This offer of appointment will be subject to the following condit ions ­

1. Status of the Post:

a) The post against which helshe is offered appointment is temporary , but likely to continue.

b) This appointment is provisional and is subject to the Caste I Tribe Certificate being verified through the proper channels and if the verification reveals that the claim to belonging to Scheduled Caste (SC), as the case may be, is false or not covered under the ~.ules applicable, the services will be terminated forthwith without assigning any further reasons and without prejudice to such further action as may be taken under the provisions of Indian Penal Code for production of false certificate.

c) Hislher services will be terminated at any time by giving 01 (one) month 's notice by either side i.e . the appointee or the appointing authority , without assigning any reasons whatsoever It will be open to the Administration to pay salary in lieu of notice period falling short of the stipulated one month Similarly, if helshe wishes to resign from the post , helshe may do so by depositing pay and allowances in lieu of the notice period by which it falls short of one month alongwith hislher resignation notice , failing which an interest @ 18% p.a. will be charged w.e.f. the date of submission of resignation notice.

... 2 ... d) His/her services will, however, be terminated by the Government without

prior notice at any time during the service, in case of following circumstances ­

" i) If it is satisfied on medical evidence that he/she is unfit and is likely for a considerable period to continue unfit for reasons of ill health for the discharge of his/her duties, provided that the decision of the Government that he/she is likely to continue unfit , shall be conclusive and binding on him.

ii) If it is satisfied on verification that his/her character and antecedents is such that he/she is not fit for being retained in Government Service

iii) If any of the declarations or information furnished him/her at the time of applying or joining is proved to be false or if it is found at any later stage that he/she has willfully suppressed any material information

iv) If it is found that he/she has mis-conducted.

2. Probation:

He/She will be on probation for a period of 02 (two) years , which can be extended under the Service Rules.

3. Rules, Regulations and other Service conditions:

He/She will be governed by the Punjab Civil Services Rules/Regulations, as applicable to the employees of the Union Territory, Chandigarh in force from time to time.

4. Pay:

He/She will be entitled to get pay in the pay scale of Rs 10300-34800 + Rs.4600/-+ such allowances sanctioned by the Chandigarh Administration from time to time.

5. leave/lTC:

He/She will be entitled to get the above said benefits in accordance with the Punjab Civil Services Rules , as applicable to the employees of Union Territory, Chandigarh in force from time to time

6. CPF and Pension:

He/She will be governed by the I\lew Defined Contributory Pension Scheme issued by the State of Punjab for its employees, as applicable to the employees of the Union Territory, Chandigarh .

7. Medical Reimbursement:

He/She will be allowed reimbursement of Indoor Medical Treatment in the Government/Government authorized hospitals/institutions, subject to completion of all other formalities, as admissible under the Punjab Medical Attendance Rules, 1994

8. Duties and responsibilities:

Duties and responsibilities of the Staff Nurse will be fixed by the Director Principal from time to time. He/she will be required to perform such work as may be needed in the legitimate interest of patient care in the hospital and exigencies of servi ces .

9. Accommodation :

This is to made clear that the above offer of appointment will be subject to the condition that the "Rent free accommodation to the staff nurses , if available , is mandatory with free electricity and water charges within the prescribed limit , as has been fixed by the Chandigarh Administration from time to time and the house rent wil l be admissible only, if it is certified by the Chief Warden (Hostels) that the accommodation in the hostel of th is institute is not available"

10. Discipline and conduct:

He/She will have to conform to the Rules , Regulations , discipline and conduct prevalent in this institute and those imposed by the institute on all the employees from time to time.

11 . Marriage Certificate:

This offer of appointment IS subject to the condition that he/she is having only one living spouse. However, in case he/she is having more than one living spouse , he/she will submit a declaration of his/her having more than one living spouse and that his/her second marriage is void by reason of its taking place during the life time of such living spouse. The affidavit should be duly attested by the Executive Magistrate only .

12. Medical Examination:

This offer is subject to his/her being medically fit to jo in the Government Service by the Medical Board duly constituted by the Principal Medical Officer, Government Super-specialty Hospital , Sector 16, Chandigarh . In support of which he/she must subm it the medical certificate issued by the standing Medical Board , so as to enable him/her to join the post.

13. Collection of Medical Examination Papers:

He/She should manage the Medical Examination papers from the market or vendors/shopkeepers near the Estate Office, UT, Sector 17, Chandigarh at his/her own level and thereafter submit the same duly filled in , to this office alongwith 04 (four) unattested photographs for onward submission of the same to the Principal Medical Officer, Government Multi-specialty Hospital , Sector 16, Chandigarh. However, he/she must ensure that he/she has appeared before the Principal Medical Officer, Government Multi-specialty Hospital , Sector 16, Chandigarh, well in advance before the day of medical examination, so as to complete the formalities required for medical examination.

14. Verification of GNM/BSc./MSc. and other professional qualification will be got verified by the Institute at the expenses of the candidate .

, In case, the offer of appointment on the above said terms & conditions is

acceptable , he/she should report alongwith the medical examination papers in the Establishment Branch-III , Level III , Block '0', Hospital Building , Govt. Medical College & Hospital, Sector 32, Chandigarh . However, he/she is required to join his/her duties within 20 (twenty days) from the date of issue of this appointment order. If he/she fails to report within the stipulated period, it will be presumed that he/she is not interested in offered appointment and the offer will stand automatically cancelled .

At the time of joining , he/she must bring all original documents/certificates, 04 (four) latest passport size photographs, 02 (two) ticket size photographs and one blank Service Book, failing which he/she will not be allowed to join "
