Purposive: Inclusion Criteria.
DATA GATHERING
A. Those nurses who passed the return demonstration(with matrix skills as basis) were observed FOUR TIMES B. Charting time and accuracy were recordedC. Modified questionnaires were given after the observation

CONCLUSION The new innovation has proved valuable in enhancing
patient management, facilitating report generation, thereby improving efficiency, quality and accuracy of nurses’ documentation which is essential to competent and optimum patient care.

1. Hospital Administrators of the selected hospitals.
2. Members of the healthcare team, including the clinical instructors and students affiliated in the hospital must be well-educated and oriented about the new innovation as means of vital signs documentation.
3. A bigger study sample is needed for another phase of clinical trial.
4. A qualitative study using open-ended questionnaires or interviews are recommended.
5. Make the application available to other operating systems such as iOs (Apple), Blackberry OS, and Windows Phone OS.
