TTA confers stifle stability by aligning the straight patella tendon with the tibial plateau by advancement of the tuberosity such that cranial shear force is mitigated. This model is supported by cadaver studies (Apelt et al, 2007) and clinical reports with the technique have been encouraging. Interestingly, TPLO inadvertently achieves the same as TTA by caudal rotation of the plateau such that patella tendon-plateau angle is similarly 90o. Identical contact mechanical ex vivo studies to those performed with TPLO reveal TTA restores normal contact mechanics (Kim et al, 2008) where TPLO doesn’t. However, recent static radiography has similarly documented chronically subluxated stifles following TTA as for TPLO (Skinner et al, 2013). The last five years have seen several variants on TTA instrumentation and implants unfold such as the MMP and TTA rapid procedure; the results of these variants await peer reviewed critical appraisal and publication.

TTO is a ‘hybid’ procedure that achieves concurrent plateau levelling and tibial tuberosity advance-ment. Ex vivo cadaveric studies are currently lacking confirming that the procedure restores static stifle stability and effects on contact mechanics are unknown but it is likely that a degree of caudal translatory contact shift is induced with reduced contact area but that this is less severe than for TPLO. There are also limited studies and numbers evaluating surgical outcome, and no objective outcome studies currently exist.

Cranial cruciate ligament (CCL) disease is a common cause of hind limb lameness in dogs. Lameness appears to be predominantly attributable to the instability caused by partial or complete rupture which reliably results in progressive osteoarthritis; overt instability predisposing to subsequent meniscal injury. The condition affects young adult large breed dogs and frequently affects both stifles within a year of diagnosis. The estimated impact of treating dogs with CCL disease in the USA alone as been estimated in 2003 to be just over $1 billion (Wilke et al, 2005).

The advent of tibial osteotomies in the 1980’s followed recognition of tibial thrust as a primary force in the CCL deficient stifle. These include closing wedge ostectomy (CWO), TPLO, TTA and triple tibial osteotomy (TTO).

TPLO was a refinement of CWO when attempts to preserve axial patella alignment compared to CWO, however some distal migration had been documented in a recent study (Pozzi et al, 2013). Ex vivo studies have revealed cranial tibial subluxation to be neutralised with a TPA following TPLO of 6.5 ± 0.9o. However ex vivo contact studies reveal post TPLO that contact is both reduced and caudally translated on the plateau (Kim et al 2009). Confusingly, static weight bearing radiography also shows subluxation may persist (Kim et al 2012). A significant refinement of surgical technique has been the application of locking plate technology which greatly reduces the risk of inducing torsional deformity of the bone as well as expediting surgical time by negating the need for precise plate contouring.



Our understanding of the role of the meniscus and appropriate strategies for meniscal surgery have advanced recently. Medial mensical release; either via a mid body transaxial cut caudal to the medial collateral, or via sectioning of the caudal medial menisco-tibial ligament have long been advocated as a means of reducing the risk of late meniscal injury. However, meniscal release destroys hoop stress negating the function of the meniscus such that release is no different to removal. Give the propensity for comparatively low rates of late meniscal injury with the most commonly used cruciate procedures meniscal release can no longer be advocated.

High quality studies using objective follow up to compare surgical techniques have, until recently, been lacking in the literature (Aragon et al, 2005). Two high quality, objective, recent studies have shown clinical results following TPLO to be superior to those of fabello-tibial suture (Gordon-Evans et al, 2013, Nelson et al 2013). A further study comparing TPLO, TTA and Tightrope showed TPLO to be superior to TTA, but subjective outcome measures were employed (Christopher et al, 2013).

Very recent work utilising biplanar fluoroscopic kinematography has allowed, for the first time, definitive objective quantification of the stifle joint when dogs are walking following stifle stabilisation surgeries (Boettcher, 2012). This pioneering work has revealed some disconcerting results that are supported by the aforementioned static weight bearing radiography studies, namely that in a proportion of cases TPLO and TTA do not stabilise the stifle reliably and that at the beginning of the stance phase of gait, caudal subluxation of the femur (rather than cranial subluxation of the tibia) clearly occurs. An explanation for these results, together with recommendations for further refinement of the TPLO / TTA procedure are sure to ensue.

