The Strengths Assessment
Identifies:• Strengths (not deficits)• What the clients have done in the past• The resources they have• Their Dreams & Aspirations
Personal Goal Plan• As a mutual agenda for work between support
worker and client
• Focuses on achieving the goals the client has set
• Enables the Client and Practitioner to discuss the long term and short term goals (or steps),
Personal Goal Plan• Assigns responsibility to each task,
• Sets target dates for accomplishment
• Assists in the generation of ideas
• Uses the Strengths Assessment as the primary source of information and guidance
