    DEFINITION OF TERMS

    ACCESSORY:A device which is mechanically attached to, or used with, the facilities furnished by theCompany and which is independent of, and not electrically, acoustically or inductivelyconnected to, the conductors in the communications path of the telecommunications system.

    ALTERNATING CURRENT SUPPLY:Electrical energy which is used for the operation of bells and signal devices.

    ATTENDANT POSITION:An attendant switchboard or console which provides an answering point for customer-providedcommunications systems. It is used for answering and distributing the incoming calls and forassistingcertain users.

    AUTHORIZED PROTECTIVE CONNECTING MODULE:A protective unit designed by the Company and manufactured under the control of Companyquality assurance procedures; such unit is to be incorporated in a conforming answeringdevice.

    BASE RATE:A schedule rate for any class-of-exchange service available within the base rate area.

    BASE RATE AREA:That portion of the exchange area in which Exchange Service is furnished at schedule ratesfor each class of service without mileage or construction charges.

    BASIC SERVICE AREA:An area in which designated Local Exchange Services are provided at rates shown in Part 4 ofthe Guidebook. When Local Exchange Services are furnished outside their designated basicservice area, mileage charges are applicable. Urban basic service area is the base rate area.

    BUILDING:The term "same building" is to be interpreted to mean a structure under one roof or two ormore structures on one premises which are connected by an enclosed or coveredpassageway.     CENTREX:Centrex Service provided by switching equipment which is located on the Company's premises.

    CENTREX CONTROL SWITCHING EQUIPMENT:The switching equipment, located on the Company's premises, used to provide Centrex Servicefurnished in accordance with Centrex Service provisions of the Part 6 of the Guideook.

    CENTRAL OFFICE CONNECTING FACILITY:A facility furnished to an Other Common Carrier by the Company (in accordance with theCompany's Facilities for Other Common Carrier's Interstate Guidebooks andTariffs) between theterminal location of the Other Common Carrier and a point of connection on the Companypremises.

    CLASS OF SERVICE:A description of customer service which denotes the nature of its use, i.e., business or residenceservice.

    COMMON CONTROL SWITCHING ARRANGEMENT (CCSA):A service arrangement at certain Company premises to interconnect private line telephoneservice channels of a switched service network provided to a customer. Use of the commoncontrol switching equipment on the Company premises is shared with other service offerings orcustomers.

    COMMUNICATIONS SYSTEM:Channels and other facilities which are capable, when not connected to the telecommunicationsnetwork, of two-way communications between customer-provided terminal equipment and/orCompany stations.

    COMPANY:Southwestern Bell Telephone L.P., d/b/a AT&T Missouri.

    COMPOSITE DATA SERVICE:The combined use of terminal and data switching equipment provided by a Composite DataService Vendor with the use of telecommunications services of the Company to perform dataswitching for others.

    COMPOSITE DATA SERVICE VENDOR:A customer that has been certificated by the proper state regulatory body and/or the FederalCommunications Commission pursuant to Section 214 of the Communications Act of l934, asamended, to acquire and operate facilities to perform data switching for others. A customershall be classified as a Composite Data Service Vendor only with respect to use of thoseexchange services which are utilized for the provision of Composite Data Service.

    CONFORMANCE NUMBER:An identifying number assigned by the Company to a particular model of conforminganswering device incorporating an authorized protective connecting module when that modelor device is in conformance with the provisions set forth by the Company in its technicalreference for conforming answering devices.

    CONFORMING ANSWERING DEVICE:A customer-provided device which automatically answers incoming calls, transmits aprerecorded voice message or appropriate audible signal to the calling party, records a voicemessage from the calling party if so designed and arranged and automatically disconnectsfrom the line in a prearranged manner on completion of the last of the functions for which itwas designed and arranged as described in this paragraph. The conforming answering devicemay include remote interrogation and/or device function control. A conforming answeringdevice must incorporate an authorized protective connecting module and must bear a validconformance number.

    CONNECTING ARRANGEMENT:The equipment provided by the Company to accomplish the direct electrical connection ofcustomer-provided facilities with the facilities of the Company.

    CONNECTING COMPANY:A corporation, association, firm or individual owning and operating one or more central officesand with whom traffic is interchanged.

    CONNECTING STATION:An exchange station of a connecting company.

    CONSTRUCTION CHARGE:A separate initial charge made for construction of pole lines, circuits, facilities, etc., in excessof that contemplated under the rates quoted in Part 2, Section 5.

    COVERED PUBLIC MALL:A covered public area, originally intended for, or formerly used as, a public street or alleyrunning between two or more buildings.

    CUSTOMER:The person, firm or corporation which orders service and is responsible for the payment ofcharges and compliance with the regulations of the Company.

    CUSTOMER PREMISES INSIDE WIRE:That wire including connectors, blocks and jacks, within a customer's premises that extendsbetween the demarcation point to those locations within the customer's premises to whichterminal equipment can be connected for access to the Company Network.

