BASED ON WHERE THE DOCTOR’SOFFICE IS LOCATED

To choose a doctor based on convenience to your homeor work location, turn to the listing of primary carephysicians by county. In this listing, the name of eachprimary care physician is grouped in alphabetical order by the city in which his or her offices are located.

BASED ON THE DOCTOR’S HOSPITAL AFFILIATION

If you are interested in which hospitals are affiliated withHealth Net, please refer to the hospital index. You shouldconsult with your primary care physician to determinewhether he or she has admitting privileges at the hospitalyou prefer. Your physician will coordinate all your caredirectly.

HOW TO COMPLETEYOUR ENROLLMENTFORMAfter you’ve chosen a primary care physician, you’reready to complete your Health Net enrollment form. Just follow the steps below:

1. Use one of the methods mentioned on the previouspage to choose a primary care physician.

2. Verify the physician’s office location. Confirm that thelocation is convenient for you. To ensure that youhave reasonable access to care, we ask that you resideor work within 30 miles of your chosen doctor’s office.

3. On your enrollment form, write the primary carephysician enrollment code of the physician you’vechosen. You can choose a different primary carephysician for each member of your family.

SOME FREQUENTLY ASKED QUESTIONSBecause you may come across some terms that areunfamiliar to you, we’ve included some commonly askedquestions relating to these terms.

HOW IS A REFERRAL SPECIALISTDIFFERENT FROM A PRIMARY CAREPHYSICIAN?

Your primary care physician is your main medicalprovider. He or she is the doctor who is responsible forcoordinating all of your medical care and authorizing allspecialized treatment or hospitalization. As a member ofHealth Net, you must obtain a referral from yourprimary care physician to see a specialist. Women mayself-refer to a network OB/GYN at any time withoutauthorization from their primary care physician. Referralspecialists are not available for selection as your primarycare physician.

WHAT ’S THE DIFFERENCE BETWEENEMERGENCY CARE AND URGENT CARE?

You need emergency care for a sudden, serious andunexpected illness, injury or condition, including severepain, that a reasonable person with an average knowledgeof health and medicine would believe requires immediatemedical attention.

Urgent care, on the other hand, is care provided formedical conditions that require treatment within severaldays but are not considered immediately life-threatening.These may include sprains and fractures, a sudden highfever or pneumonia, or a flare-up of a chronic conditionsuch as asthma.

Health Net covers care received from any licensedprovider or treatment center anywhere in the world forany emergency situation. Situations requiring urgent careare covered when received from or authorized by themember’s primary care physician. All non-emergencycare must be provided by or through the member’sprimary care physician.

HOW CAN I GET A LIST OF SUB-ACUTECARE FACILITIES?

Sub-acute facilities provide sub-acute care for the special nursing needs of patients who do not needacute care and are too ill to be cared for by most skillednursing facilities (SNFs). These include transitionalsettings following acute hospitalization (such as thosedischarged from hospitals but not fully recovered).

To request a Sub-Acute Care Facility list, call the Health Net Customer Contact Center at the numberlisted on your Health Net Life ID card.

SHOULD I CHOOSE A DOCTOR WHO ISCLOSE TO HOME OR CLOSE TO WORK?

That decision is entirely up to you. But your choice will probably depend on when you think you’ll access care most often. If you prefer to makeweekday appointments, choosing a doctor who’sconvenient to your work location can make sense.However, for those times when you wake up feeling ill,you’ll probably appreciate having a doctor whose office is closer to home. Our general advice – to ensure thatyou have the greatest access to the care you need – is that you choose a doctor whose office is located within 30 miles of either your work or your home.

WHEN CAN I CHANGE TO A NEW DOCTOR?

You may choose a new primary care physician on a monthly basis. If you choose a new doctor before the 15th of the month, you can begin seeing your newdoctor anytime after the 1st of the next month. If youchoose a new doctor after the 15th, you’ll need to wait as long as six weeks before you can begin seeing yournew doctor. For example, if you call the CustomerContact Center on August 12, you can begin seeing your primary care physician anytime after September 1. If you call on August 18, you’ll be able to see your new primary care physician on October 1.

HOW CAN I OBTAIN INFORMATION ON DOCTORS WHO ARE IN THEHEALTH NET NETWORK?

When doctors agree to provide care to Health Netmembers, they give us some key information about theirqualifications. To help you make informed decisionsabout your care, we make much of this informationavailable in our directories, on our website, or by callingthe Customer Contact Center. You can learn whetheryour doctor is board certified, where your doctorattended medical school, where your doctor performedhis or her residency, your doctor’s address and phonenumber, the hospital(s) at which your doctor practices,and which languages (in addition to English) your doctormay speak.

HOW HEALTH NETWORKS

HOW TO RECEIVE CARE

Your primary care physician is responsible for performingor authorizing all your covered medical services,including hospitalizations. (Women may self-refer to a network OB/GYN at any time without authorizationfrom their primary care physician.) Your primary carephysician’s office operates just like any other privatepractice doctor’s office. When you need to see yourdoctor, just call for an appointment. It’s that simple.

ABOUT YOUR HEALTH NET LIFE IDCARD

After you enroll in Health Net Life, you will receive aHealth Net Life ID card. This ID card is your “passport”to all of your care as a Health Net Life member. It contains all the information that anyone needs toknow about you as a member of Health Net Life,including your level of benefits and the number to call for authorization in an emergency. Because it is soimportant, it’s a good idea to keep your Health Net LifeID card with you, wherever you go. If you lose yourcard, call the Customer Contact Center for a new one. Ifyou ever change your primary care physician, we’ll sendyou a new card in the mail.

HOSPITALIZATION

Your primary care physician may find it necessaryto refer you to a hospital for further care, on eitheran inpatient or an outpatient basis. You will usuallybe hospitalized where your physician has admittingprivileges. Please consult your primary care physician to determine where he or she has admitting privileges.

It is important to remember that your primary carephysician must first be contacted for initial treatment and consultation before you receive any care or treatmentthrough a hospital. Your primary care physician will be responsible for determining the most appropriate site and location for your additional care. Admission to and treatment rendered by any hospital listed in thisdirectory must be authorized by your selected primarycare physician.

BEHAVIORAL HEALTH PROVIDERS

Health Net has contracted exclusively with ManagedHealth Network (MHN) specializing in mental health and chemical dependency services.

Health Net members can directly contact ManagedHealth Network by referring to the telephone numberlisted on your Health Net Life ID card.

GLOSSARY OF TERMSAuthorization: If you need a special type of care, your primary care physician will refer you to a specialist. If your doctor’s request for referral is approved,you will be given a form called an Authorization forConsultant’s Services. This form lists the type of care orservices that you need and the number of visits that youcan schedule. Always check with your primary carephysician to make sure all your visits to a specialist arecovered by Health Net.

Primary care physician: The physician who isresponsible for performing, prescribing, directing orauthorizing all of your health care, including routine,referral and follow-up treatment. If you need a specialtype of care, your primary care physician will beresponsible for referring you to an outside specialist.

Provider: This term applies primarily to any facility,physician or other health care professional contractedwith Health Net to provide care to members on apreauthorized or contractual basis.

Referral: See authorization.

Referral specialist: Your primary care physician is responsible for administering your care, and he or she will authorize a referral to an appropriatespecialist when necessary. You may not select a referralspecialist physician as your primary care physician.

Urgent care: When conditions require treatment within several days but are not considered immediately life-threatening, they require urgent care. Examples of urgent medical conditions are sprains and fractures. If your primary care physician is not available to see youon a same-day basis, you will be referred to a facilitywhere you can receive the care you need.
