Definition
The failure modes and -analysis (FMEA, engl: Failure Modes and Effects Analysis) is
•a formalized, analytical method
•for systematic collection and avoid potential errors in the development of new products
•as well as in the planning of manufacturing and assembly processes.

Systematisches Vorgehen
Das Produkt oder der Prozess werden in einer Top-Down-Vorgehensweise in einzelne Komponenten untergliedert.
Das systematische Vorgehen wird dabei durch die Verwendung eines Formblattes unterstützt.
Risikoanalyse, -bewertung und -minimierung
Der Ablauf der FMEA gliedert sich in Risikoanalyse, Risikobewertung und Risikominimierung. Letztere muss solange wiederholt werden, bis die Risikoprioritätszahl unter einem definierten Grenzwert liegt.

Mit der Risikoprioritätszahl wird ein Maß für das Risiko festgelegt. Je höher die RPZ, desto höher das Risiko.
Die RPZ ergibt sich aus der Multiplikation der jeweiligen Bewertungspunkte für Auftreten (A), Bedeutung (B) und Entdeckung (E) der Fehlers:
RPZ = A * B * E
Diese Berechnung ist für alle Fehlerursachen durchzuführen.
Die Risikoprioritätszahlen geben einen Anhaltspunkt für die Reihenfolge der anschließend durchzuführenden Verbesserungsmaßnahmen im Rahmen der Risikominimierung. Fehlerursachen mit einer hohen RPZ sollten zuerst betrachtet werden.
Darüber hinaus sollten unabhängig von der RPZ auch die Einzelbewertungen betrachtet werden:
hohe "A"-Werte: Fehler treten häufig auf. Solche Fehler müssen vorrangig beseitigt werden.
hohe "B"-Werte: Die zu erwartende Verärgerung des Kunden ist besonders groß. Es sollte über eine konzeptionelle Änderung nachgedacht werden.
hohe "E"-Werte: weisen auf konzeptionelle Schwachstellen hin
In der Praxis werden oft nur Fehlerursachen mit einer RPZ größer als 125 oder Einzelbewertungen größer als 8 als besonders kritisch angesehen. Aufgrund der Subjektivität der Risikoprioritätszahlen ist von dieser
Vorgehensweise aber abzuraten! Unter dem Gesichtspunkt einer Null-Fehler-Strategie stellt jeder Fehler eine Beeinträchtigung der Produktqualität dar und sollte deshalb vermieden werden.

Basically: Fault avoidance measures are preferable to missing rent nationwide!
i.e. Quality is produced not merely achieved!