    CUSTOMER-PROVIDED TERMINAL EQUIPMENT:Devices, apparatus and their associated wiring provided by a customer which are used withthe network control signaling unit, DATAPHONE Data Set or other equipment furnished by theCompany and does not include customer-provided communications systems.

    CUSTOMER-PROVIDED TEST EQUIPMENT:Denotes test equipment located at the premises of the customer that is used by the customerfor the detection and/or isolation of a communications service fault.

    DATA ACCESS ARRANGEMENT:A protective connecting arrangement for use with the network control signaling unit, or in lieuof the connecting arrangement, an arrangement to identify a central office line and protectivefacilities and procedures to determine compliance with criteria set forth in Part 2, Section 9and Part 20, Section 2 of the Guidebook.

    DATA SWITCHING:When used in connection with Composite Data Service, denotes the switching of data (non-voice) messages by the interchange, controlling and routing of data messages viacommunications facilities, wherein the information content of the message remains unaltered.

    DEMARCATION POINT:That point (referred to as Demarc Point or Network Interface) of interconnection between theCompany’s facilities and the wiring at the subscriber’s premises. The Demarc Point shall consistof wire or a jack conforming to Subpart F of Part 68 of the Federal CommunicationsCommission’s Rules and Regulations. The Demarc Point will generally be within twelve inchesof the protector or, absent a protector, within twelve inches of the entry point to the customer’spremises. If conforming to the twelve inches is unrealistic or technically impossible, the DemarcPoint will be the most practicable minimum point of entry to the customer’s premises.

    The network interface may be located at a point other than the normal demarcation point wherethe network interface is already established by the presence of network equipment. For multiunit(e.g., apartments, college campuses, shopping centers) the structure owner shall make the finaldecision on whether the structure shall be treated as a multipremises structure with onedemarcation point per premise or, as a single premise with one demarcation point for the entirestructure. The structure owner shall have the option of having the demarcation point placed at alocation other than that determine by the Company provided the structure owner pays anyadditional construction costs and such location is consistent with the minimum point of entrystandard.

    Upon request of the subscriber or their agent, the Company shall provide additional regulatednetwork entrance facilities and/or demarcation arrangements in single tenant multiunit ormultibuilding situations in accordance with Part 2, Section 5 of the Guidebook. Each additionalregulated network entrance facility will terminate in a demarcation arrangement located at aminimum point of entry within a specified designatedtelecommunications equipment space.

    With regard to premises for any structure that is built to be mobile (e.g., mobile homes,recreational vehicles), the Company may place the Demarc Point on a post or pole at or near thepad where such structure is intended to rest. Boat docks and similar premises may be treatedby the Company as a single unit premises, with the Demarc Point being placed on the shore.

    DIRECT CURRENT SUPPLY:Electrical energy for talking and signaling purposes, other than ringing, except in the case ofintercommunicating systems when direct current may be used for ringing the telephone bells.

    DIRECT ELECTRICAL CONNECTION:A physical connection of the electrical conductors in the communications path.

    DIRECTORY LISTING:The publication in the Company's directory of information relative to the customer's telephonenumbers, by which telephone users are enabled to ascertain the telephone number of a desiredtelephone.

    EQUIPMENT SPACE:An area or areas, agreed upon by the subscriber and the Company, located on or within astructure that is specifically designated for the purpose of terminating regulated Companyprovided services and facilities. The necessary security, lighting, commercial power andenvironmental controls are provided within this area.

    EXCESS MILEAGE:(See Urban Mileage)

    EXCHANGE:A telephone system which provides for service within a specified area known as the "ExchangeArea." An exchange is termed a "Potential Exchange" when the customers are served (MT) by acentral office located in another exchange area. (See Serving Office District)

    EXCHANGE ACCESS ARRANGEMENT:An Exchange Access Arrangement (EAA) is a telephone facility which permits access to and fromboth the customer's premises and the telephone exchange network point. A telephone exchangenetwork point is also known as a serving central office.

    EXCHANGE ACCESS LINE:Company-provided and maintained central office equipment and Company facilities, including theNetwork Interface which provide access to and from the telecommunications network formessage long distance and local calling.

    EXCHANGE AREA:The area within which the Company will furnish complete telephone service at the exchange ratesapplicable within that area.

    EXCHANGE SWITCHING SERVICE:The classification of Exchange Service furnished to a service telephone.

    EXCHANGE TELEPHONE:A telephone receiving Exchange Service.

    EXCHANGE TELEPHONE SERVICE:That service which entitles the customer to have his telephone connected with any otherexchange telephone within the exchange area or with any other telephone to which suchconnection is provided at charges specified in the Part 3 and Part 4 of the Guidebook.

    EXTENDED AREA SERVICE:A Local Exchange Service that is provided between two or more contiguous exchange areas.

