for the product that you are selling.* once you have selected an advertising piece(s) check the GASBG Agent Advertising Approvals chart (also found on AgentView, under Business Building > Creating Ads) to ensure that what you have chosen is available in your state and whether it is state specific or generic.

2 . once you have verified that the piece(s) is available in your state please fill out and submit the Pre-Approved Advertising request form (GASBG-9-0051-FORM, also found on Agentview under Business Building > Creating Ads) and a member of the Marketing/communications (Mktg/comm) staff will contact you.

Mktg/comm will prepare the advertising piece you select to include your personal information and then will submit it to our compliance Department.

3. After the piece gets the final approval from compliance, Mktg/comm will email you the final piece with the approval to use it.

You can forward the artwork to a printer (if applicable) or Mktg/comm can work with your printer to get the appropriate files to him. GASBG does not cover any printing costs and all print liabilities for the final printed piece belong to the agent. Please allow 5 days to process your request.

All brochures can be ordered from our Supply Department via email at [email protected] or by using the Supply order form. Most brochures are state specific, log on to Agentview and look under ‘Products’ to see what version is approved in your state.

POLICY PROVIDES LIMITED BENEFITS.The purpose of the marketing material is solicitation of insurance and that contact will be made by an insurance producer or insurance company.

THIS IS A LIMITED BENEFIT - CANCER ONLY POLICYRENEWABILITY CONDITIONS: The policy is guaranteed renewable. Premium rates may be changed on a class or state basis.

CANCELLATION: You may cancel this policy at any time by written notice delivered or mailed to us, prior to its renewal date or expiration date. We shall refund to you the pro-rata portion of such premiums paid for any period beyond the end of the policy month in which the cancellation occurred. Cancellation shall be without prejudice to any claim originating prior to the effective date of cancellation.

If this is a Single Parent plan or an Individual plan, upon the death of the Insured or the payment of the First Diagnosis Cancer Benefit Amount for the insured, the coverage ceases. We will refund the pro-rata portion of any premium paid. If this is a Family Plan, upon the death of the Insured or the payment of the First Diagnosis Cancer Benefit Amount for the Insured, the coverage will be changed to a Single Parent Plan. The Insured’s spouse will become the Insured.

POLICY LIMITATIONS AND EXCLUSIONS: This policy provides benefits only for first diagnosis of internal cancer or malignant melanoma. This policy does not cover any other disease or sickness or incapacity. No benefit is payable for the first diagnosis of skin cancer other than malignant melanoma. The First Diagnosis Cancer Benefit Policy and the First Diagnosis Heart Attack and First Major Heart Surgery Rider contain a 30‡-day “waiting period” which means that no benefits are available until the policy has been in force at least 30‡ days from the “effective date” shown in the policy schedule. Cancer and Heart Attack diagnosed within the 30‡-day “waiting period” will not be covered, and First Major Heart Surgery performed within the 30‡-day “waiting period” will not be covered.

Benefit amounts will not be paid for any diagnosis resulting from a pre-existing condition. A PRE-EXISTING CONDITION means a sickness or injury which existed prior to the Effective Date of the policy.

10-DAY‡‡ RIGHT TO EXAMINE POLICY: you have 10 days to review the policy after you receive it. If for any reason you are not satisfied, you may return it to us for a full refund of the premium you paid.

‡There is no waiting period in Indiana, Missouri and South Dakota. In Kansas the waiting period will be waived if the policy is replacing another specified disease policy and the waiting period has been satisfied.‡‡There is a 30-day Right to Examine Policy in WY.
