User’s Guide: Keeping You Active

O steoarthritis occurs when cartilage in the joints wears away, causing bones to rub directly against

each other as the joint moves. this painful chronic condition can occur in any joint, but it’s most common in knees, hips, hands and spine.

today’s patients have many options for treating osteoarthritis—and the full range is available at AmitA Health. typically, patients start with nonoperative treat-ments, which tend to work best when diagnosed early. these include injections, stem cell therapy, physical and/or occupational therapy, nonsteroidal anti- inflammatory drugs (NSAiDs) and even weight loss.

An injection called viscosupplementation offers relief for knee osteoarthritis by coating the cartilage and lubricating the joint. “this series of injections can be performed in a physician’s office, and it takes all of five or 10 minutes,” says Theodore Suchy, DO, a board-cer-tified Orthopedic Surgeon at AmitA Health. “it works in about 80 percent of patients and gives those patients about 80 percent relief, with that relief lasting anywhere from six months to five years.”

if joint pain continues to interfere with quality of life, it may be time to consider surgery. “We determine the surgical approach based on patients’ anatomy and their personal expectations after a discussion with them,” says Daniel Kuesis, MD, board-certified in orthopedic surgery, sports medicine and joint replacement.

Surgical alternatives to joint replacement include joint resurfacing; partial knee replacement; and a minimally invasive procedure called hip arthroscopy, which involves repairing cartilage or damaged tissues

GOT JOinT Pain?or removing bone spurs or loose fragments in the joint. “there’s a big push for hip preservation, and that’s maintaining your current joint,” says Dr. Kuesis.

AmitA Health also offers the minimally invasive, muscle-sparing anterior approach to hip replacement. “Patients’ pain is way less, and many leave the hospital one or two days after surgery, instead of three to five days after standard hip replacement,” says Dr. Kuesis.

in general, joint replacement procedures have advanced, says Dr. Suchy, meaning patients are expe-riencing shorter operating times and hospital stays and quicker recoveries. improvements include smaller inci-sions, more precise operating techniques, advances in local anesthetic techniques and longer-lasting materials.

What’s another factor contributing to excellent surgi-cal outcomes? “AmitA North offers a comprehensive preoperative patient education program so patients know what to expect,” says Dr. Suchy. “We also offer ‘joint camp’—group therapy twice a day in the hospital. in days past, patients would lie in bed for three days before they were allowed to be moved. Now we do the surgery in the morning and the patient is walking in the afternoon.”

SIGN UP FOR THE 5K at the AMITA Health Fitness for America Sports Festival on July 23 in Hoffman Estates. For info, visit allcommunityevents.com/ gfasf/gfasf_register.

How to train for Your first 5Ktips for beginners from Ciro CirrinCione, MD, CHief of ortHopeDiCs at amita HealtH st. alexius medical center,

Hoffman estates

If you have cardiovascular risk factors, see your doctor before beginning an exercise program.• Start developing a fitness and strength base about three

months before the race. “The average person who has not exercised before should start in the gym,” says Dr. Cirrincione. On the stationary bike, build up to at least 20 minutes three times a week with significant resis-tance and get your heart rate to at least 80 percent of the maximum rate. Don’t have access to a gym? Walk with ankle weights. Focus strength training exercises on muscle groups including quads, glutes, hamstrings and core.

• Select proper, supportive footwear—and use orthotics if needed.

• Start gradually and cross-train. Once you start running, begin with half a mile and increase distance gradu-ally over six weeks until you reach 3.1 miles. Run every other day, up to three days a week. On the other days, cross-train on the bike or elliptical or in the pool, and do strengthening activities. Remember to rest as well.

• Warm up and cool down—and stretch properly. “The ideal way to stretch is to warm up a little bit on a bike or by walk-ing and then stretch muscle groups including the ham-strings, quads and lower back,” says Dr. Cirrincione. Cool down by walking, then stretching and finally icing the joints.

• Listen to your body. Injuries occur when you try to do too much, too fast.

• As race day approaches, do light workouts two to three days before. On race day, stay hydrated and stick to the pace at which you’ve been training.

If you’re a walker, many of these tips—including wearing sup-portive shoes and gradually increasing distance—still apply.

