    New for 2019!Plan Changes for 2019Aetna/Innovation Health• Starting January 1, pre-certification will

    not be required for complex radiology, such as MRI and CT scans. These services will be subject to a $75 copay when provided in-network (deductible applies).

    • LabCorp will now be a participating lab provider in 2019. Laboratory services can be performed at either Quest or LabCorp and be considered in-network.

    CVS/Caremark• New programs coming soon to support

    individuals with diabetes as well as changes to covered glucometers and test strips.

    • Check the CVS website (https://info.caremark.com/fcps) for any changes to formulary medications or prior authorization requirements.

    Kaiser Permanente• Visit limits are changing for physical,

    occupational and speech therapy. Currently, these services are limited to 90 consecutive days of treatment. In 2019, participants will be eligible for 60 visits (per condition) for each therapy.

    Minnesota Life is Now Securian FinancialMinnesota Life, the administrator of our Group Life Insurance Program, has adopted the name of its parent company, Securian Financial. There is no change to life insurance benefits or coverage, which will continue to be underwritten by Minnesota Life.

    Liberty Mutual Is Now Lincoln Financial for STD and LTD BenefitsLincoln Financial recently acquired Liberty Mutual, the third-party administrator for Short-Term Disability and Long-Term Disability ben-efits. You will soon be seeing rebranding of the Liberty Mutual name to Lincoln Financial. There will be no change to the short-term disability and long-term disability benefits or coverage.Liberty Mutual will continue to administer Workers’ Compensation claims. The claims reporting line remains the same: 1-800-524-0740.

    New Diabetes Management ProgramCVS will roll-out a new diabetes management program which uses advanced technology to engage members and improve health outcomes by providing high-touch personalized support. The comprehensive program includes a free smart glucometer that uploads glucose measurements and provides member-specific educational content. The program also offers personalized health coaching by a diabetes educator and pharmacist as well as two (2) free Minute Clinic diabetes monitoring visits. Participants eligible for this new program will receive mailings this fall from CVS.

    Introducing OptumNew Flexible Spending Account Vendor for 2019We are excited to announce that Optum Bank® will be the new plan administrator for the Flexible Spending Account (FSA) plan, effective January 1, 2019. This change will provide FSA participants with enhanced servicing, including the added convenience of a debit card that can be used for eligible health care and dependent day care expenses. The debit card permits immediate payment of eligible FSA expenses, however, participants may also continue to submit electronic claims. Reimbursements will continue to process daily.Also for 2019, the maximum annual contribution for the Health Care FSA will increase from $2,600 to $2,650; the Dependent Care FSA maximum continues at $5,000. The minimum annual contribution for both accounts will increase from $120 to $250. Participants in the Health Care Flexible Spending Account (FSA) will continue to be able to carryover $500 in unused funds to the following plan year.

    For Current FSA Participants:If you currently participate in the Health Care FSA or Dependent Care FSA with WageWorks in 2018, please read the following important information:• To allow for transition of claims processing from WageWorks to Optum, there will be a

    blackout period in early January, when 2018 claims cannot be processed. We encourage you to submit your 2018 claims to WageWorks no later than January 6 to avoid the blackout window. Look for additional communications about submitting 2018 claims for processing later this fall.

    • Unused 2018 health care FSA funds will be rolled to Optum after the blackout window. You will be notified when the rollover processing is complete, and your funds are available.

    • Remember you must re-enroll to participate in 2019! Current participants are not automatically re-enrolled for next year.

    Consider Enrolling in an FSA for 2019!If you aren’t currently participating in an FSA, you may want to consider enrolling in 2019 as a way to reduce your out-of-pocket expenses. Depending on your tax bracket, you can save 20-30% by paying for these expenses with pretax dollars.

