The Laborist: An Emerging Healthcare Provider Obstetrics is a healthcare specialty focused on childbirth. The majority of the patients will most likely be hospitalized for a short period of time. At the same time, the provision of obstet- rical care is increasingly complex. A key ele- ment of this practice which continues to evolve is the role of the laborist. The primary function of the laborist is to focus on inpatient manage- ment of women during pregnancy. Using this model, pregnant women would be cared for prior to childbirth by communitybased provid- ers. When the woman enters labor, she is ad- mitted to the hospital to be cared for by an interdisciplinary healthcare team including the laborist. After returning home, the woman will seek care by her community-based provider. (continued on page 4)

Infant Nutrition during a Disaster By Marie L Bosco, BSN, RNC, IBCLC

The American Academy of Pediatrics (AAP) has chosen to address infant nutrition during a disaster. The state-ment addresses breastfeeding and other options when faced with an emergency situation. The AAP statement emphasizes that mothers and their infants should re-main together in a disaster. Human milk is, after all, the cleanest and safest food for an infant. It protects against infectious disease through the enteromammary pathway which allows for mothers to produce antibodies specific to disease in the immediate environment. Use of formula during a disaster could pose significant problems because water may not be clean for mixing formula and cleaning bottles. Additionally, there may not be refrigeration to preserve formula, or it may become contaminated.

For these reasons breastfeeding remains the optimal choice for infant feedings in a disaster. Women who have weaned may successfully regain their milk supply with proper manage-ment. If exclusive breastfeeding is not possible for any reason, donor milk is the best next choice. If donor milk is not an op-tion, ready to feed infant formula should be used.

Prescrib-ing has become a major part of the role of advanced prac-tice nurses while at the same time, prescribing has become more complex and polypharmacy is more prevalent with the possibility of adverse interactions. Thus, a course of this caliber is critical to the preparation of advanced prac-tice nurses.

As healthcare consumers, we face the threat of antibi-otic resistant infections. And as healthcare practitioners, we face patient pressure to prescribe antibiotics, often from self-limiting viral infections. Recent complications continue to emerge now that some grocery store phar-macies offer free antibiotics. With this promotion, the person presents an antibiotic prescription that will be filled by the pharmacy without cost to the consumer. While waiting for this to be filled, the anticipation is that the patient will shop for analgesics, cough medicine, juice, ice cream, soup, or other such items that found in the grocery store. In turn, the profit from customers pur-chasing these products helps to compensate for the cost-free antibiotic prescription. One of the major issues to consider is whether or not the availability of free antibiotics will encourage patients to ask for these prescriptions. The concern with this is that providers may yield to these requests and prescribe an antibiotic when not warranted. To date, little study has been done on this issue. At the same time, in-creased antibiotic use is one of a number of factors in the burgeoning problem of resistant pathogens and few new antibiotics are currently in development.

In fact, the Infectious Diseases Society of America (IDSA) suggests that instead of offering free antibiotics a better public health strategy would be for retail outlets to provide free influenza vaccinations.

Who We Are…

To my family, friends and colleagues: I took on the challenge of completing the Disney Half Mara-thon (13.1 miles) on January 10, 2009 as part of the Ameri-can Stroke Association’s Boston Training team to End Stroke. I did this in honor of many people whom I love, some of whom have had strokes, including both my grandmothers, my dear friend David Tibbetts, and my Uncle Ed. I also did this in honor of my patients who have had strokes, particu-larly Mrs. O. and Mrs. R. You, the members of the NP community, supported me in this endeavor with your encouragement and generous finan-cial support. Together, we raised more than $2,000 to sup-port the American Stroke Associate’s research and advo-cacy efforts. I spent a few moments prior to the event think-ing about our stroke heroes and carried this with me during the event. The weather was perfect for a Bostonian taking on a half-marathon; it was cool, and dry. At our 6am start the tem-perature was a mild 50 degrees warming to the low 70s by end of the race. I had trained well, something that helped significantly given that I came down with a horrible cold a few days before the event. (By the way, I participated in the 2007 Disney Half Marathon with a terrible cold. History re-peats itself). While my time was slower than I had antici-pated, I completed the event within the time restrictions. I even felt like I had a time along the way to work with a few first time half-marathoners who needed a bit of encourage-ment to keep pushing on. Additionally, I met a critical care nurse along the way who is thinking of becoming a nurse practitioner so, of course, I couldn't resist some recruiting for the NP profession!

I did not look at anything else (i.e. school books, notes, other exam prep books) and heard Margaret in my head repeatedly during the exam. Thanks for these great products!

 Question: I am an NP who, due to a number of professional and personal reasons, has not practiced in more than 10 years. I would really like to get back into NP practice but am not sure where to start. Do you have any advice?

Answer: I hear this question on a regular basis from NPs who wish to return to practice. While each per-son's issues are unique, here's my input with some recommendations for products and programs that my company offers which have worked for many nurse practitioners.

• Take a pharmacology course (i.e. an in-depth, 45 hour pro-gram) so that you can familiarize yourself this critically important part of our practice. • Investigate what you need to do to become nationally certified again. I strongly advise that you take a live NP review course because this will provide a great overview of what new graduates are expected to know. Additionally, this will serve as a start-ing point for your study and will help you prepare for your state boards. • If you are many years removed from all nursing practice, please consider taking an RN refresher course. This will help you brush up basic nursing skills.

• Volunteer in a free clinic. Working side by side with RNs, NPs, PAs and MDs you will be able to see firsthand what type of practice interests you. Also, ask colleagues if you can shadow them for a half day or so to get another view of current practice.

• Contact your local NP program to see if you can return for a clinical rotation as an independent study. You will need to pay tuition for this, but the school will provide you will faculty input that will be very valuable.

• Consider returning to school to retool your NP career. For example, perhaps you always wanted to practice in woman's health. Now might be the time to go back to school as a post MS or DNP student to do this.