    FLAT RATE SERVICE:Exchange Service furnished for a specified sum without regard to the amount of use.

    FOREIGN SERVING OFFICE MILEAGE: (See Part 15, Section 1)

    FOREIGN SERVING OFFICE SERVICE:Exchange Service by means of a circuit connecting a customer's station with a serving office ofthe same exchange but outside of the serving office district in which the telephone is located.

    FOREIGN EXCHANGE MILEAGE: (See Part 15, Section 1)

    GRADE OF SERVICE:A description of residence or business service which denotes the number of parties (accesslines) that a telephone line is designed to serve, i.e., one-party (individual), two-party or four-party service.

    GRANDFATHERED COMMUNICATIONS SYSTEMS:The term "Grandfathered Communications Systems" denotes communications systems(including their equipment, premises wiring and protective circuitry, if any) connected at thecustomer's premises, in accordance with any Company's Guidebook, and that are consideredto be grandfathered under the Federal Communications Commission's Rules and Regulations,because such systems are connected to the telecommunications network prior to January 1,1980, and are of a type of system which was directly connected (i.e., without Company-provided connecting arrangements) to the telecommunications network as of June 1, 1978.

    GRANDFATHERED CONNECTIONS OF COMMUNICATIONS SYSTEMS:The term "Grandfathered Connections of Communications Systems" denotes connections viaCompany-provided connecting arrangements of customer communications systems (includingtheir equipment and premises wiring) at the customer's premises, in accordance with anyCompany's Guidebook, and that are considered to be grandfathered under the FederalCommunications Commission's Rules and Regulations, because such connections to thetelecommunications network are made via Company-provided connecting arrangements priorto January 1, 1980, and such connecting arrangements are of a type of connectingarrangement connected to the telecommunications network as of June 1, 1978.

    GRANDFATHERED TERMINAL EQUIPMENT:The term "Grandfathered Terminal Equipment" denotes terminal equipment (includingprotective circuitry, if any) connected at the customer's premises, in accordance with anyCompany's Guidebook, and that is considered to be grandfathered under the FederalCommunications Commission's Rules and Regulations, because such terminal equipment wasconnected to the telecommunications network prior to July 1, 1979, and is of a type of terminalequipment which was directly connected (i.e., without Company-provided connectingarrangements) to the telecommunications network as ofOctober 17, 1977.

    GRANDFATHERED CONNECTIONS OF TERMINAL EQUIPMENT:The term "Grandfathered Connections of Terminal Equipment" denotes connections viaCompany-provided connecting arrangements of customer-provided terminal equipmentconnected at the customer's premises, in accordance with any Company's Guidebook, andthat are considered to be grandfathered under the Federal Communications Commission'sRules and Regulations, because such connections to the telecommunications network weremade via Company-provided connecting arrangements prior to July 1, 1979, and suchconnecting arrangements are the same type of connecting arrangement connected to thetelecommunications network as of October 17, 1977.

    INDIVIDUAL (ONE-PARTY) LINE SERVICE:A grade of Exchange Service which provides for a maximum of one main station on a line.

    INSTALLATION/MOVE CHARGE:A nonrecurring charge made under certain conditions covering the cost or portion of the costof the work connecting, furnishing or moving telephone service. (See Construction Chargeand Service Connection Charges.)

    INTERCOMMUNICATION SYSTEM: (See Private Branch Exchange)

    INTEREXCHANGE CHANNEL:That portion of a channel which interconnects exchanges in which the stations are located.

    INTERFACE:That point on the premises of the customer at which provision is made for connection ofother than Company-provided facilities to facilities provided by the Company. (See NetworkInterface)

    INTERNATIONAL RECORD CARRIER:A carrier, certified by the Federal Communications Commission, to provideoverseas/international telecommunications service other than voice communication (e.g.,teletypewriter, facsimile, data).

    JACK: A fixed socket provided by the Company to accomplish the direct electrical connectionbetween telephone facilities by means of cords ending in plugs.

    JURISDICTION: A geographic area meeting each of the following conditions:

    A. Presided over by the same regulatory body;

    B. Within the boundary of a single state;

    C. An area in which Southwestern Bell d/b/a AT&T Missouri is authorized to provide service.

    LOCALITY RATE:A rate quoted as a single item but made up of base rate, plus an average or commutedmileage or construction charge, applied uniformly to all customers to the same class of servicewithin a defined part of the schedule area, not a part of the base rate area of the scheduleunder which service is rendered.

    MEASURED SERVICE:A class of service for which a charge is made in part on the basis of network access and, inpart, on the basis of four local usage pricing elements, number of local messages, the durationof the message, the distance of the message within the local area and the time of day themessage was placed.

    MESSAGE:A telephone conversation of any prescribed length between two telephone stations.Messages may be classified as follows:

    Local Message: A communication between telephone stations located within the areadefined as the local service area of the station from which the message originates.