    2019 Premiums

    1 All benefits-eligible employees in active status pay the same rates, regardless if part-time or full-time. Employees have deductions taken September through June. 2 Employees and their spouses who both work for FCPS in benefits-eligible positions are eligible for a spousal discount on their health and dental insurance. The FCPS Spousal Rates reflect an employee contribution of 20% of total premium for medical and dental coverage. If you are eligible but not currently receiving this discount, complete the FCPS Spouse Health Plan Deduction form (HR Form 134) and submit during Open Enrollment. As a reminder, employees are required to notify the Office of Benefit Services within 30 calendar days of any event that would cause an employee to qualify for (or cease to be eligible for) the discounted rate; including marriage, divorce, termination of employment, or commencement/termination of a leave of absence.3 COBRA premiums apply to employees/dependents who are eligible and have elected COBRA continuation coverage.     Continuing Programs for 2019Employee Assistance Program (EAP)Work-life Balance Support Did you know that your Employee Assistance Program (EAP), Guidance Resources, can help you with your to-do list and give you back some time? In addition to providing confidential counseling as well as legal and financial resources, Guidance Resources also has work/life specialists who can act as your “personal assistant”. They will research your needs on topics, such as eldercare, childcare, pet care, vacation/event planning, college tuition, moving, and much more. Whether your request is small or large, they can help save you time and effort! Guidance Resources can also help you jump-start your personal wellness plan. Their telephonic health coaches are available to help you manage stress, start a fitness program, eat healthier, lose weight, and/or sleep better. The EAP services are free, confidential and available 24/7 for you and your household members by calling the FCPS dedicated line at 855.355.9097.     Don’t Forget to Take Your Health Assessment and Earn a $100 Wellness Incentive!Help manage your health risks and save money! Employees participating in an FCPS health plan will continue to have an opportunity to earn a $100 wellness incentive for completing their health vendor’s online health assessment (HA). For 2019, you now have the entire year to complete the assessment. If you took the HA in 2018, you can still take it again in 2019 to qualify for the wellness incentive and compare how your health status has changed.The HA is a questionnaire related to different aspects of your general health and well-being that is easy to complete in about 15-20 minutes. It provides a snapshot of your current health status and includes a wellness score, so you can become familiar with any potential health risks and strategies to help reduce them. Your individualized health report may also include information on how to obtain health coaching and/or receive other health or wellness programs that are offered through your health plan.The HA is confidential! Only you and your health plan receive the individual information.     Staying HealthyFree Flu ImmunizationsAll FCPS employees can obtain a free flu immunization this fall. This includes full-time and part-time employees, as well as hourly/temporary or substitute employees currently on assignment. In addition to most schools holding their own flu clinics, Employee Wellness will sponsor several large clinics throughout the season during Open Enrollment events and at the large Administrative Centers. Appointment registration is required for these clinics. Schedule an appointment by visiting the Open Enrollment website and clicking on the flu shot scheduling link. You must present your employee ID when you receive your immunization. Family members of employees (18 and older) may also receive a seasonal flu immunization at any FCPS clinic.     Know Before You GoWhen you need medical care, you have choices! If your family doctor’s office is closed and you need care for a non-life-threatening illness or injury, consider using a participating walk-in clinic or urgent care center. You are typically seen more quickly, and it can cost you less money than an Emergency Room (ER) visit.

    Call Your Doctor or Visit a Walk-in ClinicWhen you have:• A cold• A muscle spasm• An ear infection• A sore throat

    Your Primary Care Physician (PCP) is usually your best option. However, if you cannot see your doctor, walk-in clinics provide convenient, low-cost treat-ment for minor illnesses or injuries.

    Urgent Care Center

    When you:• Cannot reach your doctor• Need care after hours or on a weekend• Need medical care today but feel safe to wait a

    few hours• Received instructions from a medical profession-

    al to go to an Urgent Care

    Urgent Care Centers cost more than walk-in clinics, but much less than the ER. These facilities are overseen by doctors, with nurse practitioners or physician assistants on site.

    Emergency Room or Call 911When you have:• Trouble breathing• Chest pain• Uncontrolled bleeding• Severe pain or have suffered a serious injury

    If you’re experiencing something that could be life-threatening, don’t wait! Call 911 or go straight to the nearest ER.

    Changes to Retiree Health Eligibility Provide Additional Flexibility for CoverageEmployees planning on retaining FCPS health coverage into retirement will want to review upcoming changes to retiree health eligibility. These updates will make it easier for current employees to meet eligibility criteria to retain coverage into retirement. The changes also allow retirees to cancel coverage and potentially re-enroll in the plan at a later date (under certain conditions) and require longer employment tenure for employees hired in the future. This provides flexibility to retirees who may wish to explore plans available through another employer or the marketplace, while responsibly addressing the growing cost of health insurance and impact on FCPS’ financial liability.Under current rules, employees who wish to retain health coverage at retirement must be covered for sixty (60) consecutive months immediately prior to retirement. If FCPS coverage is dropped in retirement, coverage cannot be reinstated.Starting January 1, 2019, current employees are eligible to retain FCPS coverage as a retiree if:

    • You are covered under an FCPS health/dental plan for sixty (60) consecutive months immediately prior to retirement; or

    • You have 15 or more years of FCPS service.In all cases, you must have coverage at time of retirement in order to maintain coverage as a retiree.Additionally, starting next year, a retiree covered under the FCPS plan may choose to drop FCPS coverage and re-enter the plan at a later date if Medicare-eligible. A retiree who wishes to re-enter the plan must also show proof of other health coverage for 12 or more consecutive months. A retiree may only re-enter the plan one (1) time. The Deferred Health Option plan will be eliminated and replaced with the re-entry rights.

    • Covered by the FCPS health/dental plan(s) at time of retirement, and– Have 15 or more years of FCPS

    • 15 or more years of FCPS service. FCPS coverage must be in effect at time of retirement.