More than 1/4 That’s the fraction of U.S. adults who reported having chronic joint pain in a 2013–2014 survey. Women were more likely than men to report having chronic joint symptoms, with nearly 30 percent of women reporting having pain, aching or stiffness in or around a joint for more than three months.Source: Morbidity and Mortality Weekly Report,

A Closer LookA Closer Look

if you have never attended a support group, you may wonder what these groups are all about, how they op-erate, or why you may want to join one. Here’s an over-

view from the point of view of participants and experts.Simply put, a support group is a gathering of people

who share a similar interest or health concern. typically, a support group focuses on a specific circumstance, such as loss of a loved one, or a common condition, such as addiction, depression, breast cancer or diabetes.

the format of support groups differs—ranging from those led by the group members themselves to those fa-cilitated by a professional, such as a nurse, psychologist, social worker or chaplain. the emphasis of some support groups is on shared experiences and emotional sup-port; others are more formal and informational, perhaps featuring presentations about topics that suit the group’s interests and needs; still others have a spiritual basis and help members navigate their particular spiritual journey, explore the power of prayer or talk with others of the same faith tradition.

Note: A support group is not the same as a group ther-apy session, which is a formal type of mental health treat-ment, experts explain. in group therapy, a trained mental health provider guides gatherings of several people with

similar conditions. Other alerts from experts: Attending a support group shouldn’t substitute for regular medical care, nor are support groups for everyone. Some people may prefer exploring their issues on their own, perhaps through journaling or in a one-on-one relationship with a counselor or chaplain.

SkIp The WAITIng RoomSave time with our easy-to-use online tool and schedule appointments at our ERs, immediate care locations and more

AMITA Health’s online self-scheduling lets you check estimated treatment times at AMITA Health’s ERs and book appointments for a variety of healthcare services.

“Patients love it,” says Lisa Jordan, MSN, RN, CEN, CPN, director of Emergency and Trauma Services, AMITA Health St. Alexius Medical Center, Hoffman Estates. “People don’t want to waste time. If they know there’s a wait, they have the option to do so in the comfort of their own home.”

To use the tool, visit waitathome.org. At a glance, you’ll see waiting times at our ERs in Elk Grove Village and Hoffman Estates, as well as at immediate care facili-ties in Addison, Elk Grove, Bensenville, Hanover Park, Mt. Prospect, Schaumburg and Palatine. With a click, you can also see available appointment times in the next 24 hours for certain primary care physicians, laboratory services and some other care services.

FYI: Your online registrations send an alert to AMITA Health staff with your name, phone number and medical issue, Jordan adds. “It gives us time to plan for that patient who’s coming in and we know what we’re dealing with,” she says. “It can be a really good thing because it also allows us to triage the cases. If needed, we can tell patients to call 911 or come in immediately.”

4U ( Ultimately, patients with life-threatening or more serious injuries will be seen at AMITA Health facilities prior to those with non-life-threatening issues. Please remember to dial 911 in the case of any emergency.

For years, medical experts have told us we should limit cholesterol in our diet to help reduce artery-clogging LDL (bad) cholesterol in the bloodstream and lower our risk for cardiovascular disease. Now, new advice is being recommended in proposed updates to the Dietary Guidelines for Americans published every five years by the U.S. Department of Health and Human Services and the U.S. Department of Agriculture. Dawson explains what’s up.

What changes about cholesterol have been pro-posed by the Dietary Guidelines Advisory Commit-tee (DGAC)? Previously, the Dietary Guidelines for Americans (DGA) recommended that cholesterol intake be limited to no more than 300 mg/day. The 2015 DGAC changed these recommendations because available evidence shows no appreciable relationship between consumption of dietary cholesterol and serum (blood) cholesterol. In making its recommendations to eliminate this limit, the DGAC cites an American Heart Association and Ameri-can College of Cardiology report that concludes there’s insufficient evidence to show that restricting dietary cholesterol lowers LDL (bad) cholesterol in the blood. The fact that elevated serum cholesterol is a major risk factor for cardiovascular disease (CVD) isn’t in dispute, however. Higher serum LDL cholesterol does correlate to higher CVD risk. The question is whether eating a large amount of high-cholesterol foods leads to high serum LDL, and whether limiting dietary cholesterol intake lowers serum LDL.

Does it mean that healthy folks can now eat as much shrimp, lobster, eggs and other high-choles-terol foods as we want? No, because we want to eat all foods in moderation. We also have to remember that a lot of high-cholester-ol foods also contain high levels of saturated fat. Re-search is more definitive about saturated fat, and the DGAC continues to list saturated fat as a “nutrient of concern for overconsumption” and states that decreas-ing saturated fat will lead to lower CVD risk.

Is there controversy among experts about the pro-posed cholesterol recommendations? The Center for Science in the Public Interest and Physicians Committee for Responsible Medicine are among those that still think evidence supports restric-tions on dietary cholesterol. I advise people to talk to their healthcare providers regarding their serum cholesterol levels, personal and family history of CVD, and weight and lifestyle habits, including your diet. Together, you and your provider can decide what ap-proach is best for you.