    Toll Message: A communication between two telephone stations, the called stationbeing outside of the local service area of the station from which the message originates.

    MESSAGE RATE SERVICE:A class of Exchange Service, not including coin box service, for which charge is made whollyor in part on the basis of amount of use.

    MOVE:Inside Move: A relocation of telephone service from one location to another location on thesame premises.

    Outside Move: A discontinuance of telephone service at one premises and its installation atanother premises, all within the same jurisdiction, between rate jurisdictions of a BellOperating Company or between two jurisdictions in separate Bell Operating Companies, ifappropriate.

    MULTILINE:An individual flat rate business Exchange Service furnished by means of two or moreexchange lines terminating in apparatus which facilitates pick-up by associated stations.

    Multiline rate exceptions are as follows:

    A. Multiline rates do not apply where the PBX trunk rate applies or on Package PBX orCentrex Service.

    B. Multiline rates do not apply to announcement lines.

    C. Multiline rates do not apply to Secretarial Answering Lines terminated at a TelephoneAnswering Bureau. Extensions terminated at a Telephone Answering Bureau are notconsidered when determining whether multiline rates apply.

    D. Functions performed by data sets are not considered when determining whether multilinerates apply.

    MULTILINE HUNT GROUP:Rotary (hunting) service provided in Electronic Switching System (ESS) offices where astandard seven-digit number is assigned only to the first line in a customer's number group.Subsequent lines are designated by using two-, three- or four-digit numbers dependent on thetype of ESS office. The Company reserves the right to determine when Multiline Hunt Groupnumbers are assigned.

    NETWORK ACCESS LINE:The exchange line from the serving office terminating directly at the customer-providedcommunications system.

    NETWORK ACCESS TERMINAL:A special arrangement at a PBX, Centrex or an instrument on the customer's premises toprovide access to a private line switched service network for incoming and/or outgoing calls.

    NETWORK CONTROL SIGNALING:The transmission of signals used in the telecommunications system which perform functionssuch as supervision (control, status and charging signals), address signaling (e.g., dialing),calling and called number identification, audible tone signals (call progress signals indicatingre-order or busy conditions, alerting, coin denominations, coin collect and coin return tones) tocontrol the operation of switching machines in the telecommunications system.

    NETWORK CONTROL SIGNALING UNIT:The terminal equipment furnished for the provision of network control signaling.

    NETWORK INTERFACE:The point where the Company's installation and maintenance responsibility, on a tariffed andguidebook basis, ends and the customer's installation and maintenance responsibility begins.It is a Company-provided jack or its equivalent. (See Demarcation Point)

    NONLEAD NUMBERS:Any number in the customer's number group that is not the first number.

    NONPUBLISHED EXCHANGE SERVICE:An Exchange Service, the listing of which, at the customer's request, is omitted from thealphabetical list of names of customers. Nonpublished telephone numbers are not divulgedexcept in situations where the Company is served with valid legal process or is in receipt oflawful demand of a law enforcement agency.

    PRIVATE BRANCH EXCHANGE:

    A. Dial Systems

    1. Switchboard Systems: A dial switchboard system provides automatic switchingequipment to interconnect lines terminating in the switching equipment or to connectthese lines to central office trunks. Incoming calls are manually switched toterminating lines by keys or cords.

    2. Key Station Systems: A key station system provides automatic switching equipmentto interconnect lines terminating in the switching equipment or to connect these linesto central office trunks with supervision of incoming calls at one or more masterstations.

    B. Manual Systems

    Switchboard System: A manual switchboard system provides a manually operatedswitchboard operated by keys or cords to interconnect the lines terminating in theswitchboard or to connect these lines to central office trunks for incoming or outgoingcalls.

    C. Intercommunicating Systems

    No. 1 and No. 2 Systems: A system in which each station is equipped with a set ofpushbuttons that can be used to signal or connect that station to other stations connectedto the system or to central office trunks.

    D. Circuits and Equipment Ordinarily Furnished Include the Following:

    PRIVATE BRANCH EXCHANGE TRUNK LINE: A circuit which connects a PrivateBranch Exchange with the central office for the purpose of telephone communication.

    ROTARY DIAL TELEPHONE SERVICE:That service which utilizes rotary dial pulse equipment (as opposed to Touch-ToneEquipment) to originate calls.

    RURAL BASIC SERVICE AREA: (See Basic Service Area)

    SCHEDULE AREA:An area within which a given exchange rate schedule is available, with mileage charges whenapplicable.

    SEASON RATE:A rate for Exchange Service during all or any portion of a specified season period or during adefinite part of the calendar year. (See Short Term Rate)

    SERIES COMPLETION:Telephone numbers assigned in a rotary (hunting) number group where each numberassigned is a standard seven-digit number. The Company reserves the right to determinewhen series completion numbers are assigned.

