- “Elderly Health Assessment Pilot Program” launch in 2013-2015
- Participant: 2,000 elderly people aged 70+
- Outcome:
90% did not receive comprehensive health assessment in past 3 years
Participants are newly identified :
Hypertension – 149 (7.5%)
Diabetes Mellitus – 74 (3.7%)
Hyperlipidemia – 501 (25.1%)
Colorectal cancer screening – 184 (9.2%)

Project nature : Tripartite Partnership to Give Family Support

Target group: Elderly with chronic illness, caregivers, residents of Wong Tai Sin District
Total no. of participants: 5577
Objective:
1. To develop cost-effective support network for the elders through integrating social resources
2. To establish partnerships among medical service, social welfare service and the community, so as to provide comprehensive medical care that enable elderly to manage their health conditions
3. Build up an atmosphere of healthy society and caring neighborhood in the community

• to provide outreach medical consultation and treatment services by a multi-disciplinary team (e.g. pain therapy , Fall prevention, cognitive training, nutritional guidance)