TTA is an orthopaedic procedure to correct a deficient cranial cruciate ligament. TTA changes the angle of the patella ligament to neutralize the tibiofemoral shear force during weight bearing.

Freelance Surgical provide individual components as well as full kits for completing the TTA procedure.

All our TTA implants are manufactured in Tuttlingen in Germany by a specialist company dealing with human and veterinary neuro-surgery and musculo-skeletal titanium implants.

Screws: The TTA screws are titanium. They are available in 2.4mm, 2.7mm and 3.5mm diameter with lengths from 6mm to 40mm.

Forks: The TTA forks attach the TTA plate to the tibial crest, the prongs of the fork are passed through the holes in the plate and introduced into the corresponding holes in the tibial crest and pushed into position.

Plates: The plate is for stabilising the osteotomy, they are specifically designed for the corresponding forks.

Forkless Plates: If a forkless plate is used then you do not need the additional fork, the plate is attached to the tibial crest using a quantity of 2.4mm titanium screws.

The standard TTA procedure assumes that there is no co-existing patella issues. The tibial tubercle is simply advanced. In cases where there is patella luxation in addition to cranial cruciate rupture it is possible to move the tibial tubercle either laterally or medially to help re-align the quadriceps mechanism. This is achieved by adding TTA Spacers either to the cranial screw or the caudal cage screw.

TPLO Air Powered Saw


TPLO is used to stablise the stifle joint after ruptures of the cranial cruciate ligament. The cruciate ligament prevents forward and backwards movement of the femur on the tibia bone.

This surgery involves creating a curved osteotomy in the top of the tibia and then rotation of the plateau segment, this is done until the previous slope in the bone is no longer present. The bone is subsequently fixed in this new position using a bone plate and screws.

Cortical Screws: These have closely-spaced, shallow threads and larger core-to-outer diameter ratios than cancellous screws. Cortical screws are stronger than cancellous screws of the same outer diameter.

They are usually blunt ended. When used to fix plates to long bones, fully-threaded screws are used in order to fixate both cortices so that maximum stability is achieved.

The blunt end should extend only a few millimeters into the soft tissues beyond the far cortex, this minimizes damage to the soft tissue.

Linear fixation kits

External Skeletal fixation (ESF) is a useful tool for the veterinary surgeon providing advantages in the management of numerous fractures types and locations.

ESF helps maintain joint mobilty and use of the limb as well as providing a lower risk alternative to internal fixation where infection is present.

IMEX™ Veterinary, Inc. of Longview, Texas was founded in 1990 with the mission of providing the veterinary surgeon with premium, affordable external skeletal fixation (ESF) pin technology.

Unlike traditional and newer KE frames the IMEX™ ESF system features dramatically improved connecting rods and positive threaded pins used directly through the primary clamp bolt to enable much simpler frames whilst improving stablility and patient safety. With the IMEX™ ESF system there is much less emphasis on full-pins and frame complexity with safe pin corrider philosophy to reduce patient morbidity. This makes the overall system more economical, reduces operative time, makes it safer and more effective.

IMEX® DuraFace® half-pins represent the first major ESF pin improvement in veterinary surgery in more than 20 years. DuraFace® half-pins offer increased stiffness, increased strength and longer cyclic fatigue life without an increase in pin tract diameter in the corresponding bone. SKTM Clamps

The Single SKTM Clamp utilises a wrench, and a stainless steel external rod (sizes dependent on clamp size). The single SK™ ESF clamp is the primary construction element of any linear fixator and is available in 3 sizes (Mini, Small and Large). (U.S. Patent #5,921,985).

The Double SKTM Clamp is used for connecting two external rods. Stability is provided by positive interlock between clamps. Two wrenches are required. Available in 3 sizes (Mini, Small and Large) (U.S. Patent # 5,921,985).

The SKTM rods are stainless steel, titanium or carbon fibre dependent on size and type required. The rods are either smooth surfaced or a hybrid rod. The standard hybrid rods have a 6mm threaded section at one end of the rod, this allows coupling with components of the IMEXTM circular ESF system to create a complete hybrid system.

