GUIDELINES FOR THE M. THOMAS MOORE FAMILY SCHOLARSHIP AWARDS FOR SECONDARY SCHOOLS History In 2015, Beatrice L. Moore and her daughters Teresa, Stephanie, Jennifer and Elisabeth, in a generous act of philanthropy, expanded their existing endowed scholarship fund at the Catholic Community Foundation to include the awarding of scholarships to students attending Elyria Catholic High School and Cleveland Central Catholic High School. Purpose To provide scholarships to students in financial need attending Elyria Catholic High School and Cleveland Central Catholic High School in the Diocese of Cleveland. Awards In the inaugural year (2016-17), $2,000 from the M. Thomas Moore Family Scholarship Fund shall be used to fund (1) one thousand dollar ($1,000.00) scholarship for a student enrolled at Elyria Catholic High School and (1) one thousand dollar ($1,000.00) scholarship for a student enrolled at Cleveland Central Catholic High School. In each successive school year until 2018, each school will receive an additional $1,000 scholarship from the Fund until fully funded with three (3) one thousand dollar ($1,000.00) scholarships at each of the two schools for the 2018-19 school year. Eligible In the inaugural year (2016-17), Elyria Catholic and Cleveland Central Catholic High School students currently in grade 9 who will be entering grade 10 may apply for the $1,000 scholarship, which may be renewable for their junior and senior years provided they continue to satisfy the scholarship criteria. In each successive school year until 2018, each school may select another sophomore to receive a scholarship until there are three Moore Scholars at each school in the 2018-19 school year. Criteria Financial The scholarship is required in order to attend the school The student’s family or relative(s) will provide a meaningful contribution to the tuition in relation to their resources The family must have completed/submitted a PSAS application. Merit As recommended by the secondary school principal and the Office of Catechetical Formation and Education based on such factors as: Scholastic achievement in accordance with the student’s potential Demonstrated character and diligence Service to the community Ambition and leadership potential Letter The parent/guardian and/or the student are required to write a letter (one page maximum length) stating why he/she believes the student fulfills the eligibility requirements.

Selections Each principal at the eligible schools is asked to review his/her student population for potential candidates for the scholarship. Candidates are to be selected based upon the criteria stated above.

A Diocese of Cleveland application is to be given to the candidates selected . Each secondary principal may select one candidate for each available scholarship. Example: For the 2016-2017 school year there is (one) $1,000 scholarship to be awarded. Therefore, each principal may select one candidate. Review The Grants Administration Office of the Office of Catechetical Formation and

Education will review the application materials of the selected candidates for completeness. In the event that the application materials of a candidate do not satisfy the criteria, the school will be asked to submit another candidate.

Awards The DTA team of the Office of Catechetical Formation and Education will have

final approval of the recipients. The M. Thomas Moore Family Scholarship Fund Committee will be notified of the scholarship recipients by the Grants Administration Office of the Office of Catechetical Formation and Education.

Deadlines March 1 - Parents/Guardians must return their completed applications with their

letters and a copy of their PSAS application, to their Catholic secondary school principal by March 1.

March 15 - Principals will send their selected nominations to the Grants Administration Office of the Office of Catechetical Formation and Education by March 15.

April - The DTA team from the Office of Catechetical Formation and Education, will select the scholarship recipients.

May - The M. Thomas Moore Family Scholarship Fund Committee will be notified of the scholarship recipients.

Notification In May, the Grants Administration Office of the Office of Catechetical Formation

and Education will send written notification to each of the scholarship recipients with a copy to the respective Catholic secondary school principal.