    SERVICE CONNECTION CHARGES: Refers to nonrecurring charges made to customers for thepurpose of reimbursing the Company for the cost involved in connecting, moving, changingand disconnecting telephone service.

    A. STANDARD RESIDENCE AND BUSINESS SERVICE - Residence or business servicefurnished by means of line(s) terminating only in non-button or one-button (non-key)telephone instruments.

    B. COMPLEX RESIDENCE AND BUSINESS SERVICE - Service provided as a part of, or inconnection with, a system in which two or more exchange access lines terminate in anapparatus which facilitates pick-up or shared access to exchange access lines.

    SERVICE POINT:When used in connection with customer-provided communications channels, denotes thepoint on the customer's premises where channels provided by or furnished to the customer areterminated in switching equipment used, at least in part, for communications with stations orcustomer-provided terminal equipment.

    SERVICE STATION LINE:Telephone facilities provided by the customer in isolated and sparsely developed areas whereit is not practicable for the Company to make such facilities available.

    SERVICE TERMINATING ARRANGEMENT:Company-provided equipment which terminates WATS or Exchange Telephone Service at acustomer's premises. The Service Terminating Arrangement provides a clearly delineatedinterface which facilitates the design, isolation and testing of WATS or Exchange TelephoneService. Where a protective connecting arrangement is required, the Service TerminatingArrangement is provided as a part of the protective connecting arrangement.

    SERVING OFFICE:The office in the service area from which a customer would normally be served for LocalExchange Telephone Service. Where the serving office is not located in the service area, atheoretical location has been established by the Company for billing purposes.

    SERVING OFFICE AREA:The specific area normally served by a serving office. A serving office area may be served inseveral ways, namely:

    A. By a single-unit serving office with one telephone number prefix.

    B. By a multi-unit serving office with multi-telephone number prefixes.

    C. By a theoretical serving office with switching equipment located in another exchange orserving office area.

    SHORT-TERM RATE:A rate quoted for service for a period less than the regular service term and quoted specificallyfor such period of service.

    SIMPLE INSIDE WIRE:Any inside wiring other than complex wire, e.g., wire on the customer's side of theDemarcation Point that terminates in telephone apparatus that does not facilitate pick-up byassociated stations.

    STATION:The network control signaling unit, data set or other equipment at the customer's premiseswhich enables the customer to establish the communications connection to effectcommunications through such connections; also, denotes a termination of an individualexchange line or PBX trunk provided in accordance with the provisions of this Guidebook inswitching equipment located in an ex-change foreign to the exchange in which the customer islocated.

    SUBSCRIBER'S COIN BOX SERVICE:A classification of subscriber's service furnished to stations equipped with coin-collectingdevices as part of the Company's service at rates which are dependent, wholly or in part, uponthe number of local messages within a given period.

    SUBSCRIBER:A person who has contracted for and is receiving telephone service. The contract may bewritten, verbal or merely an understanding.

    SUBURBAN BASIC SERVICE AREA: (See Basic Service Area)

    SUPPLEMENTAL CONTRACT:A contract for service, equipment or facilities in addition to that provided for under the originalcontract.

    SUSPENSION OF SERVICE:An arrangement made at the request of the customer or at the initiative of the Company fortemporarily discontinuing service without terminating the contract or removing the telephoneequipment from the customer's premises.

    SWITCHED CIRCUIT AUTOMATIC NETWORK (SCAN):A service arrangement at certain Company premises to interconnect Private Line TelephoneService channels of a switched service network provided to certain agencies of the FederalGovernment. The common control switching on the Company premises is provided on a four-wire basis. Such service may be arranged for cryptographic transmission.

    SWITCHED SERVICE NETWORK (SSN):A private line network that uses SCAN and/or CCSA-type common control switching.

    SWITCHING EQUIPMENT:Equipment which performs the function of establishing and releasing connections between:

    A. Two or more Company-provided services, or

    B. Company-provided service or services and a communications system or systems providedby the customer or Other Common Carrier.

    Such equipment shall operate to establish each connection for the purpose of the transmissionof communications and shall operate to release the connection or generate a supervisorysignal for the manual release of the connection by an attendant immediately following theconclusion of each call.

    TELECOMMUNICATIONS NETWORK:All facilities of the Company that are used to provide its services.

    TELEPHONE COMPANY:“Company”, Southwestern Bell Telephone L.P., d/b/a AT&T Missouri.

    TELEPHONE NUMBER:A designation assigned to a subscriber's station for convenience in operating. Telephonenumbers may include the name of a central office which is termed the "Central OfficeDesignation."

    Message Toll Telephone Service is that of furnishing facilities for telephone communicationbetween telephones in different local service areas in accordance with the regulations andsystem of charges specified in the Long Distance Message Telecommunications Service, Part9 of the guidebook. The toll service charges specified in the Guidebook are in payment for allservice furnished between the calling and called telephones.

