If you follow news headlines about healthcare issues, you know there’s a growing interestacross state governments to engage in policy that ultimately increases awareness of theimportance of long term care planning. As the growing U.S. population ages into SocialSecurity and Medicare, the burden of long term care planning is up to the individual. Andthe priority health issue among older Americans is the cost of care.

Genworth Financial commissions an annual survey because it helps us keep an eye onrising costs in different markets and how that affects long term care issues. We also sharethe survey results because helping people like you understand your options and preparefor uncertainty in a world of shifting responsibilities is at the heart of what we do atGenworth Financial.

This survey is the most comprehensive of its kind in the industry and includes data pointsfrom over 10,000 nursing homes, assisted living facilities, and home care providers acrossthe country. Financial professionals and consumers rely on these survey results each yearas they plan for long term care needs.

This year, we took advantage of four years of consistent data collection and methodologyto identify trends across the care types and regions. Our findings were enlightening.Overall, we found the cost of care within facility-based providers steadily increased overthe four-year time frame, yet home care costs remained relatively flat with a compoundannual inflation rate of just 2.74%. We believe this relative stability represents an increasein demand for these services and the proliferation of providers, which allow older Americansto receive care in the comfort of their own home. We share some of the results of the four-year data trends in a new section in this year’s survey: Historical Trends and Findings.

While the increases in the costs of home care services have remained stable over the lastfour years, the overall costs for these services as well as facility-based care remain high.That is why we encourage you to review the results of our study and understand the ratesby region and type of care. It’s not a surprise that the cost of care remains high, but wehope this tool helps you make informed decisions when planning for those costs today andin the future.

This raterepresents a 16.2% decline comparedwith the Genworth 2006 survey results.Viewed historically over four years, thecost of this service has remained relativelyflat as evidenced by a 0.3% compoundannual inflation rate from 2004 to 2007.

Homemaker Services

• Non-certified but Licensed Provider Rate.Nationally, the average hourly private payrate charged by a non-certified but licensedhome care provider for homemakerservices is $17.46/hr. This rate representsa 1.5% decline compared with Genworth2006 survey results. Viewed historicallyover four years, the cost of this servicehas remained relatively flat as evidencedby a 1.6% compound annual inflation ratefrom 2004 to 2007.

Assisted Living Facilities

Assisted living facilities (ALFs) weresurveyed to determine the private-pay ratesfor one-bedroom units. All ALFs surveyedwere licensed by the state as assisted livingfacilities or other comparable residentialcare facilities.• Nationally, the average monthly cost for a

one-bedroom unit is $2,714.38 (implying adaily rate of $89.24/day), reflecting anaverage annual cost of $32,572.56. Theaverage monthly cost increased about 1%compared to the 2006 average monthlycost ($2,691.20). These rates exclude anyone-time community or entrance fees.

E X E C U T I V E S U M M A R Y

Genworth Financial’s current cost of caresurvey is based on information gathered byCareScout in January and February 2007.Telephone interviews were conducted in 90separate regions1 across the U.S., with morethan 10,000 home health, assisted living, andnursing home providers completing surveys.

Home Care

Home care providers were classified intotwo types according to their certification andlicensure status. The two provider types are:

• Medicare certified home care providers:these providers are also licensed.

• Non-certified but licensed home careproviders.

Researchers surveyed certified home careproviders to determine the hourly privatepay rates charged by such providers for theservices of a “home health aide.”Researchers surveyed non-certified butlicensed home care providers to determinethe hourly private pay rates charged by suchproviders for the services of a “home healthaide” and for the services of a “homemaker.”

Home Health Aide Services

• Certified Home Care Provider Rate.Nationally, the average hourly private payrate charged by a certified home careagency for home health aide services(including rates associated with thedelivery of skilled care) is $32.37. This raterepresents a 10.6% decline comparedwith the Genworth 2006 survey results.Viewed historically over three2 years, thecost of this service has increased yearlyas evidenced by a 2.25% compoundannual inflation rate from 2005 to 2007.

• Non-certified but Licensed Provider Rate.Nationally, the average hourly private pay

• Viewed historically over four years, thecost of a one-bedroom unit in an assistedliving facility has increased steadily yearupon year, as evidenced by a 4.2%compound annual inflation rate from 2004to 2007.

• Boston, Massachusetts ($57,042 excludingcommunity fees) and the non-urban areasof New Jersey ($56,801 excludingcommunity fees) have the highest annualcosts for one-bedroom units in ALFs.

