I. Background:

Although HIV/AIDS pandemic is a global disaster, countries in Sub-Saharan Africa and the Great Lakes Countries in particular have beendisproportionately burdened in various ways. In the last decade, countriesin the Great Lakes Region were considered the epicenter of the epidemicbecause of the high prevalence rates and the socio-economic impact onthe populations. During this period the Great Lakes Region also facedunbearable years of turmoil, war and armed conflict across borders. Therewas mass movement of armies, displacement and migration of entirecommunities from one country to another despite the high prevalence ofMM/AIDS.

Inspite of this, various countries have put in place programs andinterventions to fight HIV/AIDS at various levels. It is notable that althoughthe trends of HIV/AIDS are on the increase in Eastern Democratic Republicof Congo, declining trends in Uganda has shown that the spread ofHIV/AfDS can be controlled.

With the strengthening of regional cooperation, there is increasing ease ofcontinuous migration of people from country to country for social,economic, cultural and other reasons. This prompted the leaders in thesecountries to set up a regional initiative to strengthen collaboration in thefight against HIV/AIDS.

II. What is GLIA?

GLIA stands for the Great Lakes Initiative on AIDS. In August 1997,leaders in Burundi, Democratic Republic of Congo, Rwanda, Uganda,Tanzania, and WHO/AFRO signed a protocol of cooperation on Preventionand control of epidemics including Polio eradication in the Great LakesRegion.

A consultative meeting was held in Kampala in March 1998, attended bythe National Program Managers of AIDS Control Programs andrepresentatives of UNAIDS Theme Group from Burundi, DRC, Kenya,Rwanda, Uganda and Tanzania. It was resolved in this meeting to createthe Great Lakes Initiative on HIV/AIDS (GLIA) in the spirit of protocol ofcooperation in the region. Subsequently GLIA was officially launched on27/April/1999 in Kigali, Rwanda in the presence of the Executive DirectorUNAIDS, Peter PIOT.

Mission statement of the GLIA

The mission of the QUA is to contribute to the reduction of the HIVinfection and to mitigate the socio-economic impact of the epidemicin the Great Lakes Region by developing regional collaboration andimplementing interventions that can add value to the efforts of eachindividual country.

III. Objectives of GLIA:

-" Strengthen collaboration and co-operation between the GLIA countriesand Agencies in the GL region (GLR) for a strong and co-ordinatedresponse;

- Promote dialogue and sharing of experience between the GLIAcountries;

- Support innovative and catalytic projects responding to priority needsdefined by the GLIA;

- In the spirit of IPAA, encourage all partners , especially the PLWHA, theprivate sector, NGOs and communities, UN agencies and donors, toparticipate to an expanded and co-ordinated response in the GLR;

- Strengthen regional co-ordination to ensure better use of resources andto mobilize additional financial resources;

- Establish a regional forum to share research findings and bestpractices.

IV. Structure and Operations of GLIA

The Governing Board of GLIA, which is composed of Ministers in charge ofHIV/AIDS interventions, determines the political direction and policies to beimplemented.

GLIA technical committee is composed of managers of the National AIDSControl Programs, UNAIDS Country Program Advisors of GLIA countries,UNAIDS-ICT Abidjan and Pretoria. The GLIA coordinnator is also amember of the technical committee.

The Executive Secretary is the head of GLIA secretariat and is assisted bydeputy coordinator, in charge of the daily follow-up of the plan of action anda bilingual Administrative Secretary and other support staff. Although thesecretariat is based in Kigali, it was recently made independent of theMinistry of Health and the local AIDS Control Program.

The GLIA budget is part of the UNDP Trust Fund in the country hosting thesecretariat and all expenditures will be in accordance with the work planapproved by the Technical Committee. The secretariat shall operate inclose consultation with the chair of the Technical Committee inmanagement of the budgetary funds.

V. Progress made so far

Since the establishment of the initiative , some progress towards fulloperations of GLIA has been made namely:

1. GLIA Secretariat has been established in Kigali, Rwanda and iscurrently implementing some activities according to the initial 3 yearwork-plan;

2. An additional staff was appointed under a contract from UNDP to assistthe AIDS Control Program manager, also GLIA coordinator . Mr Jean-Baptiste GATALI has been ensuring close follow-up of theimplementation of the plan of action

3. Truck drivers and sex workers HIV/AIDS campaign program has beeninitiated in some of the countries in collaboration with NGOs and privatesector and other countries have already designed their programs. InMay 2001, a technical consultation is going to harmonize programs andapproaches used by the different implementing agencies.

4. A review and consultative meeting attended by GLIA representativesfrom the member countries was conducted in Nairobi in October 2000.This meeting also discussed GLIA operational framework.

5. Rwanda government has recently appointed an Executive Secretaryand provided autonomy to GLIA as part of strengthening the operationsof this initiative. In undertaking its activities GLIA has receivedsubstantial support from the UNAIDS Secretary , Geneva and theUNAIDS office in Rwanda.

6. A two-year plan of action has just been elaborated and is being agreedupon by member countries

VI. Challenges / constraints

Making sure that every GLIA activity is adding value to what eachindividual country is doing ; this will be the main challenge during the twocoming years .A resource mobilization strategy will be elaborated. The support from theUNAIDS Theme Group will be crucial to make the Secretariat fullyoperational.The UNAIDS Theme group in GLIA countries will be mobilized to supportthe GLIA activities in countries while regional contributions will bemobilized to ensure the implementation of inter-country initiatives.A GLIA plan of action is attached to this document, together with the GLIASecretariat needed budget.It is expected that by end of "2003, GLIA countries will have started tocontribute financially to the running costs of the Secretariat.

VII. Future perspectives

A two-year plan of action is attached to this document. The main focus ofthe plan will be :

1. Establishment of mechanisms for efficient communication and regionalnetworking;

2. Consolidation and scaling up of the road axis initiatives, also taking intoaccount other entry points

3. Intensive Advocacy for political will and support from the Governmentsin the member countries as well as increased participation andcontributions of technical and financial resources to the implementationof programmes of GLIA;

4. Establishment of collaboration with other regional bodies and regionaloffices of partner organizations to increase coordination and realizesynergy

5. Combined efforts for better accessibility to drugs in the region6. Harmonisation of protocols where needed7. Innovative approaches to prevent HIV/AIDS among displaced persons8. Operationnal research on cross-boarder issues9. Resource mobilisation for management, operations and implementation

of GLIA programs from donors and other development partners.
