Dear Mr. Huong,

After checking your site on 26 September 2015, we noted that your corrective actions from Weekly

Environmental Report by your l::O was not strictly complied. Many environmental issues still remain

on your CW1 C work package. Besides remaining issues, we highlighted some new outstanding

ones (e.g. air pollution from cement transfer in the river); thus we would highly recommend you to

enforce your EO presence and periormance to take corrective action efficiently.

• Good practice: The road was clean frequently after many spoils felt down during dumping and

transporting.

• Metal waste was stored next to the office and worker accommodation without a proper storage; this

material should be stored in a designated area for metal waste with barricade and (label) notification

broad as reference. This issues still remain from months ago, contractor must take quick action on this

issue.

• Sand materials locates in improper place. This sand block the access way for the workers in the

laboratory. Contractor must remove and transfer sand to designated stock yards area for material and

open the access road for workers.

• The excavated soil dumped on the approved/designated storage was not flattening as our requirements.

• Excavated soil was not only located on site without any notification board for temporary storage, but

also dumped in illegal local households without any permission. Contractor must follow project

requirement for excavated soil, this illegal soil dumping quantity must be taken out of local households

and store in the designated area.

• There was overspreading excavated soil on site without any proper storage. This soil must store in

designated area for disposal area. For reuse purpose, this soil should store in a temporary place with

barricade and notification broad.

• The huge dust pollution from cement gas from damaged pipes during transferring this materials from

barge on the nver to the truck. After transfernng, the remammg gas st1ll flow out w1thout any control

from workers/truck drivers. Contractor was violated the requirement of air emission which use

disqualified materials resulting to air pollution. Please request to re-training of environment and safety

issue for those relevant workers and re rt the corrective actions to DDIS.
