    Annual Report

    Friends, We are honored to be the medical and dental home for so many of our neighbors. To grow with our community and to keep pace with the demand for our services, we added more providers and more exam rooms. Opening our health center on New Holland Avenue gives us eighteen exam rooms in addition to the 44 at our other sites, improving access for those who seek our care.

    Now offering services in all four quadrants of the City, we struggled with the decision to change our well-recognized and rooted name from SouthEast Lancaster Health Services. The time had come for us to change while keeping our same strong mission. We are excited to be Lancaster Health Center, reflective of our connections throughout the community.

    Increasingly, our patients want to be in control of their healthcare. We resolved to empower our patients by offering them on-line access to health information and self-scheduling. As a nationally recognized Patient-Centered Medical Home, we have been intentional about putting our patients in the center of their care.

    Our teams’ care extends beyond our patients’ health. We keep them well by partnering with others in the community to sponsor Lancaster City’s Bike Share program and by leading Project Lazarus Lancaster County, an initiative to engage the community in effective education and outreach to those impacted by opioid use disorder.

    We thank you for helping us lift up our patients, community, and each other.

Opening our health center on New Holland Avenue gives us eighteen exam rooms in addition to the 44 at our other sites, improving access for those who seek our care.

    Now offering services in all four quadrants of the City, we struggled with the decision to change our well-recognized and rooted name from SouthEast Lancaster Health Services. The time had come for us to change while keeping our same strong mission. We are excited to be Lancaster Health Center, reflective of our connections throughout the community.

    Increasingly, our patients want to be in control of their healthcare. We resolved to empower our patients by offering them on-line access to health information and self-scheduling. As a nationally recognized Patient-Centered Medical Home, we have been intentional about putting our patients in the center of their care.

    Our teams’ care extends beyond our patients’ health. We keep them well by partnering with others in the community to sponsor Lancaster City’s Bike Share program and by leading Project Lazarus Lancaster County, an initiative to engage the community in effective education and outreach to those impacted by opioid use disorder.
