6 laser worker eye injuries since 2001 at DOE laboratories• No eyewear during any of these injuries• 4 of the 6 injured were STUDENTS• 5 involved use of Class 4 laser (like our Ti:Sap)• 1 involved use of Class 3b laser (like our diode)• Injuries result from poor laser training and hazard controls

Integrated Safety Management => Event Root Causes• Training -> weakness in training, lack of compliance with codes• LSO/LSS ->authority and role model for laser safety• Internal Oversight -> inadequate line management• Personal Protective Equipment (PPD) -> absolute enforcement

Audit Finding: Leading indicator of good laser safety is good “Housekeeping”.

Jlab In-House Safety Assessment, May 2005• Agenda set by DOE and ANSI code standards• Findings

1. LSO approves wording on laser signage2. Formally designate the deputy LSO: John Kelly3. Formal designation and documentation of LSS quals4. Formalize procurement process for purchase of lasers 5. Review DOE eye injuries and root causes with Laser Workers6. Evaluate and implement refresher training where necessary (LSO)7. Formalize laser specific training, must indicate depth and scope of on the job training

• Review DOE lessons learned• Demonstrate that safe work is happening:

– Know procedures– CLEAN UP!!!!

• Establish clear roles, responsibility, and authority for laser operations• Demonstrate active oversight of all aspects of laser Operations• Monitoring and evaluating mentors and LSSs that oversee students• Establishing and clearly explaining personal accountability, including

reprimand policy for employees that do not meet safety performance specifications
