· Thesis consultation Letter of Assignment

· 4 copies of Thesis (for the Thesis Test) must be submitted to each examiner after test schedule is released

· A copy of Thesis Consultation Report

· 2 copies of National Indentity Card

· Filling out the SKPI form (academicreport.umm.ac.id)

PHOTO REQUIREMENTS:

· Photo must be taken currently and printed on doff paper along with its soft copy.

· To wear suit and tie, plain shirt (for male), no shirt is allowed, neat and tidy hair cut.

· It is strongly suggested to wear formal and non-transparent (for female), no hair and/or hejab accessories.

· Photo must be in formal pose

· Photo background : light blue

· Not wearing glasses

· The photo must shows ears of the applicant, no photo manipulation.