The Mini SKTM rods are available in stainless steel (3.2mm Ø) and stainless steel hybrid (3.2mm Ø and 4mm thread)

The Small SKTM rods are available in titanium (6.3mm Ø) , carbon fibre (6.3mm Ø) and titanium hybrid (6.3mm Ø and 6mm thread)

The threaded portion of the hybrid rods is approximately 25mm in length.

The Small and Large hybrid rods can be used with the Small Hybrid Kit.

The Mini hybrid rods are used with the Mini Hybrid Starter Kit.

SKTM Hybrid adaptor

The small SKTM Universal Hybrid Adaptor offers the advantage of using standard small SKTM titanium or carbon fibre rods to create hybrid frames. In addition it offers up to 650 of adjustment compared to 150 using standard spherical nuts and washers.

- Attaches easily to a 50mm, 66mm, 84mm or 118mm ring with two 6mm x 16mm hex bolts and 6mm hex nuts. Third hole can be utilised for additional hardware placement.

- Rod-gripping element provides a secure fit for 6.3mm rods while allowing for 650 of adjustment. Accepts standard small SKTM ESF titanium or carbon fiber rods.

- Rod-gripping bolts should be alternately tightened using a lag effect to secure rod.

- Does not reduce need for additional side bar or support struts.

Circular Fixation Rings:Various styles and sizes of rings serve as the basic structural element of circular ESF and hybrid ESF frames. IMEX™ hybrid and CESF systems are divided into two basic sizes, the standard and mini. The standard is constructed around 6mm threaded rods and bolts while the mini utilizes 4mm fasteners and rods.

Angular Hinges:Angular hinges are typically used in pairs on CESF frames for gradual correction of angular limb deformities. However, they are often also used on hybrid frames to create an articulation or strut to provide additional support for a ring. Available for mini and standard CESF/hybrid ESF frames. (U.S. Patent # 5,968,043)

Spacers, plates & posts:Spacers, plates and posts are additional construction and support elements that are modular in nature and allow the operating surgeon great flexibility when constructing hybrid ESF or circular ESF frames.

Fixation wires:Fixation wires are the key bone fixation elements in circular fixation and most hybrid ESF frames. They are available in several diameters and in smooth and stopper wire versions.

Linear Distraction Motors:Linear distraction motors are the simplest method of constructing CESF frames for limb lengthening and are designed to simplify daily patient management. Available only for the standard size CESF system. (U.S. Patent #5,968,043)

AcrylxTM System

AcrylxTM is a methacrylate based composite resin designed to simplify construction of pin and acrylic external fixators on boy breeds, avian, exotics and mandibular fractures.

Unlike other commonly used products, AcrylxTM has minimal odor and does not require pre-mixing before application. AcrylxTM is near-radiolucent while providing abrasion resistance and high strength.

AcrylxTM Resin Cartridge

A single cartridge provides 50ml of mixed product - enough material to construct several small patients frames.

Maximum shelf life is maintained by refrigerated storage but not required.

If refrigerated, it is recommended to allow the cartridge to reach room temperature before use to avoid any alteration in setting characteristics.

A ten to twelve minute set time is typical. Each cartridge purchase includes five yellow and five pink mixing tips.

The VetSEAL generator offers all the functions of an Electrosurgery unit with bipolar thermo-fusion for sealing upto 7mm vessels. VetSEAL delivers a high frequency power of up to 150W inducing a conversion of collagen and elastin. The Advantage EcoCAM is ideal for both flexible and rigid applications. This camera is ideal for those wanting to set up a new endoscopy system, to provide an additional endoscopy system to ambulatory vets or adapt and improve existing bulky low quality units. EcoCAM is a new single-chip CDD camera with high integrated digital signal processing guaranteeing true colour reproduction. A 1/3” high resolution CDD-chip and a fast acting electronic processor ensure the higher quality image, high clarity and sharp detailed reproduction. This camera comes with a soakable camera head and an anti-moiree filter, this is particularly useful when connecting flexible endoscopes. This camera comes with a 28mm focal adaptor.