• North Dakota and Arkansas have thelowest annual cost for one-bedroom unitsin ALFs (approximately $20,000 excludingcommunity fees).

• The 2007 Genworth survey shows that,on average, the cost of care for a one-bedroom unit in urban areas acrossAmerica was 14.7% greater than in non-urban areas. The areas with the greatestdisparity in ALF rates between urban andnon-urban regions are New York, New York(53%), Austin, Texas (43%), andPhiladelphia, Pennsylvania (42%).

• 35% of the ALFs surveyed charged a one-time fee, commonly referred to as acommunity or entrance fee, ranging from$25 to $120,000 with a national averageone-time fee of $1,621.08.

Nursing Homes

Medicare certified nursing homes weresurveyed to determine private pay rates forprivate and semi-private rooms.

• Nationally, the average annual cost for aprivate room (single occupant) is $74,806($204.95/day), reflecting a 5.5% increaseover the Genworth 2006 survey result($194.28/day).

• Viewed historically over four years, thecost of a private nursing home room hasincreased steadily year upon year, asevidenced by a 4.7% compound annualinflation rate from 2004 to 2007.

• Alaska ($196,735), New York City($135,853), and the non-urban areas ofConnecticut ($120,479) have the mostexpensive annual private room nursinghome rates in the country.

• Louisiana and Missouri’s non-urban areas(i.e., excluding St. Louis and Kansas City),had the least expensive annual nursinghome rates, costing about $44,000.

• Nationally, the average annual cost for asemi-private (double occupancy) room is$65,985 ($180.78/day), reflecting a 5.5%increase over the 2006 average($171.32/day).

• Viewed historically over four years, thecost of a semi-private nursing home roomhas increased steadily year upon year, asevidenced by a 4.6% compound annualinflation rate from 2004 to 2007.

• The 2007 Genworth survey shows that,on average, the cost of care for a privatenursing home room in urban areas acrossAmerica was 13% greater than in non-urban areas. The greatest disparity existsin New York, New York where the cost ofcare was more than 55% greater than thecost of care in New York’s non-urbanareas. Nursing home costs in Austin,Texas and San Francisco, California are48% and 44% greater, respectively, thanthe cost of care in Texas’ and California’snon-urban regions.

For the fourth consecutive year, Genworthhas surveyed long term care serviceproviders across the country within 90Regions3. Researchers collected datarequired to calculate and communicate theaverage cost of care charged by home care,assisted living and nursing home providersfor various types of long term care services.

At one time, hospitals and nursing homes(usually called ‘rest homes’) provided themajority of care for America’s frail andelderly. Over time, the ‘continuum of care’has been expanding. Nursing homes beganoffering ‘sub-acute’ services (for example,physical rehabilitation for stroke or hipreplacement surgery) to care for patientswho otherwise would have remained in thehospital for such care. Assisted livingfacilities, catering to the ‘private-pay’ marketand generally free of federal regulations,became an attractive alternative to nursinghomes for people making long term livingarrangements. For those who want toremain in their homes for as long aspossible, home health care agenciesprovide a host of services ranging fromskilled care (offered by nurses, home healthaides, and therapists) to non-medical,personal care services.

Another industry trend is the increase inadult day health facilities and programs.Adult day health care is becoming morepopular and more available. Because theseservices are offered, typically, within‘facilities’ / buildings (i.e., not in the home),the companies and the services beingprovided are easier for state legislatures todefine and regulate. Therefore, adult dayhealth care providers are usually licensed bytheir states.

Medicare and Medicaid pay for some longterm care services so long as skilled care isneeded by the patient. However, governmentprograms do not generally pay for non-medical, personal care services needed bysome people to remain safe in their homesfor as long as possible. Over the years, theGenworth surveys have collected informationpertaining to private-pay, not Medicare orMedicaid, rates.

Benefiting from four years worth of surveyresults, Genworth recognizes someemerging trends across the long term careservices landscape.

Overall, the cost of care within facility-basedproviders has steadily increased whilehome care costs have remained relativelyflat. (See Figures 1 and 2.) For example, in2004 the average annual rate for a privatenursing home room was $65,185 comparedwith the 2007 average annual rate of $74,806.Therefore, Americans can expect to payalmost $10,000 more per year today for anursing home than they had to pay in 2004.

