Primary, dental, lab, case management and behavioral services are offered to patients within one convenient system of care. But did you know that PCHC also has two pharmacies that dispensed over 96,000 prescriptions for needy Puebloans in 2014? The pharmacies (located at 110 E. Routt and 1302 E. 5th Street) participate in the 340B Drug Pricing Program that was established in 1992 to provide patients with affordable prescription drugs. It allows qualifying providers (like PCHC) to stretch their limited federal resources to offer a comprehensive set of services. According to research commissioned by the National Association of Community Health Centers (NACHC), qualifying providers can save from 15 to 60% on the costs of prescription drugs through the 340B program, which, in turn, are passed on as savings to patients and taxpayers. It is critical that patients who are diagnosed with a chronic illness (such as diabetes and heart disease) adhere to their prescription therapy. By offering on-site and discounted pharmacy services to its patients, PCHC works to increase the likelihood that patients will take medications as prescribed. While it may seem insignificant, adherence contributes to the improvement of the individual’s health, but also to the health of the entire community by lowering costly emergency visits and avoidable hospitalizations. Pueblo Community Health Center takes care of the whole person through its integrated health care model. Primary, dental, lab, case management and behavioral services are offered to patients within one convenient system of care. Letter from the CEODear Friends,

Can you imagine a scenario in which your health care needs would be met without access to affordable medications? Probably not--which is why Pueblo Community Health Center operates two full-service pharmacies for all patients who choose PCHC for their health care home. Quality primary care cannot be achieved without professional, safe and affordable pharmacy care. In this issue, we are profiling our pharmacy program which PCHC started in 2000.

A little over half of the year is over and I would like to share a couple of key outcomes we have seen thus far:

• Access to care is strong. PCHC enrolled 327 newpatients per month for the first six months.

• Quality of care is strong. The State of Coloradorecognized PCHC as the best women’s cancer screening program in southeast Colorado.

• Outcomesforpatientsintheareasofchildhoodimmunizationsandthenumberofpatientswith high blood pressure who have this chronic disease under control exceeds the Healthy People 2020 goals established by the U.S. Health and Human Services Department and the Centers for Disease Control.

• Employee turnover rate is 12.5% compared to a health care sector rate that fluctuatesbetween 20-25%.

Most rewarding aspect of your job: I absolutely love patient interaction and being able to provide med-ications to those who might not be able to afford them in any other venue. My most rewarding memory was a woman we were able to help with a Patient Assistance program. She brought us all to tears when she held my hand and stated that “she could now afford some groceries”. I also am grateful everyday to have an excellent, compassionate team working with me.