    UNLIMITED SERVICE: (See Flat Rate Service)

    URBAN BASIC SERVICE AREA: (See Basic Service Area)

    URBAN MILEAGE:The mileage applied in establishing the rate for service to a customer outside the base ratearea and which represents the distance from the limits of the base rate area to the customer'sstation.

    ZONE: That portion of a Metropolitan Exchange which has a separate rate center.

    RULES AND REGULATIONS APPLYING TO ALL CUSTOMERS' CONTRACTS

    A. GENERAL

    The regulations specified herein are in addition to the regulations contained in other sections of thisGuidebook and Tariffs. Failure on the part of customers to observe these rules and regulations ofthe Company automatically gives the Company the right to cancel the contract and discontinue thefurnishing of service.

    The Company's obligation to furnish service or to continue to furnish service is dependent on itsability to obtain, retain and maintain suitable rights and facilities, and to provide for the installation ofthose facilities required incident to the furnishing and maintenance of that service.

    The regulations covering the connection of equipment, accessories or facilities provided andmaintained by the customer are contained in other sections of this Guidebook.

    B. APPLICATION FOR SERVICE

    Applications for service, or requests from customers for additional service, and changes in thegrade or class of service become contracts when received by the Company and are subject to theminimum contract term. The Company reserves the right to require application for service to bemade in writing on forms supplied by it.

    The terms and conditions of all contracts are subject to the rules and regulations in this Guidebookand Tariffs for the particular exchange for which service is to be furnished. Any general change inrates, rules or regulations shall act as a modification of the contract to that extent without furthernotice except that in case rates are increased, the customer may cancel his contract uponreasonable written notice and upon payment for all service, equipment and any contractual liability.

    Unless otherwise specified, the minimum term for which service will be furnished is one month.

    The Company reserves the right to refuse service to any applicant or former customer who is foundto be indebted to the Company for prior service. The Company will not establish any new serviceuntil satisfactory arrangements have been made for the payment of all such indebtedness.

    Pursuant to paragraph 394 of the FCC’s Final Report and Order in Case No. CC 96-45, a qualifyingcustomer’s request for LifeLine Service may not be denied for previous non-payment of tollcharges.

    In offices, stores, factories and all other places of a strictly business nature./2/

    In offices of hotels, halls and offices of apartment buildings, quarters occupied by clubs or lodges,public, private or parochial schools or colleges, hospitals, libraries, churches and other similarinstitutions, except in churches and boarding houses as specified below.

    At residence locations when use of the service either by the customer, members of his household,his guests or parties calling him can be considered as more of a business than of a residencenature, which fact might be indicated by advertising either by business cards, newspapers,handbills, billboards, circulars, motion picture screens or other advertising matter such as onvehicles, etc., or when such business use is not such as commonly arises and passes overresidence telephones during the intervals when, in compliance with the law or established custom,business places are ordinarily closed.

    At residence locations, where the service or an extension line is located in a shop, office orother place of business.

    In college fraternity houses where the members lodge within the house.

    At any location where the listing of service at that location indicates a business, trade or profession,except as specified below.

    /1/ An electronic Bulletin Board System (BBS) is a data calculating and storage device(s) utilized as avehicle to facilitate the exchange of information through the use of AT&T Missouri services.

    BBSs located at a residence location that do not solicit or require any remuneration, directly orindirectly, in exchange for access, and that use four or fewer local exchange access lines shall bepermitted to subscribe to a flat rate residential offering. Residential measured or message rate servicemay not be used as the local exchange access service for the operation of BBSs. BBSs eligible forresidential rates may not use paid-for advertising to promote or advertise their BBS. Those BBSs thatdo not meet these conditions will be considered businesses, and shall subscribe to the appropriatebusiness rate service for all exchange lines used to operate the BBS.

    In the place of residence of a clergyman, physician, dentist, veterinary surgeon, other medicalpractitioner, Christian Science practitioner, nurse or midwife or in their office, provided the office islocated in their residence and is not a part of an office building. In any of such cases, the listingmay indicate the customer's profession, but only in connection with an individual name. If listingsof firms or partnerships, etc., or additional listings of persons not residing in the same householdare desired, business rates apply.

    In a private stable or garage when strictly a part of a domestic establishment.

    In churches where the service is not accessible for public patronage, as in pastors' studies.

    D. ADVANCE PAYMENTS AND DEPOSITS

    The Company, may require a deposit or guarantee as a condition of new service. The depositmay be required prior to or no more than 30 calendar days after the Company actually providesservice. An advance payment may be request as stated below in 1. The Company reserves theright to make the determination as to the type of security requested of the applicant and applied asspecified in this Guidebook.

