2.0 Introduction This guide is designed to assist all applicants in completing Customer Services related forms. The content

has been developed on a non-technical level and easy to understand for any person without an electricalbackground.

This guide will assist applicants but not replace Customer Service staff and their knowledge. The guideshall speed up the applicant process, reduce incorrect information and ensure all correct documentationis submitted timeously.

The form will be available online or at any customer service centre.

4.0 Definitions and AbbreviationsApplicant Any person whom completes and submits a form. This person maybe but not limited to an

electrical contractor, developer, business owner, property owner or any person duly authorisedand mandated to act on behalf of represented persons.

Form A form may refer to but is not limited to an application form, meter card, CoC and othersupporting documents.

5.0 General Information5.1 Read all instructions and requirements carefully.5.2 Use a black pen only. Do not use a pencil, red or green pen.5.3 Complete all required fields in block letters (capital letters).5.4 All supporting documentation must be certified as original and not older than three months.5.5 Any incorrect information can lead to your application being delayed.5.6 Any mistakes must be struck out neatly and initialled or complete a new form.5.7 Fields with options shall be marked with an 'X'.5.8 If you are unsure of any fields or require advise, contact any Customer Services branch for further assistance.

6.0 Application for New / Changes to Electrical Connection

6.1 WHEN SHOULD I COMPLETE THIS FORM?6.1.1 For a completely new development i.e. no existing electrical infrastructure is at the property.

Building supplies for construction sites shall also use this application (The site will be metered).6.1.2 Application for new temporary supply (e.g. show, festival, religious, sport events). This connection

is only permissible for a maximum of fourteen (14) days only and is unmetered. You will be chargedfor estimated consumption before the event.

6.1.3 Changes to existing electrical connection e.g. changing from credit to prepaid, changing meterposition, pole, or connection type (e.g. overhead to underground vice versa).

6.1.4 Power enquiry (quotation for new or changes to electrical connection)6.1.5 Reinstatement of electrical connection e.g. connection may have been stolen, connection removed

due to non- payment.6.1.6 For Lighting applications, the applicant should advise Customer Services on quantity and wattage

of lamps used. Where low wattage consumption is required, consumer to provide actual currentrequirement.

6.2 WHAT SUPPORTING DOCUMENTS ARE REQUIRED?We have six different types of applications and supporting documents vary per type of application.

Business RegistrationDepending on the type of business, the relevant proof of registration documents need to be provided.

6.3 HOW DO I COMPLETE THE FORM?6.3.1 Section 1: Particulars of Owner

Only the property owner is authorised to make changes to their electrical connection. If land isbeing developed and has no owner, the developer must write their details.

How to get GPS Co-Ordinates using Google Maps:1. Go to maps.google.co.za2. Search for your address in the search bar (e.g. 1 Jelf Taylor Crescent, Durban)3. Zoom in/Pan to the correct location4. 6.3.3 Section 3: Electrical RepresentativeThis refers to an Electrical Contractor or Consultant. An Electrical Contractor as defined by theECASA constitution: “a person who is currently registered with the Electrical Contracting Boardof South Africa as an electrical contractor and who undertakes to perform electrical installationwork and/or the verification and certification of the construction, testing and inspection of electricalinstallations on behalf of any other person, but excluding an employee of such first-mentionedperson.”

It's up to you the customer (applicant) to ensure the electrical contractor you use is registered withthe relevant authorities and adheres to SANS and other safety regulations. Their registration withthe authorities shall be valid at the time of installation. EThekwini Electricity does not screen,regulate or suggest any panel of electrical contractors.

6.3.4 Section 4: Connection Type

6.3.7 Section 7: Date Supply RequiredThis is the date you require the electrical connection to be installed. For installations that arerequired as soon as possible, write the current date of application. Use the date format YYYYMMDD(e.g. 11 August 2014 will be 20140811).

6.3.5 Section 5: Meter Type

6.3.9 Section 9: Requested DemandResidential customers shall ignore this field.This field is for large power users such as businesses. A kVA is 1,000 Volt-Amperes referred toas apparent power. This field is best suited for an electrical consultant/ contractor to calculateand advise a customer.

6.3.12 Section 12: No. of PhasesThe electrical supply can either have single or three phase. Your electrical contractor or CustomerServices staff will be best suited to advise you on the number of phases you require.Single phase will only have one live wire, neutral and earth. Three phase has three individual livewires, neutral and earth (total of five wires).

Regular residential houses will use single phase. Three phase will be better suited for larger houseswith swimming pools, central air-conditioning and multiple rooms etc. Businesses with large loadssuch as motors, heaters, lighting, cooling should use three phase. The cost for three phase issignificantly more and thus a thorough analysis should be conducted before selecting three phase.

7.0 Meter Card7.1 What is a meter card?

A meter card is proof and pre-compliance (some electrical tests done) that the affected propertyis ready for EE staff to perform changes or install a new electrical connection.

7.2 When do I complete and submit the meter card?The meter card shall be submitted when the premises is ready for the connection / changes tobe completed by our technical staff. Premature submission (i.e. site does not meet EE standards)of the meter card will result in additional charges and delay the completion of the connection.

Failure to submit the card and incorrect information can lead to the job being delayed. 7.3 Who completes the meter card?

The meter card may only be completed by an electrical contractor. Any person (applicant or electrical contractor) may physically submit the card to Customer Services.

8.0 Certificate of Compliance8.1 What is a Certificate of Compliance (CoC)?

An Electrical Certificate of Compliance is a certificate issued by a registered person whom certifiesand deems your electrical connection safe according to minimum standards set out in legislation.

8.2 When shall a CoC be submitted?The CoC shall be submitted no later than 48 hours (two working days) after EE staff have

completed the installation (new, changes or reinstatement to connection). 8.3 Who completes the CoC?

Only a registered person that is in possession of a valid licence may issue a CoC. The personmust be registered with the Department of Labour.

6.3.15 Section 15: Terms and ConditionsThese terms and conditions need to be accepted and signed irrespective of the type of application.It must also be dated using YYYYMMDD format (e.g. 11 August 2014 will be 20140811).
