On page 10, in the numerator of equation 16, K 0 should be followed by (q n ). That is, parentheses should be around q n . On page 11, in equation 19, the r D 2 squared term should be multiplied by the letter p. Also on page 11, in table 1, the definition for W D should have r p (not r c ) in the numerator. On page 12, in the second column, five lines from the top, the F symbol should not have a bar over it. It should be either F or F. On page 19, in equation 25, the 1n should be the symbol for the natural logarithm (1n), here and in several places on pages 36–38. The 1 was not italicized whereas the n was. On page 37, in equation 26, the second left parenthesis (following the + sign) should be removed. On page 38, in the title of table 10B, in the second line, ln should be italicized to read “variance of 1n K r , J.” On page 43, in the title of table 15, in the second line, the word “adjustable” before “saturated thickness” should instead be “fixed,” to read “fixed saturated thickness.” We apologize for the inconvenience these errors may have caused.
