Wie stellen wir eine Glaukomprogression fest ?
5
Affiliated Academic Hospital of the Faculty of Medicine at the University of Zurich

Publikumsfrage 1
Besteht eine funktioneller Progression?
A: Nein, beide Augen stabil
B: Ja, beide Augen schlechter
C: Ja, nur linkes Auge schlechter
D: Ja, beide Augen schlechter

«Although OCT has a valuable role in assessing glaucoma progression, visual field testing remains the primary method of assessing glaucomatous damage»
«..it is the consensus that both structural and functional tests should be monitored with equal diligence for optimal assessment of glaucoma progression»
«Although visual function is what matters most to patients, progressive structural changes can precede functional loss, and patients with faster changes on OCT are at increased risk of worsening visual losses, offering the possibility of escalating treatment at an earlier stage to preserve vision better»

«Particular, more recent studies have shown that harnessing and understanding of structure-function relationships can improve glaucoma diagnosis and detection of worsening»

RGC in early GlaucomaMean RGC Loss 28.4% (95% C.I. 24.9-31.9%; 6-57%)
F-U 6.7 (4.4 – 13.3 years)

«Nevertheless, despite current enthusiasm about improved structural testing and criticism of the current SAP paradigm, the importance of visual field testing should not be underestimated»
«Although structural imaging has improved, it is not yet ready to replace functional testing in glaucoma management»
«Visual field testing is the gold standard for the diagnosis and management of glaucoma, and as such will be difficult to replace»

«In conclusion, ophthalmic imaging innovations have revolutionized glaucoma diagnosis and management, and the tools, algorithms, and biomarkers that are relevant to clinical practice are evolving constantly»

Take home message

A: Nein, beide Augen stabil
