Comes up with new/innovative practices that contributed to enrichment of food safety culture 3. Award Packet Guidelines

Submit one electronic PDF document that includes each of the following numbered documents and

email the PDF to Cathy Blood, [email protected]. Save and send the document with following

format: Team/company’s name-Team Award.

Award Packet Cover Form - Please see attached.

Summary

A narrative summary not exceeding three (3) single spaced pages (Times New Roman, 12 point)

detailing how the team:

Enhances the food safety culture in the whole company.

Comes up with new/innovative practices that contributed to enrichment of food safety culture.

Impacted the food safety system, for example by reducing the amount of “on hold” product year

over year, etc.

Shows clear evidence that the effective “product or output” of the team effort exceeds the simple

sum of the parts contributed by individual members.

Displays effective integration across units, departments, and functions of the company.

Has gained recognition by colleagues, professional groups, industry, public agencies, etc.

Professional Achievements

Team Award: List selected products emerging from the activities of the team, including awards or other

professional achievements. Not to exceed two (2) single-spaced pages (Times New Roman, 12 point).

1. Letters

These letters should be organized into categories — department manager/director (required), peers, current and former customers/clientele or others with no more than two (2) letters in each category. If the manager or director nominated the individual, that letter can suffice for the required letter in this section. Do not exceed seven (6) letters total. Letters of support should be included in the single PDF document.

2. 300-Word Biography for Background of Team for the FSS Website

3. A High Resolution Photo of the team.

Selection Process

Selection committee will review the award nomination packets and make their decision. The team

award finalist will be announced at the Food Safety & Sanitation Workshop in November.