Similarly, the monthly rate for a one-bedroom unit within a licensed assistedliving facility has increased almost 4.2% peryear, over four years.4 This does not includeany one-time community fees that havebeen charged by approximately one-third ofall assisted living facilities surveyed,according to the Genworth studies.

In contrast to facility-based care, ratescharged by home care providers haveremained relatively flat over four years. For example, whereas in 2004 the national,hourly private pay rate charged by a non-certified but licensed home health agencyfor a home health aide was $18.43, the2007 hourly rate was found to be $18.57.Despite a spike in these rates in 2006, thehistorical inflation rate has been 0.3% overtime. Among the many potential reasons forsuch a trend, one might assume that homecare rates have remained flat due toincreased competition among agencies,availability of unskilled labor, and becauseagencies can avoid, for the most part, thecosts associated with maintaining healthcare facilities.

CareScout, a Wellesley, Massachusettscompany that specializes in eldercare providerdatabases and helping families selectappropriate providers nationwide conductedthe survey for Genworth. CareScoutcontacted over 45,000 providers in order tocomplete over 10,000 surveys of nursinghomes, assisted living facilities, and homecare providers in all 50 states and the Districtof Columbia in January and February 2007.

Genworth’s study is believed to be the mostcomprehensive and in-depth survey of itskind. In terms of scope, the Genworthsurvey includes nursing homes, assistedliving facilities, and two types of home careproviders (“Medicare certified and licensed”providers and “non-certified but licensed”providers).

CareScout researchers conductedtelephonic interviews among a sample oflong term care providers within 90 regions.The 90 regions cover every state includingthe District of Columbia. In some statesmore than one region was surveyed toaccount for more heavily populated areas.The heavily populated regions correspondgeographically with their MetropolitanStatistical Areas as defined by the USCensus Bureau.

Home Care

Researchers polled, on average across bothcertified and non-certified provider types, atleast 13% of home care providers. Exceptin a small minority of cases, at least 10providers completed surveys. In manyareas, the sample size was significantlylarger. Researchers completed over 5,440surveys. All rates are weekday (Monday –Friday) rates.

CareScout separated home care providersinto two different categories based uponcertification status: Medicare certified homecare agencies and non-certified but licensedhome care agencies.

Unlike previous surveys, the 2007 surveyexcludes the non-certified and unlicensedhome care provider category. The amount ofcompleted surveys upon which to rely for astatistically significant survey of home healthaide and homemaker rates charged by non-certified and non-licensed home careproviders has dwindled to the point whereCareScout no longer surveys this category.

Despite this change, home care optionscontinue to expand to meet strong consumerdemand for care in the home. For example,informal caregiving is on the rise - but thistype of care is frequently provided by familyand friends, not by formal entities. The formalentities that do provide this type of care mustbe aware of licensing regulations within thestates. More and more states have laws thatenable providers to become “licensed” assome form of a home care provider whenproviding home health aide services. Aminority of states have licensure categoriesfor providers who offer homemaker services.

Because CareScout surveys agencies offeringthese two types of non-medical home care(i.e., home health aide and homemaker) thelicensure distinction is often lost. That is,agencies offering homemaker services maynot need to be licensed but they are alreadylicensed because they sought a license tooffer home health aides services. Therefore,while informal caregiving services provided byinformal entities are on the rise, non-medicalhome care services provided by formalentities tend to be provided under a “license.”

Assisted Living Facilities

Across 90 individual regions, researcherspolled approximately 11% of the licensedfacilities and in no region other thanDelaware did less than 10 facilities completethe survey. In many areas, the sample sizewas significantly larger. More than 4,200providers completed the survey.

Unlike nursing homes, there is no national,uniform regulatory protocol for assistedliving facilities. As a consequence, stateshave instituted licensing standards that varyfrom state to state. CareScout’s researchersinterviewed assisted living facilities whichwere licensed according to the licensurerequirements of the state in which theassisted living facility was located.

According to independent researchconducted by CareScout, there are morethan 70 different names or designations forfacilities licensed as some form of anassisted care facility. Generally, only about50% of these care facilities use the term“assisted living facility” as a part of theirformal name or licensure designation.Another popular name or designation is“residential care facility.”

