eUT Help – TQA TQA Process Additional guidance on the TQA process is available via the eut web-site Help Documents sub-tab. These documents have a great deal of reference information on TQA. It is important that EPT personnel review them at least once to fully understand and appreciate the TQA process. Trainers should discuss this with personnel during training rollouts. There are multiple documents embedded within the TQA Guidelines, so you may need to allow a few seconds after opening to see these additional PowerPoint and Word documents, as this system runs on the Sunbury server and the refresh rate is slow from some locations, such as Houston Entering TQA via the Job Reports Tab in eUT There are two ways to get into the TQA system. Entering TQA via the Job Reports Tab in eUT There are two ways to get into the TQA system. The first, and often the easiest, is to first click on the red job reports tab. This will display as shown in Figure 1:

Note: for TUL’s, TL’s, and TQA champions, there are additional buttons available for views at the larger team levels. The typical EPT task leader will not need to be aware of these functions, but they allow searches and displays by teams and technology units rather than by individuals. This screen will bring up all of the eUT work orders in which you are assigned as the job leader. You will need to verify that the proper month and year is shown and then click the view selected report button if this is not the most current month. For updating older jobs or TQA’s, you may need to put an earlier month, as jobs that have been closed out for several months sometimes fall off of this list. You can also put in another job leaders name to the left if you are doing this for someone else or entering a task against another job leaders work order. By scrolling through the screen, you can quickly verify whether there are any gray colored 0’s (zero’s) under the Green/Pink TQA Key. If this is the case and you or someone else has charged any hours or costs to this job, then this qualifies for the need for at least one TQA task. Until time or expenses are incurred, there is not an expectation that you know enough of the scope of the job to determine whether it could potentially fall into the zero tolerance or criticality assessment areas. By clicking on the 0, will take you to the Add Task screen, this will have the selected job number pre-populated and clicking next will bring up the Add New Task pages with the appropriate information, which saves a few keystrokes in entering this yourself. The remainder of the steps to completing a new TQA will be similar to those described later in entering a new task. If there is already a number 1 or higher in a green box, then there is already 1 or more tasks entered against this job number and they are non-zero tolerance. If there is a number 1 or higher in a pink box, than there is one or more tasks entered against this job number that have been defined as zero tolerance. By allowing the curser to rest over the number for a few seconds, a small pop up box will display and will show how many tasks are open, how many tasks are closed, how many total tasks exist for that job number, and also how many zero tolerance tasks. By clicking on a number other than zero, this will take you to a screen that shows all tasks initiated by anyone for this job. Also on this page is search functionality to review other all other tasks within the system other Figure 2

Proper use of the EPT Funding Model will allow recovery of time for many of the small jobs in which advice is given by e -mail, phone, or other means, such that a TQA will then need to be documented anyway. Note: for TUL’s, TL’s, and TQA champions, there are again additional buttons available for views at the larger team levels. By clicking on a highlighted task title, this will take you to another screen in which this task can easily be updated (steps covered elsewhere in manual).

Entering TQA via the TQA Tab in eUT The second way to get into the TQA system is to click on the TQA tab. This will display a screen as shown in Figure 3 :

This screen either allows a person to add a new task(second sub-tab), or review/edit a task in section 2. If updating in step 2, there are numerous means of finding the task to be updated based on selection from the drop boxes. By selecting and then clicking the show tasks button (after a few seconds), the jobs and associated tasks will then display further down on this screen. By clicking on the appropriate task, a screen will display that will allow viewing, updates, or closing the task. If a new task is to be entered, then clicking on the Add Task button will bring up an additional intranet screen and display as shown in Figure 4:

Note that a pop up box may come up if you have previously partially filled out a task and closed this without saving the information. If you wish to continue this particular task, click on the OK button. Otherwise, cancel and proceed. Depending on the number of individual screens previously populated, you may be able to click on the next button more than once to progress through, as some data is now entered on individual screens. Similar to the previous TQA website, simply search on the category of information you have readily available, such as a word in the job title, the job leader name, or by a particular business unit location, and click on the submit button. By clicking on the TQA icon at the beginning of the job row will bring up the first page in the TQA Task Add process Figure 5

User should complete all the task header details. The Task Leader will default to the user logged on but an alternate can be selected from the dropdown. The red * indicates a mandatory field be populated before the continuing. The stoplight at the top left will then turn from red to green and the statement behind it will change from “All required data has not been entered, cannot proceed.” to “All required data has been entered, ready to proceed.” The Next button will then become available. This stoplight will work similarly through the remainder of the TQA website until the TQA task is updated. Clicking on the Next button will display the Zero Tolerance page Figure 6.

