Pendahuluan

Infertilitas meliputi 13 – 18% pasangan suami-isteri

Infertilitas pria :

- Infertilitas tanpa gangguan spermatogenesis

- Infertilitas karena gangguan spermatogenesis1. Penurunan androgen2. Tanpa penurunan androgen infertilitas idiopatik

Androgen bekerja dengan berikatan pada reseptor androgen di dalam sel terget.

Reseptor androgen adalah reseptor intrasel yang bekerja di dalam nukleus.

Kompleks reseptor-androgen mengatur ekspresi gen tertentu di dalam sel target dengan menstimulasi aktifitas transkripsi.

Reseptor androgen dikode oleh gen AR pada lokus Xq11-12, terdiri dari domain transaktivasi, DNA binding, hinge dan hormon binding.

Exon I gen reseptor androgen memiliki daerah pengulangan CAG yang sifatnya polimorfik.

Polimorfisme pengulangan CAG pada gen reseptor androgen menyebabkan variasi poliglutamin yang menyusun domain transaktivasi reseptor androgen aktifitas reseptor dalam mentransduksi sinyalandrogen.

Polimorfisme pengulangan CAG gen reseptor androgen telah diketahui berasosiasi dengan patogenesis penyakit tertentu.

Masalah :

Reseptor androgen menentukan efektifitas stimulasi androgen pada pengaturan spermatogenesis.

Polimorfisme pengulangan CAG pada gen reseptor androgen mempengaruhi efektifitas stimulasi androgen seperti yang terjadi pada penderita SBMA (Kennedy disease) dan karsinoma prostat.

Spermatogenesis sangat mungkin dipengaruhiperubahan pengulangan CAG pada gen reseptor androgen, karena spermatogenesis tergantung sepenuhnya pada efektifitas stimulasi androgen.

Beberapa hasil penelitian menyimpulkan adanya hubungan yang inkonsisten antara polimorfisme pengulangan CAG dengan kegagalan spermatogenesis pada etnis tertentu.

Perlu dibuktikan adanya hubungan polimorfisme pengulangan CAG pada gen reseptor androgen dengan gangguan spermatogenesis pada pria Indonesia.

Manfaat :

Memahami patofisiologi gangguan spermatogenesispria infertil khususnya pada kasus infertilitas idiopatik.

Memberi sumbangan pemikiran pada penanganan kasus infertilitas idiopatik yang relevan dengan abnormalitas reseptor androgen.

CAG repeat number of the AR gene in oligozoospermic/azoospermic men and normozoospermic men. The CAG repeat fragments of the AR gene were amplified by PCR, electrophoresed in 6% denaturing gel polyacrylamide and stained with silver nitrate. Differences of the CAG repeat number among samples were shown. Slot 1 to 20 were sample with 21, 25, 22, 25, 25, 28, 22, 22, 25, 29, 30, 22, 25, 25, 19, 28, 25, 23, 23, 31 CAG repeats respectively

Kesimpulan

Pada pria Indonesia :

1. Jumlah pengulangan CAG gen reseptor androgen pada pria oligozoospermia/azoospermia berbeda (lebih besar) dari pria normozoospermia.

2. Jumlah pengulangan CAG gen reseptor androgen tidak berkorelasi dengan konsentrasi spermato- zoa baik pada pria oligozoospermia maupun pria normozoospermia.