The mechanics of, and provider reaction to,state regulations leads us to the conclusionto eliminate the non-certified, non-licensedcategory from the Genworth survey.However, that does not mean that thecontinuum of private pay long term careservices is shrinking. To the contrary, thiscontinuum is expanding on the non-medicalcare side. Increases in informal caredelivered by informal entities and adult dayhealth are good examples of the marketreacting to increasing demand.

For the 2007 Genworth survey, researcherssurveyed home health providers todetermine hourly private pay rates chargedfor the services of certified home healthaides and non-certified but licensed homehealth aides. Home health aides providepersonal care but not medical care. Forexample, a home health aide will typicallyhelp with bathing, dressing, transferring andtoileting but not with catheters or injections.

Also, researchers surveyed non-certified butlicensed providers to determine the hourlyprivate pay rates charged for the services ofa homemaker.5

In a minority of cases, where an agencycharges a “per-visit” rate researchersdetermined the number of estimated hoursper visit to calculate the hourly rate. Wherea rate range was provided, the midpointwas calculated and applied. The surveyexcludes weekend and holiday rates.

The survey evaluated the daily charge for aprivate room (single occupant) and a semi-private room (double occupancy) in Medicarecertified nursing facilities. Medicare certifiednursing homes represent more than 90% ofall nursing homes in the U.S. Rates fortransitional care units (typically hospitalbased certified nursing homes) were notincluded. The daily room charge usuallyincludes services beyond room and boardsuch as three meals a day, laundry, sundries,basic nurse supervision, and generic-brand,non-prescription pharmaceuticals.

To determine historical trends, CareScoutrelied upon Genworth survey results datingback to 2004 to calculate compound annualinflation rates.9

The survey evaluated the monthly privatepay rate for a one-bedroom unit in anassisted living facility. Researchers alsoprocured community / entrance fee data.Community and entrance fee rates are notincluded in the average monthly assistedliving rate calculations. The monthly ratetypically includes three meals per day andsometimes includes basic care-relatedsupervision. It does not typically includeadditional costs for care such as therapy,rehabilitation, or medication costs.

To determine historical trends, CareScoutrelied upon Genworth survey results datingback to 2004 to calculate compound annualinflation rates.7

Nursing Homes

Across 90 individual regions, researcherspolled at least 13% of all certified andlicensed nursing homes and in no region didless than 10% of providers complete thesurvey. In no region other than Alaska didless than 10 facilities complete the survey.In many areas, the sample size wassignificantly larger. More than 2,000providers completed the survey.8

Despite the steady home care rates overthe years, the level of complexity within thehome health care landscape is surprisinglygreat. Complexity arises for at least tworeasons: inconsistent licensing standards,and shifting rates.

Inconsistent Licensing Standards

Understanding the regulatory environmentin the home care industry remains acomplex issue for consumers of long termcare as well as providers. Each state canhave its own set of licensing requirements.On the positive side, most states do havesophisticated licensing regulations designedto protect patients and families. However,as a consequence of licensing requirementsvarying from state to state, standards ofcare across state lines are less easy to under-stand and predict than some consumersmight otherwise wish.

Factors Affecting Home Care Rates

Researchers found that many factors affectrates for home health care.

Time of Service Affects Rates. An agency maycharge different rates for similar servicesbased upon the ‘shift’ (day rate vs. nightrate), and whether services will be providedon a weekend or holiday.

Certification status impacts rates (Medicarecertified vs. non-certified providers). Therates charged by certified home healthagencies for the services of a home healthaide can be materially much moreexpensive than the rates charged by non-certified but licensed agencies.

While both certified and non-certifiedproviders may offer the services of a homehealth aide (as paid for by private funds –that is, not Medicare), the structure of theservice is fundamentally different andtherefore helps explain why, on average,certified agencies charge more than $30per hour whereas non-certified but licensedagencies charge about $20 per hour.

Certified home care providers usually offerthe services of a home health aide, if at all,on a per-visit basis in association with aplan of care initiated by a doctor’s order forthe delivery of skilled services. Therefore,while certified agencies surveyed oftenquoted hourly private pay rates for a homehealth aide in excess of $70 per hour, thesewere typically per-visit rates associated withthe delivery of skilled care in the home.Typically, agencies represented toresearchers that a ‘visit’ was one time andlasted about one hour to provide a bath.

State Licensure Status Does Not Seem To

Impact Rates. Even though licensureregulations may vary from state to state,we believe that the amount one pays forcare is more affected by geographicalfactors such as supply and demand than bylicensure status.
