Conferences: The World Congress on Contraception at Jaipur: a highly successful event attended not only by gynecologists but various organizations associated with population control. A sure miss for all those who didn’t attend as it was a highly academic event and truly a focused conference.

Workshops: Successful initiation of the Endofert and IUI Workshops, PCOD workshops, safe surgical practice workshops. I sure am proud of my Vice Presidents. Also Critical Care in Obstetrics and Eclampsia workshops and AVD workshops receive a huge appreciation all over. Truly appreciate the participating members and societies.

Public Awareness

Huge rally at Baramati, Maharashtra, for ‘Save the Girl Child’, with MP Supriya Sule.

Visit to Mandla Project where free of cost outreach services provided by members of the Jabalpur Society to the tribes over a period of week performing mammoth surgeries.

For the patients:

FOGSI will adopt the Patients Rights’ Charter. This will be displayed in each FOGSI member hospital. Hard copy of the same will reach every one. Can be downloaded from the FOGSI website.

Village adoption scheme of FOGSI now will be modified to apply to the needy parts of the county to help offer care where it is most needed.

Important interventions like use of intravenous iron, magnesium sulphate prefilled syringe, availability of folic acid supplementation at PHCs, non-pnuematic anti-shock garments being made available, availability of misoprostol tablets, availability of cryo cautery and VIA at PHCs, insisting on obstetric facilities at the PHCs, demanding proper placements of the EmOC trained MOs, acquiring accreditation for obstetric care for FOGSI hospitals and the 108 ambulance services to be sought with continuous dialogue with the policy makers through FOGSI.

The Task Space

Dear FOGSIANS,

If you want to walk the untrodden path, if you want to dream BIG, if you want the organisation to take risks, if you want to have conviction, if you want to go against the conventional wisdom, if you want to take tough decisions – then the opportunity lies in occupying FOGSI Leadership Posts and having the courage to accept the TASK of turning the Vision of our President into a Reality.

I have had the courage to make a commitment to take on the position of “TASK MASTER” because I believe that a plausible impossibility is better than a convincing possibility! As I am just beginning to understand the magnitude of the tasks, I have the fullest faith in the FOGSIANs in various task groups, as much trust that I have in our President who has mooted this idea.

In the first phase, we assign ourselves with the following five tasks for brainstorming deliberations and to pave the path for further tasks in future.

n WORK ALLOTED TO CHAIRPERSONS – Task force to guide, monitor and evaluate and synergise the efforts by and within various committees of FOGSI.

n To review the conduct of Managing Committee Meetings and take inputs from experienced seniors and fresh views from youngsters – to make the process time-effective.

n To draw up a realistic syllabus for PG course for five days / Faculty pool / CDs or work book materials / Exam rehearsals / Online upgradation and follow ups / Online practice exams and practice sessions and peer group review with model answers.

n Liaison with State and National governments to assist them to lay out the policies on matters related to women’s health.

n Vision and implementation of Public Private Partnership Modules to strengthen the public health systems.

Our commitment to openness and fairness is exemplified by the famous adage

“In God we trust, everybody else brings data to the table”

The theme of the year “Reaching the unreached” is taken up seriously by the office bearers and chairpersons of committee of FOGSI, under

the able guidance of our President, Dr. Sanjay Gupte. Excellent idea! I want to bring to your kind notice only one part of the theme in this issue and that is how to reach FOGSI Members? When I was looking at the details of Presidents & Secretaries of our member bodies, many had not mentioned PINCODES, few had not mentioned mobile numbers and their e-mail ids. How do you expect FOGSI to reach their members if this vital information is not provided? We have realized that few of the Presidents / Secretaries and Chairpersons do not open their emails regularly, because we do not get the response from them for weeks. I personally feel that the only way FOGSI can reach important members is by sending SMS when an urgent message has to be informed. We have worked out the nity gritty of the same and we want to implement it now. I am sure, the person getting SMS will atleast see it on the same day and respond. This will be our first step towards Reaching the Unreached FOGSI Members. I humbly request you to send your MOBILE numbers & email ids to local societies and the Presidents & Secretaries of Member bodies of FOGSI should pass on this information to FOGSI office. We can hold FOGSI election via Mobile phones! Please think and respond positively. If we have mobile numbers of all the Members, they will get the information from FOGSI office at a click of a button. Long live FOGSI. The rest of it in forthcoming issues.

The Assisted Vaginal Delivery workshop launch was hosted by Jalandhar Ob-Gy Society on 14 February 2010.