    Retiree re-entry rules

    Note: May only re-enter type of coverage in effect (i.e., medical and/or dental) when coverage was dropped.

    Employees may drop coverage at retirement (or retirees may drop coverage after the date of retirement) and re-enter the FCPS health/dental plans at a later date if: • Meets the definition of retiree (begins drawing a retirement benefit from one of

    the FCPS retirement systems immediately upon termination of employment), and

    • Meets the requirements above to retain coverage, and• Is eligible (and any covered dependents are eligible) for Medicare when request-

    ing to re-enter the plan, and• The retiree (and any covered dependents) provides proof of other coverage for

    the preceding 12 consecutive months

    Your Open Enrollment ChecklistBefore Open Enrollment: Log into UConnect to verify your personal information is correct, such as:

    – Your mailing address and phone number – Your dependent(s) SSN(s)

    Go to www.fcps.edu and search “UConnect”. Your log-in information is your FCPS user ID and password. Note: you can update your personal information in UConnect at any time of year.

    Review your health care claims to help estimate your out-of-pocket expenses for 2019. Think about enrolling in a Flexible Spending Account (Health Care and/or Dependent Day Care) to help reduce your out-of-pocket costs by saving money on taxes in 2019.

    Become familiar with each vendor’s tools and resources, which will help you determine what plan might best fit you and your family’s needs.

    If you will be requesting coverage for your spouse and/or dependent children who are not currently covered under an FCPS plan, begin gathering legal documentation needed to add them to your coverage.

    During Open Enrollment: Review this Benefits Briefing carefully as you consider your plan choices. Take

    advantage of the many open enrollment resources available to you, including information on the FCPS and benefits vendor websites, attend a Benefits Open House, or visit the Gatehouse Administration Center.

    Log into UConnect and review your elections for calendar year 2019. Make changes if needed. Remember, Open Enrollment ends at 4:30 p.m. on Friday, November 9.

    If you participated in the Flexible Spending Account program in 2018, you must re-enroll to continue participating in 2019.

    After Open Enrollment: Review carefully the benefits confirmation statement that will be sent to your

    FCPS email account in mid-November.

    If you changed plans during Open Enrollment, look for new ID cards in late December.

    If you enrolled in an FSA, look for your Optum Bank debit Mastercard in late December.

    Employees covering spouse/dependent children, please note –FCPS must collect your spouse/dependent child(ren)'s SSN for reporting health plan enrollment to the IRS. Please review these SSN’s to ensure they are accurate. If you are requesting coverage for your spouse and/or dependent children, you must supply the appropriate legal documents to support eligibility for coverage under the plan. Go to www.fcps.edu and search “dependent eligibility” for a list of required documents.     Your Benefits and the LawMedicare Prescription Drug (Medicare D) Plan Information for covered employees/dependents age 65 or older. All FCPS medical plans include prescription drug coverage that is currently more comprehensive than the Medicare prescription drug plan. As an active employee, your FCPS medical coverage is primary to Medicare and you do not need to enroll in a Medicare Rx (Medicare D plan).     Women’s Health and Cancer Rights Act If you have had or are going to have a mastectomy, you may be entitled to certain benefits under the Women’s Health and Cancer Rights Act of 1998 (WHCRA). For individuals receiving mastectomy-related benefits, coverage will be provided (in a manner determined in consultation with the attending physician and the patient) for:• All stages of reconstruction of the breast on which

    the mastectomy was performed• Surgery and reconstruction of the other breast to

    produce a symmetrical appearance• Prostheses• Treatment of physical complications of the

    mastectomy, including lymphedemaThese benefits are subject to the same deductible and coinsurance applicable to other medical and surgical benefits provided under the plan. Refer to the summary plan documents available on each vendor's website for more information.

    Medicaid & the Children’s Health Insurance Program (CHIP) Offer Premium Assistance for Health Coverage for Children and FamiliesIf you or your children are eligible for Medicaid or CHIP and you are eligible for health coverage from your employer, your state may have a premium assistance program that can help pay for coverage, using funds from their Medicaid or CHIP programs. If you or your children are not eligible for Medicaid or CHIP, you will not be eligible for these premium assistance programs but you may be able to buy individual insurance coverage through the Health Insurance Marketplace.     If you or your dependents are already enrolled in Medicaid or CHIP and you live in a state offering premium assistance, contact your State Medicaid or CHIP office to find out if premium assistance is available.If you or your dependents are NOT currently enrolled in Medicaid or CHIP, and you think you or any of your dependents might be eligible for either of these programs, contact your State Medicaid or CHIP office at 1-877-KIDS NOW or visit www.insurekidsnow.gov to find out how to apply. If you qualify, ask your state if it has a program that might help you pay the premiums for an employer-sponsored plan.