Figure 6 Each individual task that is entered must first be checked against criteria of whether it fits the zero tolerance definitions or not, examples displayed in Figure 6. If so, that category should be checked at the button to the left of the zero tolerance description. If it is not zero tolerance, then the non-zero tolerance category is selected at the bottom of the page. Each Zero Tolerance criteria has a hyperlink to individual documents on the EPT HSE website. The examples available via the links show both what IS zero tolerance, and what IS NOT zero tolerance, to assist EPT task leaders in determination of zero tolerance is applicable. RACI charts are also hyperlinked. These RACI charts specify the steps, Responsibilities, Accountabilities, who to Consult, and who to Inform, in the event that a task falls within any zero

tolerance category. The actual format of these RACI charts varies somewhat between the various zero tolerance categories, but still provides additional guidance to the task leader in necessary quality assurance steps and review to properly access and minimize risk and verify the receiving Business Unit personnel, Team Leaders, etc. are properly involved. A Key Contact is identified for each zero tolerance category for any further guidance After selection of one of the zero or non-zero tolerance categories, the red stoplight will turn green and the Next button becomes available. Selecting Next brings up a screen as displayed in Figure 19.

On this screen, before selecting a Criticality Method the user has the option of specifying that the task does not require an assessment. In this case the No

radio button is clicked and the user enters a commentary as to why the assessment is not needed. Else, one of the criticality assessment methods must be selected from the dropdown. Facilities always use the General criticality assessment. However, Subsurface and Wells is further broken up into Drilling, Exploration, Subsurface, and Well Performance. After selection of one of these categories, the screen will refresh to the display shown in Figure 8.

Each of the five categories of criticality assessments has their own unique set of criteria, so selection of the proper category is essential. Some also have more than 5 individual criticality assessment areas. These are individually listed in an attachment within the”TQA Guidelines” hyperlinked document. The display in

Figure 8 is for the Exploration category. On this screen, that there are several mandatory fields, as shown by the red *. Thus, each question under the criticality assessment is mandatory. The TQA Decision Tree (available from the Help Documents sub-tab) is very helpful in the determination of whether the criticality assessment is required. On completion of the assessment the Next button will allow the user to move to the final screen, Figure 9

is closed). If not zero tolerance, and no criticality assessment is required, then it is very likely it can be changed from the default of Open to Complete at the time of entry. This also avoids forgetting to come back and close the TQA later. The SOC question should be checked No or left blank, as this is an audit check for later. Assurance in rigorous entry of data can only be done through some random spot checks of TQA tasks to verify people are using and populating the system properly. Many of the zero tolerance items and a random sampling of non-zero tolerance and degrees of criticality may trigger an audit via the SOC system. This button allows documentation and reporting. From Figure 7, if the criticality assessment was required (or there was a zero tolerance category chosen), then the stoplight change from red to green will not happen until a number is chosen for each category of criticality assessment. The comments section under the criticality assessment is optional on this screen. The date will automatically populate for the date this task is updated. The criticality rating will add up all the numbers to a current rating. Then the Next button can be clicked. Several fields will need to be completed before the complete button becomes available. The information can be saved at any time with the Save button. The way the TQA rules (and TQA website logic) work, only a self-check is required with 5 or less as a criticality rating. Thus, there are no mandatory fields. However, if this number is above 5, then a reviewer name, date, and details are required in order to complete the TQA (essentially, they become mandatory fields). Uploading documents are optional, but recommended when e-mail or other electronic data can easily be dragged and dropped or the browse button used to import such documents. Comments on the review are also optional. It is particularly important for higher criticality and any zero tolerance TQA’s be kept current and closed with proper documentation, preferably well before the job is closed by the BU in eUT. TQA champions will be monitoring this as one of the key indicators of the “health” of the TQA system.