    1. Advance Payments

    When making application for service, an applicant for service may be required to pay at the timethe application is accepted, service connection charges, if applicable, and the first month'scharges for exchange service, excluding charges for local messages in excess of the monthlyguarantee. In all cases, the regular monthly charges for service are payable as specified in Part 4of this Guidebook. In most cases, the provisions of this paragraph affect the initial payment andnot the subsequent billing and collection practices as elsewhere provided in Part 2 this Guidebook.

    The amount of the advance payment is credited to the customer's account and may be appliedagainst any indebtedness under contract with the Company. In such cases, an additional paymentmay be required to satisfy the security obligation for the new application for service.

    a. Residence Service

    The Company may require an applicant for service to make an advance payment if:

    1. Applicants With Previous Telephone Service of Twelve Months or More

    The applicant is unable to establish that he had a previous account with a telephone utilityfor a period of at least twelve (12) months for which all undisputed charges weresatisfactorily paid.

    2. Applicants With No Previous Telephone Service or With Previous Telephone Service ofLess Than Twelve Months

    The applicant’s credit record will be accessed and evaluated by means of a mechanizedretrieval system between the Company and nationally recognized credit bureaus. Thisprocess is known as Application Scoring and involves the use of empirical data and astatistical credit model in determining the applicant’s credit worthiness. No securitypayment will be required of those applicants who meet the Company’s established creditcriteria, as evaluated by the scoring model. The criteria used in assessing a score is asfollows:

    - Collection judgments- Written-off accounts- Outstanding collection accounts- Various degrees of delinquency history from 30-180 days, and not paid in full or

    current at the time of scoring- Bankruptcies- Liens- Other public records

    D. ADVANCE PAYMENTS AND DEPOSITS

    2. Deposits

    A. Residence Service

    A deposit will be required of an applicant when the applicant has an outstanding prior debtwith the Company./1/ The amount of deposit for a new applicant will be twice the averagemonthly bill for subscribers within a customer class.

    A present customer may be required to post a deposit as a condition of continued service ifundisputed charges in two (2) out of the last twelve (12) billing periods have becomedelinquent or the customer has had service discontinued at any time during the precedingtwelve (12) billing periods.

    The amount of deposit for a present customer will be twice that customer's average monthlybilling for exchange and long distance charges. The average monthly billing will be based onthe actual billing for the immediate months preceding the request for the deposit, not toexceed twelve (12) months.

    Deposits held thirty (30) days or more, shall bear interest at a rate which is equal to onepercent (1%) above the prime lending rate as published in the Wall Street Journal. This rateshall be adjusted annually on December 1 using the prime lending rate, as published in theWall Street Journal on the last business day of September of each year, plus 1%. The interestshall be credited annually to the account of the customer or paid upon the return of thedeposit, which ever occurs first.

    The Company shall permit an applicant for service to pay the deposit in two equal monthlyinstallments.

    An applicant for service or a present customer may satisfy a deposit requirement by providinga Contract of Guaranty in an amount not to exceed the requested deposit from a presentcustomer with the Company with at least two years of established service whose service hasnot been suspended for nonpayment within the last twelve (12) months. The guarantycontract shall be on a form provided by the Company which shall include the Company's rightto transfer charges to the limit of the guaranty, from a defaulted bill of the customer, fromwhom a deposit or a Contract of Guaranty was required, to the guarantor's account oraccounts and the further right to suspend the guarantor's service pursuant to ParagraphD.2.,a. of this Section. A guarantor shall be released upon satisfactory payment by thecustomer of all undisputed charges during the last twelve (12) billing periods.

    /1/ The deposit may be waived for qualifying Lifeline customers voluntarily electing to receive tollblocking at no charge, pursuant to paragraph 398 of the FCC’s Final Report and Order in CaseNo. CC 96-45.

    The fact that a deposit has been made, or a guaranty provided, shall in no way relieve thecustomer from his responsibility to pay undisputed charges prior to their becoming delinquentnor constitute a waiver or modification of the provisions set forth in this Guidebook providingfor the temporary suspension of service or the termination of the service for nonpayment ofundisputed, delinquent charges.

    Service may be discontinued for failure to post a required deposit or guaranty after theCompany has furnished ten (10) days’ written notice to the customer requiring the customer tofurnish such deposit or guaranty. Service shall not be discontinued on a day when the officesof the Company are not available to facilitate reconnection of service or on a day immediatelypreceding such day.

    Upon termination of the service, the amount of the deposit with accrued interest shall beapplied in payment of any unpaid charges for exchange or long distance service and thebalance, if any, shall be returned to the customer within twenty-one (21) days of the renditionof the final bill.

