CareWhat is Expecting Success?• Multi-hospital collaborative Learning Network designed to

improve cardiac care for Blacks and Latinos

– Using quality improvement and evidence-based practice for inpatient and outpatient MI and HF care

– Monthly standardized reporting of 23 performance measures by race, ethnicity, language; surveys of patient experience at 3 points

– Runs 29 months, with seven in-person meetings, monthly conference calls

– Designed to provide “real-time” lessons to industry, with external evaluation by NYU
