The Care Management Team received this feedback from a providerregarding a patient who attended one of our diabetic education classes: “I am writing to thank you for the education and support that you have given to my patient. She has always said she understood her diabetes. However her A1C has always been very uncontrolled. At her last appointment, shebrought in her home glucose record which indicated her sugars were muchcloser to being controlled. She had been at an A1c of 8.6 with an average sugar of 206 and her sugars are now at 94-170. Even more important than that, she is really enjoying the class, says she is learning a lot, and feels positive about being able to control her own health. This is new for her. It will not only impact her health, but also the mother she cares for who is also diabetic. Thank you for the work you do with our patients.”

An ACO is agroup of doctors, hospitals, and/or other health careproviders working together with Medicare to give youbetter, more coordinated service and health care. We share important information and resources about yourindividual needs and preferences.

This information from other health care providers will give North Bend Medical Center and other health

care providers in ACOs a more complete and up-to-datepicture of your health. Over time, you may notice that you don’t have to fill out as many medical forms that ask for the same information, you don’t need to repeat medicaltests because your results are shared among your healthteam, and other benefits because your providers arecommunicating with one another. If you choose to let Medicare share your health careinformation with CHACO 17, it may also be shared withother ACOs in which your other doctors or health careproviders participate. If you don’t want your health careinformation shared, you can ask Medicare not share it.

PCPCH Stands for Patient Centered Primary Care Home. All of the North Bend Medical Center clinics are a certifiedPCPCH at either a tier 3 or tier 4 depending on whichlocation. The goal of the PCPCH program is to put thepatient at the center of their care and have services andresources available to help meet your needs and ensure thatyou are receiving the best possible care we can provide.

THere are six sTaNdards OF Care THaT eaCH CLiNiC is HeLd TO:

• accessible: Care is available when patients need it.

• accountable: Clinics take responsibility for theircommunity and provide quality care.

• Comprehensive: Patients get the care, informationand services they need to stay healthy.

• Continuous: Providers know their patients and work with them to improve their health over time.

• Coordinated: Clinics help patients navigate the health care system to get the care they need in a safe and timely way.

• Patient & Family Centered: Patients and familiesare the most important part of health care. Care

should draw on a patient’s strengths to set goals, and communication should be culturally competentand understandable for all.

In December we successfully renewed our PCPCH status forthe Bandon, Coquille, Gold Beach and Myrtle Point clinics.Below are the scores for both 2017 and 2018 so you can see the progress the clinics have made. To gain these stepsthere are multiple services the clinics must provide andbenchmarks they need to meet. There are five available tierswith Tier 5 being the highest.
