  • How to complete the form You’ll need to include the contact and identification information for the Customer-Generator. • Name• Agreement ID• TMK• Service Address and Email

    You’ll also need the contact information for the Project Developer/Contractor • Company Name• Point of Contact• Email and Phone number

    This information allows us to correctly identify the project and verify the accuracy of the contact information in our files.

    By submitting this form, contractors “hereby declare the following to be true and correct with respect to the Project.”

    I have personal knowledge of the records of the Contractor that are the subject of this Declaration. I have the express authority to sign this Declaration on behalf of the Contractor. I have completed the verification tests of customer–owned equipment to verify protective settings and functionality to ensure that the equipment will not adversely affect the utility distribution system and that it will cease providing power to the system under abnormal conditions. The City and County building permit for the subject project is closed The Contractor has the following insurance in place to cover the subject project:

    – Personal liability, General Commercial Liability, Umbrella Policy, or Aggregate thereof− 10kW ≤ Size ≤ 30kW: $500,000 required− 30kW ≤ Size ≤ 100kW: $1,000,000 required

    The electrical contractor stated on the last submittal is accurate The inverters installed are compliant with current advanced inverter functionality for the Program. The installed system also complies with all tariff requirements. Electrical Drawings have been updated to reflect any as-built changes which may have occurred since the last submittal. Signage placed on equipment meets both National Electrical Code (NEC) and HECO’s Electric Service Installation Manual (ESIM) standards.     …that by submitting this attestation I am choosing to bypass the completeValidation/Verification process and I am liable to ensure that my project is compliant through- out the life of the project under Rule 14H and all other applicable tariffs; and, I will be required to demonstrate compliance at any time if audited or risk disconnection of my private rooftop system from the Hawaiian Electric Company’s distribution system in accordance with Rule 14H

  • Sign, submit and we’ll take it from there. By giving us “your word” that all of the steps required at validation have been taken, we’ll expedite the process. We’ll have the meter switched out and the final agreement prepared for signature.

    HOW TO SUBMIT THE ATTESTATION FORM

    Email or CIT? The method depends on the program and/or when/how the program application was submitted. Email – Customer Grid Supply– NEM– Customer Self Supply (before CIT was available)

    Customer Interconnection Tool (CIT) – Customer Grid Supply Plus– Smart Export– Customer Self Supply (if the application was initiated

    through CIT)

  • Instructions for NEM, Customer Grid Supply, and Customer Self Supply applications initiated prior to the availability of the CIT: Fill out the form completely and obtain the required signatures. Scan and save the document (pdf or jpg files work best). Attach the document to an email and send to [email protected].

  • Instructions for all other programs If you already use the Customer Interconnection Tool for managing the application, you will also use it for submitting the form. If your CSS system application was submitted prior to the availability of CIT, please use email to submit the form.

    Read the information, mark the check box at the bottom of the page, then click Save and Continue

    CIT step-by-step Enter Permit Number, Permit Closure Date, Electrical Contractor Hawaii License ID and Hawaii License Holder information. You do NOT need to upload a copy of the building permit.

    CIT step-by-step Under the Inverter Compliance Section, type in “EVO ATTESTATION” in the name field and upload a copy of the signed attestation form.

    CIT step-by-step Complete the Property and Equipment Accessibility section. This is required to submit your validation/attestation. Click Save and Continue at the bottom of the page.

    CIT step-by-step Review the information on the Project Validation Review page and enter your first and last name to sign the Acknowledgement. Click Submit.

    That’s the end of your steps. Clicking submit sends a notification to Hawaiian Electric to review your form.