Advantage StarCAM


The Advantage Multicam II is an all in one camera system comprising of a high quality LCD monitor, camera unit with Sony CCD camera chip and 28mm focal/scope adaptor.

The camera benefits from a Moiré filter which softens the image when fibre optic scopes are attached.

The USB system has an integrated medicapture unit that allows you to grab images as you go, by using a button mounted on the camera head. The images can then be moved to a USB stick for archiving, manipulation or referral. The Multicam II is superior to the Cam 2 unit in that you have the ability to capture both still and motion to the USB stick.

The Multicam II has an illuminating 50W Xenon light source with an Olympus® OES light fitting and air pump.

The Advantage Medicapture USB200 connects into your system using SVHS or BNC cables supplied. One of the advantages of the USB200 is that it has a front mounted monitor, this means that it doesn’t need to be looped into the system and real time images can be viewed and reviewed.

The unit has been designed with simple plug and play performance in mind. Once installed still images can be grabbed and DVD quality motion can be recorded directly to the USB stick. Once the images and videos are on the stick they can be transferred to any computer for archiving, manipulation or referral.

Dicom Image storage can be achieved with this unit.

Locking and non-locking screwsLocking screws undergo increased higher shear stress and increased bending forces compared to non-locking screws. To counteract this they have a wider core diameter, smaller pitch thread and a smaller thread depth. The screw heads have a conically threaded profile which engages with the thread in the plate hole, this produces angle stable fixation within the plate.

Self tapping and non-self tapping screwsNon self-tapping screws require a thread to be cut into the bone prior to screw insertion. Non-self tapping screws can be screwed directly into bone and will cut their own thread.

Non-self tapping screws can be removed and reinserted without the risk of cutting a new thread. Self-tapping screws may cut a new thread if removed leading to loosening. They can also be used as lag screws, as they have a greater thread contact and generate less heat in placement than self-tapping screws. This is also helped by the tap having a sharper cutting tip than the screw and has flutes designed to clear bone debris.

Self-tapping screws have the advantages of being quicker to place as they do not require tapping before placing and they can also be superior in very thin cortical bone and cancellous bone. Self-tapping screws are useful in the proximal tibia where the cortices are thin and are therefore used often in TPLO and TTA procedures.

Fracture-fixation by bone-plate and screw is intended to provide immobilization at the fracture site and reduce the fracture gap. The role of the plate and screws is to hold the fractured bone segments in position, without allowing tensile stresses at the fractured interface, but having a level of critical compressive stress induced in it, therefore accelerating healing.

Plates are classified by the size of the screw they are used with. For example a 2.4mm plate is designed for use with a 2.4mm diameter screw and a 3.5mm plate is designed for use with a 3.5mm diameter screw.

Plate Stiffness and Size?Repairing a fracture with too stiff a plate will lead to a condition called stress protection and consequent osteoporosis.

Specific guidelines exist for the minimum length bone plate that should be applied, although no specific guidelines exist for the maximum length. The general rule is there should be a minimum of 6 cortices either side of the fracture line. This is the “6 cortex minimum rule”. In some cases fracture configuration or location may be such that insufficient suitable bone exists proximally or distally to the fracture site to enable placement of 3 two-cortex screws (for example in small/toy breed dogs where often only two screws will fit). In cases such as these the 6 cortex rule may need to be broken.

Plates are generally stainless steel or titanium. Stainless steel plates are used with stainless steel screws, and titanium plates are used with titanium screws.

Cortical Screws

Fully threaded, self tapping Cortical Screws. Available in most sizes from 1.5mm Ø to 4.5mm Ø in stainless steel and titanium.

Cortical screws are designed for use in hard cortical bone and are fully threaded along the shaft length. A cortical screw has a greater core/thread ratio than a cancellous screw. This means that cortical screws have a relatively wide core, narrow thread and shallow thread pitch compared to cancellous screws.

Cancellous Bone Screws are designed for fixation of cancellous bone. They are most commonly used in the metaphyses of long bones where cancellous bone is abundant.

They have more deeply cut and more widely spaced threads compared to cortical screw. Since cancellous bone is much less dense than cortical bone, the screw threads cut their path in the bone when the screw is inserted, i.e. cancellous screws are self-tapping.