Program started with a video message by President Dr. Sanjay Gupte. The workshop was inaugurated by Dr. Kapil Sharma (Member, Punjab Medical Council). Dignitaries present included Dr. Kamal Gupta (Patron), Dr. Behram Anklesaria (PP FOGSI), Dr. Subhash Nargolkar (Sr. Scientific Advisor, AVD workshops), Dr. Amita Sharma (President, Jalandher Ob Gy Society), Dr. Parag Biniwale (Convener, AVD).

Over 100 members of J a l a n d h a r , L u d h i a n a and Amritsar a t t e n d e d . Punjab Medical council had accredited the program with

4 CME hours. The program was covered by media as well. Dr. Behram & Dr. Parag addressed the press reporters.

Reaching out to the Members

History was created in the Punjab, on Valentine’s Day, when Dr. Sanjay Gupte’s Inaugural workshop on Assisted Vaginal Delivery turned out to be a

triumphant FOGSI event, organized by Dr. Parag Biniwale and (Jalandhar Society President) Amita Sharma. This was to be the first FOGSI AVD workshop in recent times, but would our younger members, preoccupied with ‘modern’ high tech subjects, bother to attend?

The answer was a resounding ‘yes!’ with 143 delegates streaming in from all over this frontier state. Considering that Jalandhar FOGSI has a total membership of 160, the delegate strength was simply amazing! Old war horse Avtar

A Workshop to Remember…

Dr. Behram S. Anklesairia Singh Saini of Amritsar led a contingent of young FOGSI members from rural and urban areas. Elders like Drs. Sushma Chawla and Kamal Gupta looked extremely pleased at the success of their hard work. News papers, the next day widely appreciated FOGSI’s efforts towards Vaginal delivery promotion in the midst of sky rocketing C.S. rates.

The morning began with a thought provoking Presidential message from Dr. Sanjay Gupte. I followed with my presentation on ‘ECV Revisited.’ Dr. Parag Biniwale covered medico legal aspects and led boisterous and active audience participation. Dr. Machave gave very practical tips on Vacuum Delivery. But the star of the day was forceps Icon Dr. S. Nargolkar. He showed us his masterly New forceps video and charmed young and old with his delightful insights on the modern relevance of a long neglected topic.

The Fogsi Focus on IUI (Intrauterine Insemination) was released in January 2010 at the AICOG. The FOGSI focus has been aimed to provide updated

knowledge by our experts on various aspects of intrauterine insemination. This FOGSI focus will be a ready reckoner for our gynaecologists.

We will be conducting 20 ‘Endofert’ Workshops on Endocrinology and Infertility all over India. This will be a day programme focusing on various aspects of Endocrinology & Infertility such as Hyperprolactemia, Reproduction and the thyroid, Modern Mx of dysfunctional uterine bleeding, hormone contraception today, HRT today, managing cosmetic problems of PCOS, Panel discussion on Recurrent pregnancy loss and various areas in Infertility such as Ovulation induction, Medical management of oligospermia, ART, management of endometriosis.

l Clinical research will be conducted for the same.

l Also CD’s of the programme will be released and circulated.

Workshops have been successfully held at Navi Mumbai and Aurangabad. Future Delhi, Bhopal, Ahmedabad, Lucknow, Kanpur, Jaipur, Hyderabad, Cochin.

A focused Conference on Infertility will be held on 29th 30th May 2010 at Hotel Taj Lands End, Mumbai. This will be focusing on common problems in infertility such as ovulation induction, IUI, male factor infertility as well as details discussion on recent advances such as vitrification, invitro maturation etc. There will also be a hands on IUI workshops as well as a hysteroscopy and ART workshops.

I will be overseeing the working of the following FOGSI Committees and working in close collaboration with them to ensure that Fogsi reaches out to each and every of its members.

Happy holi and greetings!

Year has begun with enthusiasm and lot of expectations from FOGSI executive and members. We are seeing a change in FOGSI governance and approach, concentrating more on academics and data collection, and a lot of freedom to persue individual projects and division of labor without loss of focus. Thanks to Dr. Sanjay Gupte year seems to be running smoothly like a well oiled team.

My aim in this year is to reach out to as many small and big societies enthuse in our members the need of data collection and reporting birth defects to BDRI, and spreading knowledge about PCOS and prevention of long term implications of PCOS and emphasise on preventive aspects of birth defects.

My projects for this year :

FOGSI MERCK SERONO PCOS Initiative

Plans to do 15 workshops on understanding PCOS from diagnosis, and management in various age groups and also prevention of metabolic syndrome. Prepared lecture modules for societies and young college going girls, multiple contributors, along with FAQs on PCOS, and questionnaire for data collection. We have launched a website also log in www.pcostalk.co.in.

