Role of the Position
To Support older people improve their function, independence and quality of life, make gains in their physical, emotional and social wellbeing and live as autonomously and independently as possible.

Role Responsibilities The following is a summary of specific duties for this role.
In Home Care
Provide in-home care assistance and social support to frail aged seniors and people with disability and their families including but not limited to shopping, domestic assistance, social support, personal care, meal preparation and meal eating supervision, transport, assisting with mobility, toileting and showering.
At all times strictly adhere to the clients’ support and medication plans as well as any other instructions in their “in home file”.
At all times ensure all services are provided in line with the Reablement and Wellness Approaches.
Help individuals maintain a safe and comfortable home environment.
Ensure your private vehicle is insured, roadworthy and maintained to the WA Department of Transport’s vehicle standards, regulations and rules.
Social Support
Provide appropriate social support activities that includes varied leisure, recreation and life skills opportunities for people with disability and frail aged participants.
Ensure all activities are of the highest quality utilising Person Centred Service models in line with the principles of Social Inclusion, Wellness and Reablement.
Ensure participants and their families are partners in all aspects of the service. Always communicate in a manner that reflects sensitivity to their individual needs and reflects the ethos of the Town of Bassendean.
Ensure all social program are delivered in an energetic and enthusiastic manner that embraces and supports independence and appropriately engage participants with varying support needs and challenging behaviours
Ensure the safe and efficient transportation of participants at all times.
Take active responsibility for setting up activities, equipment, resources, consumables, and completion and documentation of a high level of quality of hygiene and safety practices.
Within professional boundaries, support, encourage and direct volunteers to assist with the activities and outings for the benefit of clients
Assist with activities of daily living and personal care of clients eg. Toileting, assisting with meals, washing and transferring etc.
Maintain and update client details and assist in the ongoing review of needs i.e. functional ability, change in physical, emotional or family circumstances, change in care needs and inform relevant Coordinator of progress.

General
Comply with TOB SDS funding agreements and guidelines, the Aged Care Quality Standards and SDS and The Town’s Policies, Procedures, Practices and the Code of Conduct.
Contribute to a harmonious work culture, developing strong working relationships with colleagues for the benefit of clients
Develop strong communication networks and professional relationships with clients and their families/carers, the community and internal and external stakeholders to ensure the best outcomes for the Town and all concerned.

Ensure privacy and confidentiality is maintained and clients treated with dignity and respect with a focus on choice and control.
Be proactive in promoting effectiveness and efficiency within the Business Unit through problem solving and collaboration with team members and contributing to continuous improvement strategies and outcomes
Ensure that both written and verbal communications with the public is of the highest standard
Complete required administration and record keeping relating to services, clients and time management requirements ensuring accuracy of information provided.
Attend and participate in staff meetings and training development opportunities.
Provide relevant feedback on clients' progress and the appropriateness of the care plan to Coordinators and the Team Leader on a regular basis, particularly any changes in physical, emotional or family circumstances.
Carry out other duties as required by the Manager Seniors and Disability Services.
Maintain, clean and ensure weekly checking, of Seniors and Disability Services vehicles and centre, ensuring it is clean and welcoming for all as required.

Selection Criteria The Town will assess applications and suitability against the above role responsibilities and leadership capability requirements.
Possession of Tertiary Qualification, Certificate III in Community Services (Aged Care or Disability Services), Therapy Assistant TAFE Certificate or equivalent.
Essential
Previous knowledge, experience and skills in providing in-home care assistance and personal care provision to seniors and people with disability.
Good understanding of factors affecting seniors, people with disability and their families.
Exceptional interpersonal and communication skills with the ability to communicate with seniors, people with disability and their families.
Ability to facilitate and implement activities suitable for frail aged seniors and people with disability with flexibility to meet clients’ changing needs.
Knowledge and proven ability in manual handling and infection control techniques.
Proven knowledge of Occupational Health and Safety practices and hazard identification.
Ability to work effectively in a team and develop positive relationships with team members.
Current First Aid Certificate.
Satisfactory National Police Clearance Certificate.
Current WA “C” Class Driver’s Licence.
Desirable
Knowledge and understanding of dementia.
Previous working experience with people from Aboriginal and Torres Strait Island and culturally and linguistically diverse backgrounds.

Employment Conditions
Pre-Employment Medical Prospective employees are required to undergo a pre-employment medical examination (at Council’s expense).
National Police Certificate
Prospective employees are required to provide a National Police Certificate (issued less than three months ago). If you do not hold a current National Police Certificate, the Town will make this application for you (at Council’s expense).
Working with Children Check (WWCC)
If a WWCC is an essential requirement of the position, you must provide a copy of your current card prior to commencing employment.
Psychometric Testing Prospective employees may be required to undertake psychometric testing (at Council’s expense).
Drivers Licence If a current driver’s licence is an essential requirement of the position, you must provide a copy of your current card prior to commencing employment.
Probationary Period
A three (3) month probationary period may apply to this position. Reviews are conducted at 6 and 10-12 weeks following commencement. Upon successful completion, appointment will be confirmed.
Pay Days
Employees are paid on a fortnightly basis by direct credit into a bank, building society or credit union of their choice. Pay is deposited into accounts on the Thursday following the end of the pay period.
Superannuation Council will contribute 9.5% of salary to a complying superannuation fund nominated by the employee. Salary sacrifice options are available along with matching employer employee contributions of up to 5%.
Annual Leave Four (4) weeks per annum plus 17.5% leave loading.
Christmas Closure Days Employees are entitled to two additional days to be taken as part of the Town’s Christmas closure period (between Christmas and New Year). * Conditions Apply
Personal Leave For each year of service, an employee is entitled to 10 days (pro-rata) of paid personal/carers leave.
Long Service Leave Employees are entitled to 13 weeks long service leave in respect of each 10 years’ continuous service. Eligible employees are able to access their Long Service Leave entitlements pro-rata after seven (7) years continuous service. *Conditions apply
