Welcome New Partners in Caring I am pleased to announce in this issue of the Extended Care Chronicles three new facilities that join the ECC consulting family: Naperville Manor Health & Rehab Center, South Holland Manor Health & Rehab Center and Westmont Manor Health & Rehab Center. These facilities were formerly facilities operated by Manor Care HCR the second largest nursing home chain in the US. Manor Care is rich in history starting out in 1959, when Stewart W. Bainum, Sr., a former plumber, opened a nursing home in Wheaton, Maryland. The company went public in 1969. In 1998, Ohio-based Health Care and Retirement Corporation (HRC) merged with Manor Care to become HCR Manor Care. The company headquarters was consolidated in Toledo. The change of ownership and the ownership authorization to use ECC as consultants is effective October 1, 2018. We are very fortunate that a large numbers of employees from these three facilities have elected to stay with their facilities and ensure that resident care and services (our number 1 priority) will be seamless in this transition. They are made easier by the dedicated, professional, caring staff of each of the facilities and the hard work of the professional, dedicated, caring staff of ECC. It is times like these that I reflect and recognize that I am thankful for teams we have in place. Special thanks on the preliminary transition work of Karla Grzeskowiak, VP, Human Resources, David Wong, VP of IT and his staff Mir, Phil, and Russ, Darria Warnock, EMR Matrix (and now PCC Administrator) and Jake Gabay, from the Real Estate Operations of Hunter Management. Over the next few weeks all of us at ECC will be learning the Manor Care operations. We will assess and evaluate what is working well, what systems needs to be tweaked, and what systems we want to move to the ECC model. These changes will take time but overall our care and services to our residents can never be disrupted. Also over the next few weeks we will be setting up meetings with the residents and family members as well as meetings with Physicians, Hospital partners and other partners in caring. These meetings are really designed for everyone to get to know us – but will also be an opportunity to ask any questions and offer any comments to the operations of these facilities. Again, Welcome. Welcome New Partners in Caring

I am pleased to announce in this issue of the Extended Care Chronicles three new facilities that

In honor of National Rehabilitation Awareness Week the staff at Dyer recognized the caring and compassionate rehabilitation team that spend their days improving the lives of others both physically and mentally. Our comprehensive rehabilitation team consists of Physical Therapists, Occupational Therapists and Speech Therapists who offer a “patient centered” approach to care.

The therapy team is led by Amy Eagens, OTR/L. Amy is our Program Manager. Amy is an Occupational Therapist by discipline and specializes in Geriatrics where patient care and therapy outcomes are her passion. She has been an OT for 10 years, mostly in skilled nursing facilities with some experience in the hospital setting. She has been the Manager of Therapy at this facility since September of 2014 and has assisted with the development of the current outstanding therapy department!

“Our entire Rehab team provides help and hope to our Residents daily and exude compassion and dedication at Dyer Nursing and Rehab. We could not ask for a better team” says Kristina Herrera, Administrator at Dyer Nursing and Rehab and Sheffield Manor.

Gladys Cherry has been the voice and face of Prairie Manor for the last 28 years. Gladys started as the receptionist in 1990 and at 84 years young, has no sign of slowing down. She is loved by the families and residents alike and knows how to make everyone feel at home when they come through the door.

One can catch Gladys happily singing a tune in the lobby and transferring calls with a song. Upon entering Prairie Manor one can clearly see how much she loves her job. Gladys goes above and beyond the call of duty for the residents, families and community members that enter the building.

Over the years, Gladys has made many contacts with the local hospital social workers and case managers. She receives calls from time to time from them just to hear her voice and to catch up.

Gladys states that coming to work every day and seeing the residents and families makes her happy. She feels when families come in and see her sitting at the desk it gives them hope for their loved one.

Countless individuals over the years have been touched by Gladys’ kind words and radiant personality. Thank you Gladys for all you do!

ECC Indiana Teams Raise the Bar to support the Alzheimer’s Association!

Our NWI teams, Munster Med-Inn, Sebos, and Spring Mill Health Campus came together for the Walk! Munster Med-Inn raised the bar in all of NWI raising $15,000 dollars towards the Cause! Team building activities for co-workers are critically

important to help ensure success in any business. And,

forming personal bonds helps us all succeed. Several

of South Suburban’s nurses and CNAs recently took

part in a few fun team building activities, run by Helen

Friedman, VP of Business Development. Not only did

everyone have fun, but they came out, realizing that we

all have common goals, in caring for our residents and

that the ONLY way to SUCCEED is to work cohesively as

a team, to help us overcome any challenges that may

come our way!