Maternal Mortality in India – a FOGSI Survey

FOGSI is conducting the survey of Maternal Deaths in our country. The data form will reach you soon, both by e-mail and by post. We are all toiling day and night to save our mothers. In spite of our best efforts, we are continuing to face the tragedy of maternal deaths and it is important for us to know “Why Mothers Die” in our Country! Kindly spare few minutes from your busy schedule and complete the datasheet summarizing trends in maternal mortality in your institution. It is important to send the number of deliveries even if there are no maternal deaths.

You must be aware that the theme of FOGSI for this year is “Reaching the Unreached”. It always reminds me about interaction in many AICOG,

where FOGSIANs ask me what FOGSI is doing for a common gynecologist! Aim of this years theme is to focus on the unreached public and common gynecologists. As there are so many baseline problems related to Obstetrics & Gynecology in our country which we are not yet able to approach. These areas are high maternal mortality, anemia, eclampsia, PPH, APH and oncology especially advanced

cancer cervix. If we can concentrate on these targeted problems as for example correcting anemia, preventing eclampsia & PPH, encouraging institutional delivery and focusing on awareness regarding screening for cancer cervix; we can certainly decrease MMR.

For FOGSIAN we should do certain things. There are so many controversies regarding many surgical procedures starting from episiotomy, MTP, Vaccum & Forceps, CS, TAH, NDVH, Ward Mayo’s operation, laparoscopic procedures and internal illiac ligation. With this in mind we have designed the workshop on Safe Surgical Practices in OBGYN.

It also is important to detect cancer cervix in its early stages. For this, I appeal to all the FOGSIANs to pergorm at least speculum examination routinely. It also is essential that we perfect our knowledge about basics of OBGYN and surgical anatomy.

At the end we can only progress when we can realise.

Reaching out to the Members

Under the aegis of ‘FOGSI’ and ‘obstetrical and gynecological society of northern India ‘one day workshop and CME were organized by the Department of Obstetrics and Gynecology at Post Graduate Institute of Medical Education and Research (PGIMER) Chandigarh. The organizing committee consisted of Dr. L K Dhaliwal (HOD, Department of Obstetrics and Gynecology, PGI and President of Obstetrical and Gynecological Society of Northern India), Dr. Vanita Suri (Professor, Department of Obstetrics and Gynecology, PGI and Honorary Secretary) and Dr. Vanita Jain (Professor, Department of Obstetrics and Gynecology, PGI and Treasurer). The theme of the workshop was Critical Care in Obstetrics and Eclampsia. Around 200-250 doctors from Chandigarh and neighboring states of Punjab, Haryana and Himachal Pradesh attended the conference.

The whole scientific curriculum for the workshop was divided into three sessions and a panel discussion. There were 3 lectures in each session and senior obstetricians from PGI, Government medical colleges of Chandigarh, Patiala, Simla and DMC Ludhiana and practicing obstetricians delivered these. The President’s video message on the high maternal mortality and need for starting workshops for rural doctors under the FOGSI theme ‘Reaching the Unreached’ was presented. Dr. Girija spoke about Preeclampsia and HELLP, Dr. Mandrupkar presented eclampsia drills and Magnesium sulphate regimens and Dr. Suchitra Pandit delivered a talk on how to set up high risk obstetric units. The panel consisted of two gynecologists including Dr. Girija, one nephrologist, one anesthesiologist, a cardiologist, a neonatologist and a blood transfusion specialist; all from PGI. Three case summaries of critical patients ie, abruption, placenta previa and mitral stenosis were discussed in detail.

All the participants felt that the workshop was quite useful and appreciated the programme.

March 6th and 7th 2010 witnessed a congress titled “Contraception and Beyond...” at the Pink City Jaipur under the aegis of FOGSI. Concept of a Focused Conference on “contraception” brought together the experts to deliberate on the strategies and ways forward to tackle the burning issue of population explosion in India. The vision of Dr. Sanjay Gupte, President of FOGSI was executed by the able team of FOGSI lead by Dr. Ritu Joshi, the Chairperson of FOGSI committee for Contraception and Family Welfare and the members of Jaipur Ob & Gyn Society with Dr. Adarsh Bhargav as the Chairman of Conference Organising Committee.