A Cortical Screw


Threaded wires will have a much greater pull out resistance than the smooth wires. They are generally inserted using a drill which is fitted with a Jacobs Chuck or Wire Driver.

They have trocar tips, but will still be able to be self-tapping, although the extremely thick & dense cortical bone may cause issues.

Threaded wires can be used as mini-position screws, and can be cut off flush to the bone surface.

Surgical Patient Positioning

Olympic Vac-Pacs have been used for more than 30 years in thousands of hospitals for improved surgical positioning. Vac-Pacs are soft pads, which are available in various sizes and shapes, filled with thousands of tiny plastic beads. the Vac-Pac is moulded around the patient, then a suction line is attached.

As a vacuum is created inside the Vac-Pac, outside atmospheric pressure forces the beads together, and the Vac-Pac becomes firm.

When the suction line is disconnected the Vac-Pac retains its firm, moulded shape to hold the patient securely in position.

After surgery the valve is opened and as air re-enters the Vac-Pac it becomes soft again, ready for re-use.

T e r m s &C o n d i t i o n s

1.0 Definitions1.1 ‘Company’ means Freelance Surgical Ltd1.2 ‘Customer’ means the veterinary practice or any person to who the Company has agreed to supply goods1.3 ‘Goods’ means the products which the Company is to supply to the customer in accordance with these conditions1.4 ‘Writing’ includes telex, cable, facsimile transmission, e-mail or similar means of communication

2.0 Breakages and ShortagesIn the case of obvious shortages or suspected damages to deliveries of goods, the drivers delivery note must be signed accordingly by the person accepting the delivery on behalf of the customer. All claims for broken or missing goods should be reported to the Company no later that three days from the date of delivery. Where independent transport is used failure to sign or indicate on delivery note any shortage and/or damage will invalidate the Company’s claim against the carrier involved and therefore the customer’s claim against the Company.

3.0 Returned Goods Policy3.1 Under normal circumstances, goods will not be accepted for return for credit other than for the following reasons:a. To correct a Company delivery or order errorb. In response to a recall instigated by a manufacturerc. Where the products or packages are alleged to have been faulty, damaged or tampered with prior to deliveryd. Where the products have been offered on a pre-arranged ‘sale or return’ basise. Where the Company agrees, in advance of their return, to accept goods not covered by any of the above criteria 3.2 Goods returned to correct a Company errora. The goods must be returned within 7 working days of receipt. Goods requiring temperature controlled storage must be notified to the Company no late than the next working day and must have been stored in accordance with the manufactur-er’s recommendations whilst on the customer’s premises. Goods must be ade-quately packed to ensure no damage is caused during their return to the Company.b. All returned goods must be in perfect original condition; not marked in any way; should not show any evidence of being tampered with.c. To be considered for return for credit the goods must be accompanied by a copy of the original invoice or delivery note, or the number of the specific invoice on which the goods were supplied, together with a reason for their return. If this information is missing there will be a delay in issuing credit.3.3 Returns where the products or packages are alleged to have been faulty, dam-aged or tampered with prior to delivery:a. Details of the fault or damage should be notified to the Company as soon as possible in order that the manufacturer can be alerted if appropriateb. Good must be packed separately and accompanied by a copy of the invoice or delivery note (as in 3.2.c.)3.4 Where the Company agrees to accept the return of goods not covered by any of the criteria recited in clauses 3.2-3.4 Goods will only be accepted by prior agree-ment with the Company and must be accompanied by all the information detailed in 3.2(c), in addition to the name of the person who has authorized the return.Please note: Goods returned under this category may be subject to a handling charge3.5 Goods returned to stockGoods returned by customers for any of the above reasons will only be considered for credit if:a. It is known that they have been stored in accordance with the manufacturers recommendationsb. They have been assessed and accepted by a person authorized by the Compa-ny to do so. The assessment will take into account the nature of the product, any special storage conditions required and the time elapsed since the goods were supplied.3.6 Special ordersThese products will not be accepted for return or credit unless supplied as a result of a Company error. In such cases, the product must be returned within 7 working days of receipt.3.7 PackagingAll returned goods must be securely packaged to prevent damage during the re-turn journey3.8 Date expired goods.Date expired goods will only be accepted for credit in accordance with the terms laid down by the manufacturer concerned

4.0 Price of goodsGoods will be invoiced at the prevailing price at the date of dispatch to the customer. The price of goods shall be subject to alteration by the Company without prior notice to the customer. The price is exclusive of any Value Added Tax for which the customer shall be additionally liable to pay the Company.

