CONTINUING PHARMACYEDUCATION CREDIT Education sessions will be offered for CPE credit and registrants may earn up to 10.0 contact hours (1.000 CEUs) at this meeting (ICHP determines the number of contact hours for each session).

Please note: PTCB requires CPhTs to earn only pharmacy technician-specific (‘T-specific’) credit to qualify for recertification. In addition, PTCB allows 5 hours of in-service training, however, no inservice training will qualify for CE starting in 2018. Please advise all pharmacy technicians at your site of these requirements.

ACCREDITATION The Illinois Council of Health-System Pharmacists is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. Registrants at the 2018 ICHP Spring Meeting may earn a

maximum of 10.0 contact hours of continuing pharmacy education in states that recognize ACPE providers. Please refer to the full meeting brochure (updated regularly) for complete accreditation information.

Dr. Aaron Buchko is Professor of Management in the Foster College of Business, Bradley University, teaching graduate and undergraduate courses in Planning and Strategy and graduate courses in Executive Development and Ethics. He earned his Ph.D. in management from Michigan State University. Prior to entering the academic field, Dr. Buchko had a successful career for several years in production, sales, and marketing, and was the chief marketing officer for a publishing company. His research and writing focuses on business planning, competitive strategy, and executive development, and he has pub-lished a textbook, research monograph, and over twenty-five articles in various academic journals and professional publications, including the Academy of Management Journal, Advances in Strategic Man-agement, The Journal of Business Research, Human Relations, and The Journal of Management Studies. Dr. Buchko is a Past President of the Midwest Academy of Management, and has received Bradley University’s Mergen Award for Public Service, New Faculty Achieve-ment Award for Teaching Excellence, and the MGP Products Award for Outstanding Teaching in the Foster College of Business. He is a con-fidential advisor and consultant to executives in many organizations, conducts numerous professional seminars, and is a frequent speaker to corporate, trade, and civic associations. Dr. Buchko serves on the Boards of several organizations.

1. There have been a couple of changes last year and several proposed changes this year to the Controlled Substance Act to address the opioid crisis. What can ICHP do to combat this crisis? Would educational programming be of value or are there other tools or services ICHP could develop alone or in collaboration with Illinois Colleges of Pharmacy that would help frontline practitioners, first responders and community leaders?

2. Having the Residency Program Design and Conduct (RPDC) here at last year’s annual meeting was very well received! What other activities or programs could ICHP create or bring to Illinois to support preceptor development and education for both residents and students? What other activities could ICHP undertake to support establishing new residency programs around Illinois?

3. ICHP is represented on the Collaborative Pharmaceutical Task Force by Scott Meyers. The Task Force is reviewing 16 specific items mostly concerning workload issues and e-prescribing, but it will also review in general, the Pharmacy Practice Act and propose revisions to the General Assembly in 2019. Are there any specific changes you would like to see made to the Practice Act regarding workload issues, e-prescribing or any other section of the Practice Act?

4. What would you like to see ICHP do to promote provider status?5. In working for “Provider Status” in Illinois, should ICHP push for a new level

of pharmacist, such as an Advanced Practice Pharmacist or Pharmacist Clinician or should every pharmacist who passes the NAPLEX and MPJE be allowed to be a Provider?

6. The ICHP Champions Program: 1) How many of you know what it is? 2) How many have a Champion at your institution? 3) How many of you are your institution’s Champion? 4) If you don’t participate is it because it’s not worth the effort or 5) If you do participate what challenges/obstacles are there at your workplace?

7. What steps can we take to encourage members to renew and non-members to join?

8. Do you (ICHP members) use the Toolkits available on the ICHP Website www.ichpnet.org and if so, for what situations?

9. Does ICHP need to expand its disaster planning for hospital pharmacies or provide any kind of toolkit to address natural or man-made disasters?

10. In a perfect world where ICHP had ample resources, what would you like to see it provide? Besides lower or no dues!

To take advantage of important research, ICHP is excited to provide the opportunity for continuing pharmacy education credit during the poster presentation sessions at the ICHP 2018 Spring Meeting. Each session is worth 1 credit hour. You must review and discuss 4 posters in a single session to receive one (1) credit for that session.

Please NOTE: 4 posters in Session I = 1.0 Contact hour / 0.1 CEU, no partial credit is available. 4 posters in Session II = 1.0 Contact hour / 0.1 CEU, no partial credit is available.

Poster titles, presenter(s) and date of presentation are listed on pages 19 and 20.

Instructions on how to obtain CPE credit for the Poster Sessions:

1. Identify four (4) posters in a single session that interest you and that will expand your education, knowledge and continuing professional development (CPD) at the ICHP Spring Meeting.

2. Add the poster ID # and title to your Poster Session CPE Verification Forms on the gold paper provided in your meeting folder (one form for each day).

