24 loads/7 empLes from B Yard to C Yard, crew was pulling at 10 mph furthermore regressing speed while proceeding down the lead towards the C4 switch which was known to be lined against the approaching movement. At a distance of 6 car lengths from switch employee along with speed applied the RCLS brake, movement was not moving at a speed that would allow him to stop short of the switch resulLng in the movement being put into penalty furthermore running thru the C4 switch by 1 engine and 1.5 cars.

Evacuation response co-ordination between CP and
Westshore needs to be improved. Had two reported
incidents where a CP employee has gone to spot a coal train in the dumper and there was no dumper operator present. It
was later determined the dumper operator had evacuated due to high readings on his carbon
monoxide sensor.
If Westshore employees need to evacuate the dumper
Westshore supervisors should communicate this to BC Rail
who can properly notify the CP employees.

We request to perform a mock evacuation in Coquitlam Yard to identify weak points in the
evacuation process and to ensure all employees are familiar with this process.
Earlier this month we had a false alarm where the
emergency horns on top of the carshop were activated. There was no emergency broadcast from the tower to advise that
this was a false alarm. We missed the opportunity to test
the emergency broadcast system on our digital channels.
This emergency broadcast system needs to be tested and
employees should be made aware of this information via a
bulletin and the protocol tested.

Sunscreen has not been provided in the vending
machines lately even though it is a legislative requirement of the Canadian Occupational
Health & Safety Regs and the Onboard Train Regulations.
We understand that this issue has now been addressed and
the vending machines are stocked or will be shortly.

Shane Roseberry Schematic and SOP to be rolled out for new track infrastructure and operating
practices while down at Williston at Pacific.
H2S sensors at IOCO and Chevron must be used by all employees and contractors. Should see if they can provide a pair for CP crews to use at the facility. Shane Shane Roseberry to look into it.