A Q&A with Colleen Caron, RN, Laughter Yoga teacher and community liaison at AmitA Health Alexian Brothers Behavioral Health Hospital, Hoffman Estates:What are some of the underlying theories of Ageless Grace exercises?Caron: it’s all about building neuroplasticity—the brain’s ability to make new pathways, new connections and capabilities. thanks to today’s science, we now know the brain is much more elastic than we ever thought and we can make new pathways at any age. What are some of the exercises like?Caron: Basically, you’re doing things your brain and body aren’t used to. Each of the 21 exercises lasts three minutes or so and is done to music, and we recom-mend devoting 10 minutes a day to the effort. One of the exercises, for example, is called Gentle Geometry, and in doing it, you trace shapes in the air with one hand while your opposite foot does something different. What are the benefits of these types of workouts?Caron: many—including improvements in joint mobility, spinal flexibility, cogni-tive function, right- and left-brain coordination, bone density, balance, systemic health. it’s very playful, too. Everyone laughs while they do it, and it builds confi-dence and self-esteem.How will you use Ageless Grace in this area?Caron: i plan on doing trainings for the community and also training for AmitA Health activity and expressive therapists so they can offer it to our patients—dementia patients, outpatient cancer patients, the depression caregiver group. it would be good for them all. truly, it’s something everyone can do. i practice every day. i have more energy than most people my age and many people younger!

Calcium ScoreIf you are between 50 and 65 years of age and have a fam-ily history of heart disease or coronary artery disease, you may wish to consider having a calcium score completed. A calcium score is a CT scan to quantify the amount of calcified plaque in the heart arteries. This amount, or “score,” has been shown to predict the long-term risk of cardiovascular events such as heart attack. This is a 10-minute test. Ask your primary care doctor for a referral. For more info, call 847.981.3680. Fee: $99

Vascular ScreenMore than 12 million people suffer from peripheral arterial disease (PAD) and are four times more likely to die of a heart at-tack. PAD may be present if you feel pain, numbing, aching, tin-gling or cramping in your limbs or buttocks, especially while walk-ing. Consider having a screening if you have a family history of PAD or if you are experiencing any of these symptoms. This is a simple, three-part, noninvasive ultrasound test that takes about 30 minutes to complete. For more info, call 847.981.3680.Fee: $99

Briefings

Just for Women AmItA Health’s multidisciplinary Women’s Heart Program offers a coordinated approach to heart disease prevention, diagnosis, treatment and ongoing care

Did you know 90 percent of women have one or more risk factors for heart disease or stroke? You might be among them and not even know

it, since some risk factors, including high blood pressure and high cholesterol, typically don’t have symptoms. The first step toward taking control of your cardiovascular health is talking to your doctor.

AMITA Health’s Women’s Heart Program offers a risk assessment, which involves meeting with a nurse practitioner to discuss your family history; get an EKG; and screen for high cholesterol, blood pressure and other risk factors. Experts say 80 percent of heart disease and stroke events may be prevented simply through education and making a commitment to a healthier lifestyle.

Additional services at the Women’s Heart Program include:• Preventive care and lifestyle management• Full spectrum of cardiovascular treatments,

from minimally invasive techniques to open- heart surgery

• Treatment of complex coronary artery disease• Treatment of complex peripheral vascular disease• Venous procedures, including treatment of blood

clots and varicose veins• Complimentary nutritional counseling• Advanced lipid testing• Free blood pressure screenings• Coronary calcium scoring• High-risk hormone counseling • Community lectures

Improving AAA Care for WomenAbdominal aortic aneurysm (AAA) occurs when the aortic wall weakens or starts to bulge, which can lead to a potentially fatal rupture

AMITA Health is participating in a clinical trial called the LUCY Study, which aims to increase the number of women eligible for a minimally invasive approach to treating AAA.

“Women don’t think about [AAAs] enough because a lot of the research on aortic aneurysms was done in populations that were predominantly male,” says Chelsea Dorsey, MD, AMITA Cardiovascular Associates Health Vascular Surgeon and one of the study’s investigators. “There isn’t a lot of hard data specific to women, but women with specific risk factors for aortic aneurysms are at risk for higher rates of rupture.”

Risk factors include smoking, high blood pressure, high cholesterol, inflamed arteries, family history and genetic conditions such as Marfan’s syndrome.

Because women tend to have smaller access vessels, they are less likely to qualify for endovascular aneurysm repair (EVAR). The LUCY study—named after Lucille Ball, who died of AAA—involves testing a device designed for patients with smaller femoral and iliac access vessels.