True to the theme of FOGSI “Reaching the Unreached” the Ob & Gyn community reached out to the Government Officials, NGOs, demographists, population experts who made significant contributions to the success of this congress. Hon. Minister for Health and Women and Child Development Mrs. Bina Kak inaugurated this prestigious meet and Mr. Sandhu, Principal Health Secretary released the “FOGSI FOCUS” on IUCD supported by Population Services International (PSI) and thus the first insights into possible collaborative efforts were established.

(A) Dr. Paul Blumenthal USA - spoke on deprivation of resources posing a serious threat if the projected population in India is not curbed down

(B) Dr. Sanjay Gupte voiced his thoughts on advocacy on acceptability and accessibility at the level of individual client at the rural and urban centers

(C) Mr. Amerjeet Singh gave an impressive account of the “Gujarat Model” of Public Private Partnerships to expand care and reduce maternal mortality. It was an eye opener to the opportunities which FOGSI members could have - to serve the women in India.

The delegates participated with keen interest in the workshops on novel methodologies of postpartum insertion of IUCDs and about implants. The panel discussions and lectures deliberated on a host of issues related to contraception encompassing oral and injectable contraceptives as well.

This conference will be a milestone and an important landmark in making a great beginning for the following -

1. Online data on knowledge, attitudes and practices related to IUCD and safe abortions are being gathered and analysed. Participate in this survey through just a click on FOGSI website.

2. A series of workshops on training in the newer techniques (implants) and revisiting the known methods and expanding the use of IUCD insertions and injectables.

3. Establishing Public private partnerships - FOGSI - GOI - NGOs. To scale up the efforts and reach the unreached and truely make an impact on reducing population and containing the Total Fertility Rate.

4. Empowering the clinicians and the women themselves for expanded use of contraception would lead us to the objective of reducing the infant and maternal morbidity and mortality achieving the dream of “Contraception and Beyond....” Dr. Hema Divakar, Dr. Ritu Joshi, Dr. Mandakini Parihar

The fight against female feticide : The Baramati rally for Save the Girl Child

Rotary International in coordination with M.P. State Government had organised a Mega Health Camp at Mandla District from 6th March to 13th March 2010.

The camp witnessed enthusiastic participation of specialists surgeons, gynecologists, medical assistance teams and volunteers from Rotary and other NGOs, who served record number of patients in 7 days.

In this Camp around 50,000 patients were registered for medical consultation, in which total 49634 patients were given free treatment in the OPD. Besides 2088 major surgeries from all specialities were performed at 4 different Hospitals.

FOGSI Jabalpur Doctors from Private and Government sectors along with Post graduate students gave their skilled services at Mandla. About 5000 patients received OPD treatment, and about 80 major operations were performed at Katra Mission Hospital, Mandla. Majority of the operations were Hysterectomies (75) done for Prolapse Uterus, Fibroids, DUBs and Ovarian Tumors.

A team of Doctors had screened about 1000 patients for cervical cancer. PAP smears were done, 100 cervical biopsies were taken and 560 patients of age group above 40 were screened for HPV.

It was absolute dedication, self sacrifice and a keen sense to take care of poor, needy, illiterate, downtrodden section of society by the JOGS team of doctors, which made this camp a success. The postoperative care was efficient and no complications were reported.

Dr. Sanjay Gupte, President FOGSI 2010 and Jt. Secretary Dr. Girija Wagh attended the closing ceremony on 13th March at Mandla. Both dignitaries were honoured and felicitated along with MP State Vidhan Sabha Speaker Shri. Ishwar Das Rohani and Rotary Officials.

Speaking on the occasion, Dr. Gupte, stated that JOGS Obs & Gyn Society members were setting an example in Reaching the Unreached in the real sense after reviewing the activities at the camp. In his address speech at Mandla he emphasised on how to reduce maternal mortality in India, to reduce gender discrimination, and to improve patient doctor relationship. He appreciated the efforts of Rotary International, and all the doctors who made the camp successful.

Later, at Jabalpur Dr. Gupte and Dr. Wagh had a meeting with the Press along with Jabalpur Society President Dr. Swaraj Naik. He highlighted the theme Reaching the Unreached and various forth coming issues.

Dr. Gupte also interacted with the JOGS Obs & Gyn Society members. He delivered his talk on “Recurrent Pregnancy Loss - Where Are We Today”. Dr. Girija Wagh talked on “Management of Hypertensive Disorders of Pregnancy - The Recent Trends”.

Dr. Gupte aunched the 1st issue of “SPARSH - The Healing Touch”, the Magazine of JOGS Obs & Gyn Society started by Dr. Swaraj Naik. The members were overwhelmed to have their FOGSI President Dr. Sanjay Gupte and Jt. Secretary Dr. Girija Wagh to grace the occasion.