3. Visit the selected posters and discuss them with the authors/presenters. When reviewing a poster, be sure to evaluate the model used for the initiative, how the data collected was assessed and what outcomes were observed. Learners are encouraged to ask questions of the poster presenters.

4. Please request any financial disclosures from the poster presenters and indicate the response in the Financial Disclosure box on the Poster Session CPE Verification Form.

5. Ask the presenter (or co-presenter) to sign your form, once you have reviewed your selected poster. When you have all four signatures for the Session, please bring the form to the Validation Station at the Meeting Registration desk.

6. Meeting staff will review and keep your documentation for ACPE poster attendance records.

7. Meeting staff will provide the Access Code for the Session(s) you attended. Write the code(s) on your CPE Attendance Record Sheet in the appropriate place.

8. You will then complete your CPE process by turning in a copy of your CPE Attendance Record sheet at the end of your time at the meeting, going to CESally.com and evaluating all the CPE program(s), including the Poster Sessions. 2. If you attended a Poster Session, you will need to turn in your signed poster session form to re-ceive the access code for your CPE Attendance Record.

3. Turn in BOTH the White and Yellow copies at Registration check out for signature.

4. Registration attendant signs BOTH copies and keeps the White top copy. Please SAVE your yellow copy!

5. Go to CESally.com and Login if you have an account; or “Sign Up!” to create an account if new to the site. Important! You must enter your NABP eProfile ID & birthdate as MMDD to set up your account and get credit.

6. Use the Search function to find all the Program Titles you attended and Add to your To-do List.

7. Go to your To-do List to complete program evaluations and submit. Follow the directions.

8. Once completed, you will see a submission confirmation message.

9. Go to your CPE Monitor account at NABP.net to print off official transcripts or statements of credit.

DETAILED STEPS ON HOW TO COMPLETE THE EVALUATION PROCESS –Session Access Codes will be announced by the moderators during each session – write the codes on your “CPE Attendance Record” sheet (2-part white and yellow form). Please fill in your “Name” informa¬tion on your CPE Attendance Record. At the end of your participation at the conference, please turn in BOTH the white and yellow copies of your Attendance Record at the Registration desk. Registration attendants sign both copies and keep the white copy. You will get the yellow copy back. Keep the yellow copy of the CPE Attendance Record for your file as you will need the Session Codes which you wrote down to complete the process of receiving your credit.

3. Enter your NABP eProfile ID and birthdate as MMDD when prompted. CESally.com will confirm in real time your eProfile information with CPE Monitor and let you know if there is an error, which you may then correct.

4. Once you have an account, you may use the Search Box in the upper right corner to find your activity by typing in the title, or type in 2018 ICHP Spring Meeting to see all the sessions. You have several options for completing or sav-ing for later.

Please pay CLOSE attention to the Title and the Pharmacist or Technician label at the end of each title. PHARMACISTS: You may NOT receive credit for Technician programs. TECHNICIANS: As of January 1, 2015, PTCB requires Technicians use only Technician-specific CPE credit to recertify.

5. Identify the program(s) attended and choose either to: a) Click on that Activity title to open the information page, and you will see your options in the right hand column

on the information page.

b) OR Click on the checkbox inside the small information box, then go to the bottom of the page and see your options there.

6. Choose from either a) or b) above, to Complete Now, Save for Later, OR ADD to To-do List.

7. When you are ready to complete your CPE credit process, follow directions to EVALUATE the programs. • You will verify your attendance, provide the session ACCESS code that was given to you during the program, and

complete an evaluation of the activity and the speaker(s).• POSTER SESSION Note: If you have comments for a particular poster presenter, please note the name and make

your comments in the Feedback section of the Evaluation form. We will forward your feedback to the presenters and speakers.

8. Click on “Report CE”. Your CPE credit will be uploaded to CPE Monitor automatically. You will see a submission confir-mation message when you have successfully completed the process.

10. Go to www.NABP.net to download an official copy of your CPE transcripts. Click on the CPE Monitor link and log in to access your CPE Monitor records.

Important: Per ACPE requirements, official transcripts and statements of credit may now be accessed ONLY through CPE Monitor. PLEASE Note: You will have until the end of day Monday, May 28, 2018 to complete your online evaluation and sub-mit your CPE credit. ICHP policy requires that CPE evaluations be completed within a specific deadline to ensure timely submission of your CPE credit to your CPE Monitor account, and to meet the ACPE deadlines. Anything submitted after the ACPE deadlines will be rejected by CPE Monitor. If you miss the deadline you will lose your credit.

Remember – if you experience any problems, we will need time to assist you. Please do not delay!

Important Notice: Only registered attendees to the CPE presentations are eligible to receive credit. Meeting reg-istration lists and CPE attendance records are used to verify that those who process credit on CESally.com at-tended the live programming.
