Soccer field, Palmer Park & Escalante Park: O Demolish concrete pole base foundations 2' below native grade O Import, place and compact earth fill at pole base void to native grade O Remove and replace chainlink fence sections at Escalate park to reach two (2) pole bases O Remove and replace 10' x 10' concrete SOG sections to expose two (2) concrete pole foundations o Remove and dispose of concrete slab on grade at power pedestal at Kiwanis soccer fields O Provide all supervision and temporary facilities to install the above scope of work Light Pole Testing Recommendations 2010 — Diablo Soccer Fields, JayCee Park, Arredondo Sports Complex Softball fields and Soccer fields #14: o Perform repairs as outlined in the Olsson Associates reports for the above listed facilities O Provide coating at all steel light poles O Remove hand hole covers, Install gasket and reinstall cover O Repair concrete pole bases utilizing the specified patching materials O Provide concrete pole base coating with specified Thorocoat product o Replace electrical duplex receptacle at one (1) pole location O Provide all supervision and temporary facilities to install the above scope of work. All work shall be In accordance with the following scope of work description: Scope of work: Pole foundation removal —

Kiwanis N. Soccer field, Palmer Park & Escalante Park: O Demolish concrete pole base foundations 2' below native grade O Import, place and compact earth fill at pole base void to native grade O Remove and replace chainlink fence sections at Escalate park to reach two (2) pole bases O Remove and replace 10' x 10' concrete SOG sections to expose two (2) concrete pole

foundations o Remove and dispose of concrete slab on grade at power pedestal at Kiwanis soccer fields O Provide all supervision and temporary facilities to install the above scope of work

Light Pole Testing Recommendations 2010 — Diablo Soccer Fields, JayCee Park, Arredondo Sports Complex Softball fields and Soccer fields #14:

o Perform repairs as outlined in the Olsson Associates reports for the above listed facilities O Provide coating at all steel light poles O Remove hand hole covers, Install gasket and reinstall cover O Repair concrete pole bases utilizing the specified patching materials O Provide concrete pole base coating with specified Thorocoat product o Replace electrical duplex receptacle at one (1) pole location O Provide all supervision and temporary facilities to install the above scope of work.

There are three (3) ballfields in the City of Tempe parks that the City has removed, or are going to remove, the light pole fixtures due to corrosion at the base plate welds. With the poles removed they now are asking us to remove the concrete pole bases. The parks are fisted below along with the scope required for each location:

Escalante Park: 2150 East Orange Street Tempe, AZ 85281 Bonfield on the NE corner of River Road and Orange Street.

Light poles have been removed. Demo and haul six (6) each concrete pole bases to 2 below grade. Fill the hole with earth material and compact in place. At the two (2) pole bases by the backstop, remove the 10' x 10' concrete slab surrounding the pole base.

Kiwanis Park Soccer Fields: 6111 South All-America Way Tempe, AZ 85283 Soccer fields are on the Northwest end of the Park. The entrance to the soccer fields is from Baseline Road just West of Mill Avenue.

Light poles have been removed. Demo and haul six (6) each concrete pole bases to 2' below grade. Fill the hole with earth material and compact in place. Demo & haul the concrete slab on grade around the electrical pedestal between the fields on the South end of the fields.
