Social care staff can help people with learning disabilities use health services They can help people live healthier lives

Social care staff can sign a charter to say what they will do to help people with learning disabilities be more healthy

This is what the charter says

All staff should understand and use the Mental Capacity Act

Listen to, respect and involve family carers so people get good support

People should be helped to understand about their health, using accessible information

If you go to a health appointment, you should have someone with you who knows you and can support you well

People should be helped to go to health screening appointments

Staff should be able to give medicines safely

People should be supported to go for health checks by:

Helping people to understand the importance of a health check

Helping them to ask the GP for a health check

Helping them to fill out a pre-check questionnaire

Working with health staff to put reasonable adjustments in place (this means making changes to services so they are easier to use)

Making sure the person has someone they know with them during the health check – if they want

Asking about any things than need to happen after the health check – and make sure these are followed up

Telling the Clinical Commissioning Group if there are problems getting health checks for people with learning disabilities

People should have health action plans and hospital passports if they want them. This can mean:

Using the health action plans and hospital passports that local services recognise

Working with the person with learning disabilities to develop their plans in a way they understand

Checking that the plan can be shared

Making sure the plan goes with the person to health appointments or hospital

If the plan is not used by health staff and the person is at risk – making sure the safeguarding team know

If people have lots of health problems we will ask for a health co-ordinator

We will tell people with learning disabilities, their families and staff about this charter
