Introduction

The National Emergency Department Survey was undertaken for University Hospitals Coventry and Warwickshire
NHS Trust between 24 October 2016 to the 17 March 2017. Some changes to the methodology were implemented
this year, including a different sampling period (September 2016) and the sample size was increased from 850 to
1250. There are also some small changes to the content of the questionnaire with three new questions being
added; two amended and one removed.
Additionally, the survey now covers both type 1 and type 3 emergency services and some small amendments to
some other questions were made in order to make them appropriate to both types of service.
Because of these changes, comparisons between this and previous surveys are not possible.
The survey required a random sample of 1250 patients seen during September 2016. There were a number of
categories of patients excluded from the survey e.g. current inpatients and minor injury unit patients.
Response Rate
274 completed questionnaires were returned from the sample of 1250 from University Hospitals Coventry and
The final response rate for University Hospitals Coventry and Warwickshire NHS Trust was 23% (274 usable
responses from a usable sample of 1207).

Recommendations
Summary
Overall, the Trust's scores are low in many areas, with a number of scores where the Trust is in the bottom
20% of Trusts surveyed. However there are a few questions where the Trust performs within the top 20%.
Action should be taken in all areas to address the issues raised in the results.
Arrival at the Emergency Department
Recommendation: Look at ways of ensuring that patients have enough privacy when discussing their
condition with reception staff, given the scores in this area
Waiting
Recommendation: Ensure that patients are able to speak to and be examined by emergency department
staff as soon as possible after their arrival
Recommendation: Further ensure that patients are given a clear and realistic indication of how long they will
have to wait to see a doctor or nurse
Recommendation: Consider what can be done to reduce the time that patients spend in the emergency
department given the Trust's score in this area
Doctors and Nurses
Recommendation: Ensure that patients are given enough time to discuss their condition and that they feel
listened to
Recommendation: Make sure that patients have been spoken to in a way that they are able to understand -
check with them that this is the case and take appropriate action
Recommendation: Some patients say that they did not have trust and confidence in the doctors and nurses
who examined them - look at how this can be addressed as a priority
Recommendation: Some patients reported that doctors and nurses sometimes talked in front of them as if
they weren't there - take action to ensure that this doesn't happen
Recommendation: Ensure that family members or those accompanying the patient are given enough time to
talk to doctors and nurses

Recommendations (continued)
Your Care and Treatment
Recommendation: Some patients say that they are not given enough information about their care and
treatment whilst in the emergency department; look at how this can be improved, considering different
methods of giving information to suit patients' needs
Recommendation: Ensure that patients can get help when needed - tell them how they do this and ensure
that response is forthcoming
Recommendation: Some patients said that one member of staff would say one thing and another
would say something quite different. Discuss with nurses and doctors methods by which reduction
in the amount of conflicting information given to patients can be achieved
Recommendation: Many patients feel that they are not involved in decisions about their care and treatment
as much as they want to be - look at how this can be addressed.
Tests
Recommendation: Ensure that patients are given clear information about why they need the tests that they
need
Recommendation: If patients are not able to receive their test results before leaving the emergency
department, make sure that this is explained to them, and that they are told when they will receive them
Pain
Recommendation: Investigate why many patients say it took over 30 minutes for them to receive their
requested pain relief medication, or said they were not given any, and consider what action can be taken to
reduce this
Recommendation: Look at why some patients say that hospital staff did not do everything they could to help
control their pain and consider what action can be taken, given the low score in this area
Hospital Environment and Facilities
Recommendation: Ensure that adequate provision is made for patients to get something to eat and drink
while they are in the emergency department
Leaving the Emergency Department
Recommendation: Ensure that all patients have the purpose of medications and any possible side effects
explained in a clear way - this is one of the most important issues from a patient's perspective
Recommendation: Make sure patients are given clear information about when they can resume normal
activity after leaving the emergency department
Recommendation: Give patients clear information on danger signs to watch for once leaving the emergency
department and details of who to contact in case they are worried
Recommendation: Ensure that patients are signposted to help them choose the right service in future to
access e.g. GP, NHS111, out of hours service, walk in service, minor injuries or emergency care centres
Page 5 of 60

Recommendations (continued)
Overall
Recommendation: Many patients feel that they are not treated with respect and dignity during all their time
in the emergency department - urgently look at how this can be addressed
Recommendation: Ensure that information about how to comment or complain (such as leaflets and posters)
are visible and available for patients in hospital; staff are up to date on comments and complaints procedures
and able to explain and easily communicate this to patients
Recommendation: Triangulate and link patient experience data, FFT and patient outcomes (PROMS) and use
in the organisation to identify specific actions that can be put in place. Such actions will, on the basis of
analysis of very large NHS datasets, have the potential to improve patients’ perceived outcomes of care

