Summary• Falls are the most common incident notified in IIMS
• Recommended screening tool: Ontario Modified Stratify
• Important to identify and put in place strategies to
reduce the patients risk factors for falls eg providing a
safe environment, strategies for safe transfer and
mobility and review medications
• Regardless of age an altered cognitive state is the
greatest risk factor eg confusion, paranoid, delirium etc
