Fire Safety Report Daily Checks
FSM Briefing 2018
Case Study – Daily Checks Date & Time: 28 March 2018 at
2120hrs Venue: Commercial Shopping Centre What Happened: Smoke emerged
from staircase at basement 1. The firewas extinguished quickly but smokefilled up the common areas atbasement 1 & 2 and evacuation wasconducted.
Construction material left uncheckedunder the staircase increased the riskof fire.
Smoke created from the discardedmaterial would have endangeredoccupants using the affected staircase.
FSM Briefing 2018
Lessons LearntHave I done my Daily Checks?
Daily checks conducted should have identifiedthe fire hazard.
Identified fire hazards should be removedswiftly.
Discarded material under staircases, wouldcompromise the safety of the smoke stopstaircases.
Improper storage of material will invite moreaccumulated trash.
There must be a system in place to report firehazards and swift alleviation of the hazards.
FSM Briefing 2018
Case Study – Locked Exit Doors Date & Time: 24 June 2018 at 1604 hrs
Venue: Commercial Shopping Centre
What Happened: Fire Alarm was triggered.Occupants escaping via EM exit doors found that thedoors were not released automatically by the firealarm. It was found to be a false alarm and thesituation reverted back to normal.
Investigation showed that the EM Lock system serverwas logged off, thus preventing the doors fromunlocking automatically.
Had it been an actual fire incident, there would bepeople trapped and consequences would be moresevere.
FSM Briefing 2018
Case Study – Locked Exit Doors
The EM Lock system server waslogged off. This interfered withthe fire alarm system and did notallow all the exit doors to beautomatically unlocked uponactivation.

FSM Briefing 2018
Lessons Learnt Access to the EM lock system should have been restricted.
Regular maintenance and testing must be done on the firesafety systems.
FSMs must ensure that the fire alarm or sprinkleractivation will automatically unlock all exit doors.
There should not be any software or wiring that impedesthis process.
The doors shall remain unlocked until the investigation tothe alarm has been completed and the building fire alarmsystem has been manually reset.
A manual release device also has to be provided within1.5m of the exit door.
Is the fire safety works maintained and in
order?
FSM Briefing 2018
Case Study – Isolation of EVCS Date & Time: 2 Jan 2018 at 0945hrs
Venue: Hotel
What Happened: False ceiling had collapsed duringrenovation works and fire alarm was triggered.
As the EVCS was isolated, no announcement wasbroadcasted.
The occupants were not informed promptly of thecollapsed ceiling.
The affected occupants were subsequentlyevacuated to the floor above.
The delay in notifying the occupants can lead todelayed evacuation that may result in casualtiesduring an actual fire.
FSM Briefing 2018
Lessons Learnt There should not be continuous isolation of EVCS.
Any isolation should be re-instated at the end of theday.
Temporary fire precautionary measures should be inplace i.e. such as extra fire extinguishers, appointingfire watch personnel, emergency responders andcommunication aids.
Designated alternate assembly area must beidentified.
A Fire Plan submission is required for any works doneon the fire protection system.
Is the premises Fire Safety Requirement
in place?
FSM Briefing 2018
Case Study – ERP/Notify SCDF
Date & Time: 22 June 2017 at 2156hrsand 2353hrs.
Venue: Light Industrial premises What Happened: Alarm activated
twice in the middle of the night, andwas reported as false alarm. FSM wasnot informed of the alarm activationand the sprinkler was found to havebeen activated.
Sprinkler activation caused waterdamage to property and no fire reportwas available for insurance claim.
Case of confirmed fire incident was notreported to SCDF.
FSM Briefing 2018
Lessons Learnt
There was no proper investigation on firealarm activation.
There should be a proper reporting processto the FSM and FCC for alarm activationafter hours.
This must be incorporated into the ERP andcommunicated to the related parties.
Any occurrence of fire incident should bereported to SCDF.
Is my ERP robust & adequate?
Should I notify SCDF?
FSM Briefing 2018
Records of Non-performance
We keep records of such lapses orfailures of FSM in discharging theirduties.
Actions such as Letter of Warning(LOW) and Notice of Fire SafetyOffence (NFSO) are included andwill be considered for the nextoffence. Repeated similar offencewill warrant a higher penalty.
In the extreme cases it can lead toremoval from the FSM appointmentor even temporary suspension orde-registration.