Standardised Data
Raw Data
Compositional Charts - Raw Data
Suppression
Used in the Benchmark Charts and Tables .
This data provides the Trust with an indication of how scores rank when directly compared with the average
scores, whilst supressing any differences that may be present due to local variation in terms of patient
demographic profile. Standardising the data in this way ensures that any comparisons drawn are reliable when
determining variations in scores and top and bottom performers.
The process undertaken to standardise the data is based on national methodology used by the CQC to produce
the national benchmark reports and should be useful to provide an indication of what a Trust’s national results
are likely to be. However, please be advised that there will be minor differences between the numbers in this
report and a Trust’s official national benchmark report as Quality Health only has access to a proportion of the
data whilst the national standardisation process will be based on the full dataset available for all Trusts.
Used in the Compositional Charts .
This data provides the Trust with an unadjusted view of exactly how patients have responded to the survey. This
view of the data is important to ensure the Trust has full visibility of the survey results as a dataset in its own
right. Comparisons with the unadjusted survey averages are also provided for information.
The compositional chart uses data as reported in the Survey Results Manual (frequency tables) at the end of this
report, for each question that is also evaluative / scored. It shows the range of responses to the specified
question for the organisation and for all similar organisations in the Quality Health database (survey average).
The vertical scale is always 0-100%. These charts exclude any non-specific responses such as don’t know / can’t
remember.
All scores within the benchmark charts and tables are supressed if fewer than 30 respondents have answered
an individual question. When scores are supressed, no RAG rating is assigned.

Important Note - Scored Questions
It is not appropriate to score all questions in the questionnaire as not all of the questions assess the trusts in
any way, for example, they may be 'routing questions' designed to filter out respondents to whom following
questions do not apply. An example of a routing question is Q12 (During your stay in hospital, how many wards
did you stay in? ).
For each question in the survey, the individual (standardised) responses are converted into scores
on a scale from 0 to 10. A score of 10 represents the best possible response and a score of zero the
worst. The higher the score for each question, the better the trust is performing.
In the above example all three responses are scored. The first response is treated as being entirely positive, the
second as partially positive and the third as negative. Such scoring should be taken into account when
comparing raw percentages in the compositional charts against overall question level scores.
For full details of the scoring please refer to the Scored Questionnaire which can be accessed here:
http://www.nhssurveys.org/surveys/985
For each scored question in the survey, the individual responses are converted into scores on a scale from 0% to
100%. A score of 100% represents the best possible response and a score of 0% the worst. The higher the score
for each question, the better the trust is performing.

Benchmark Charts and Tables - Scored Questions Only
• A red section: the lowest-scoring 20% of Trusts
• An amber section: the intermediate 60% of Trusts
• A green section: the highest-scoring 20% of Trusts
• The first column shows the question number and question text
• The second column shows the lowest score achieved across all Trusts in the Quality Health database
• The third column shows the highest value in the lowest scoring 20% of Trusts (i.e. the threshold or end of the
red segment of the chart);
• The fourth column shows the lowest value in the highest scoring 20% of Trusts (i.e. the threshold or start of
the green segment on the chart);
• The fifth column displays the highest score achieved across all Trusts in the Quality Health database
• The sixth column shows the base size or number of respondents for the question/Trust
• The seventh column shows the Trust's score for this year (as depicted by the black circle on the chart)
• The final column shows a RAG rating indicator. If a Trust's score falls within the lowest 20% of scores for that
question, a red dot will be displayed. If a Trust's score falls within the intermediate 60% of scores for that
question, an amber dot will be displayed. If a Trust's score falls within the highest 20% of scores for that
question, a green dot will be displayed. If the Trust's score is supressed, no RAG rating is displayed.
The black circle represents the score for the Trust. If the circle is in the green section of the bar, it means that
the Trust is among the top 20% of Trusts surveyed by Quality Health for that question. The line on either side of
the circle shows the 95% confidence interval (the degree of uncertainty surrounding the Trust's score).
Under each benchmark chart is a data table, detailing the following:
<Example data table here>
<Example chart here>
Each scored question has a bar that represents the range of results across all Trusts that took part in the survey
with Quality Health.
The bar is divided into three segments as follows:
The benchmark charts (example below) show results for scored questions only.

Survey results
This report sets out the results from the 2016 Emergency Department Survey for University Hospitals Coventry and Warwickshire NHS Trust , ordered in exactly the
same way as the survey questionnaire sent to service users. All the figures shown are derived from the raw, unstandardised, data.
Reading the columns of figures
The results are shown firstly in absolute numbers, then as percentage responses. The first two columns show the organisation's results from the current survey and the
final two columns show the data for the other Quality Health client organisations participating in the survey.
The purpose of presenting the figures in this way is to give a direct, at-a-glance, comparison between the organisation's 2016 results and the overall results from the
Quality Health client database for this survey.
Conventions
The percentages are calculated after excluding those respondents that did not answer that particular question. All percentages are rounded to the nearest whole
number. When added together, the percentages for all answers to a particular question may not total 100% because of this rounding.
The ‘Missing’ figures show the number of respondents who did not reply to that particular question. In some cases, the ‘Missing’ figure is quite high because it includes
respondents who did not answer that question or group of questions because it was not applicable to their circumstances (e.g. Q2).
On some questions there are also some figures/responses which are italicised and marked with an asterisk. These figures have been recalculated to exclude responses
where the question was not applicable to the respondent's circumstances, or they felt unable to give a definite answer. For example, questions such as Q5 about how
long a patient waited with the ambulance crew whilst waiting to be handed over to emergency department staff, where those not answering (Missing), those not
answering (Missing) and those saying "Don't know / can't remember" are excluded from the percentage calculated for the asterisked responses.
Changes made to the data
There are a number of questions which are ‘routed’ (i.e. where respondents are directed to a subsequent question depending on their answer to the lead question).
Sometimes there are conflicts in the answers that respondents give to these questions and the data is corrected to account for this. For example, if response 1 in
question 1 is ticked and the respondent goes on to answer questions 2-3, then any data entered for these questions will be deleted as the respondent should not have
answered them.