5.0 Terms of Payment5.1 The customer shall pay the amount of the invoice by return unless otherwise agreed in writing by the Company. If the customer fails to pay on the due date the Company reserves the right to:a. Suspend any further deliveries to the customerb. Appropriate any payment raised by the customer to such of any other invoices raised by the Company for goods supplied to the customer as the Company may think fit, notwithstanding any purported appropriation by the customer and:c. Charge the customer interest (both before and after any judgement) on amounts unpaid at the rate of 5% per month until payment is made in full. Notice of interest charges will be served in writing on the customer.

6.0 DeliveryAny dates/times quoted for delivery of the goods are to be treated as estimates only and the Company shall not be liable for any loss, injury, damage or expense arising from delay in delivery of the goods howsoever caused.

7.0 Force MajeureThe Company shall not be liable to the customer or be deemed to be in breach of the Contract by any reason of any delay in performing, or any failure to perform any of the Company’s obligations in relation to the Goods, if the delay or failure was due to any cause beyond the Company’s reasonable control. Without prejudice to the generality of the foregoing, the following shall be regarded as causes beyond the Company’s reasonable control:a. Act of God, explosion, flood, tempest, fire or accidentb. Strikes, lock-outs or other industrial actions or trade disputes (whether involving any employees of the Company or third parties)c. War or threat of war, sabotage, insurrection, civil disturbance or requisitiond. Import or export regulations or embargose. Difficulties in obtaining raw materials, labour, fuel, parts or machineryf. Power failure or breakdown in machinery.

8.0 LiabilityGoods are sold on the understanding that the customer is solely responsible for determining the suitability of the goods for the purposes for which the customer intends to use them. Accordingly the Company’s liability for any loss or damage suffered by the customer and arising by defects in the goods shall be limited to the invoice price of the goods in respect of or in relation to which loss or damages is claimed. The Company will not accept under any circumstances liability for conse-quential loss, damage or loss of profits arising from the use of the goods, service, information and advice.

9.0 Title and RiskUnless otherwise agreed in writing between the parties prior to delivery, the title to the goods shall not be transferred to the customer until the full purchase price has been paid to the Company. The risk of loss or damage to the goods shall pass to the customer on the date that the goods are received and accepted by the customer.

10.0 Insolvency10.1This cause applies if:a. The customer makes any voluntary arrangement with its creditors or becomes subject to an administration order or (being an individual or firm) becomes bank-rupt or (being a company) goes into liquidation (otherwise than for the purpose of a solvent amalgamation or reconstruction).b. An encumbrancer takes possession, or a receiver is appointed, over any of the property or assets of the customerc. The customer ceases, or threatens to cease, to carry on the businessd. The Company reasonably apprehends that any of the events mentioned above is about to occur in relation to the customer and notifies the customer accordingly10.2If this clause applies then, without prejudice to any other right or remedy available to the Company, the Company shall be entitled to cancel the Contract or suspend any further deliveries under the Contract without liability to the customer. If the goods have been delivered but not paid for, the price shall become immediately due and payable notwithstanding any previous agreement or arrangement to the contrary.

11.0 IndemnityThe customer agrees to indemnify the Company against all loss, damage, costs, legal costs and professional and other expenses of any nature whatsoever in-curred or suffered by the Company or by a third party as a result of the use of the Goods, the breach of any legislation relating to the use of the Goods or as a result of any breach of the Contract by the customer

12.0 JurisdictionThese conditions shall be construed in accordance with and governed by English Law. Each of the parties irrevocably submits for all purposes in connection with the conditons to the exclusive jurisdiction of the Courts of England.