    The amount of the deposit, with accrued interest, will be promptly returned or credited to thecustomer's account upon payment of all undisputed charges, provided these charges were notdelinquent during the last twelve (12) billing periods.

    b. Business Service

    If it is deemed necessary by the Company in safeguarding its interests, applicants for serviceor present customers may be required to make a deposit of an amount not to exceed two (2)months' exchange service charges plus two months' estimated long distance service chargesto be applied in payment of any unpaid charges for exchange or long distance service whichmay be rendered. Deposits held thirty (30) days or more will bear interest at a rate which isequal to one percent (1%) above the prime lending as published in the Wall Street Journal.The rate shall be adjusted annually on December 1 using the prime lending rate as publishedin the Wall Street Journal on the last business day of September of each year.

    An applicant for service or a present customer may satisfy a deposit requirement by providinga Contract of Guaranty in an amount not less than the requested deposit from a presentcustomer acceptable to the Company. The guaranty contract shall be on a form provided bythe Company which shall include the Company's right to transfer charges to the limit of theguaranty, from a defaulted bill of the customer, from whom a deposit or a Contract of Guarantywas required to the guarantor's account or accounts and the further right to suspend theguarantor's service pursuant to Paragraph D.2., b. of this Section.

    The fact that a deposit has been made or a guaranty provided shall in no way relieve thecustomer from complying with the Company's regulations as to the prompt payment of bills,nor constitute a waiver or modification of the regular practices of the Company providing forthe temporary suspension of service or the termination of the service contract for non-paymentof bills.

    Service may be discontinued for failure to furnish a suitable deposit, when conditions appear torequire the Company to have such credit protection, after the Company has furnished five(5)days' written notice to the customer requiring the customer to furnish such deposit. If, in thejudgment of the Company, unusual risk of financial loss exists, service may be suspended afterforty-eight (48) hours' written notice has been furnished to the customer.

    Any balance of the amount deposited and credited to the customer's account is returned to thecustomer at the termination of the contract, or it may be returned at any time previous thereto atthe option of the Company when it is deemed that the customer has established satisfactorycredit.

    3. Record of Previous Accounts

    The Company maintains a record of previous accounts by name and address in its offices.

    E. USE OF SERVICE AND FACILITIES

    1. Use of Customer Service

    Local Exchange Service is furnished only for use by the customer, his family, employees orbusiness associates, persons residing in the customer's household or authorized users, exceptas the use of the customer's local exchange service may be extended to end users of PayphoneExchange Access Service, guests of a Hotel-Motel, patients of a hospital, patrons of EnhancedServices Interconnection, or as defined in Section 37, Shared Tenant Service Arrangements, of theGeneral Exchange Tariff, P.S.C. Mo. No. 35. The Company has the right to refuse to installcustomer service or to permit such service to remain on the premises of a public or semi publiccharacter, except for Payphone Exchange Access Service, when the service is so located that thepublic in general or patrons of the customer may make use of the service. At such locations,however, customer service may be installed, provided the service is so located that it is notaccessible for public use.

    The customer is responsible for payment of all charges incurred, regardless of whether suchcharges are associated with his usage or that of any of his authorized users. The Company willrender billing to the customer of record, and that customer shall be responsible for all chargesincluding any charges transferred to the customer's account from terminated accounts billed to thesame customer. The Company will not prorate nor divide billing between the customer and any ofhis authorized users.

    Local Exchange Service may be used for providing access to resold or shared customer premiseskey equipment. Where Local Exchange Service is used for this purpose, no payment either director by means of a coin-collective device, or consideration for the Local Exchange Service is to bepaid any party other than the Company, except as provided in this Guidebook.

    Local Exchange Service may not be utilized in the provision of interexchange telecommunicationsservice (MTS and WATS).

    Business Local Exchange Service may be used in the provision of interstate enhanced serviceconsistent with the FCC-mandated enhanced services exemption. Such interstate enhancedservices use is limited to the period for which the FCC enhanced services exemption is inforce.

    Except as specifically provided for in this Guidebook or Company Tariffs, Local Exchange Servicemay not be used to extend the local calling scope of the subscribing customer.

    In the event, a customer's service is discovered to be in violation of the aforementionedprovisions, the Company shall backbill to recover the undercharge. To determine the amountof undercharge, the Company shall first review its service records to determine the period ofundercharge. Should the customer dispute the period determined by the Company, thecustomer has the burden of demonstrating a shorter period of undercharge. In cases where theCompany service records do not indicate a probable period of undercharge, a period of 12months shall be used and the burden of demonstrating a shorter period shall be upon thecustomer. The undercharge shall be assessed based upon Switched Access Service charges.

    The Company, upon failure of the customer to pay the amount backbilled, shall suspendservice to the customer until such time as the backbilled amount plus late payment penalty, ifany, are paid in full.

    2. Paralleling Service

    When flat, measured or message rates are quoted in an exchange for business main station orcommercial Private Branch Exchange Service, an applicant may, at his option, select serviceunder one method of rate treatment, but the entire service furnished at the same premisesincluding additional and auxiliary service must be uniformly on the same rate basis and not ona combination of the two.

    The foregoing does not apply to:

    a. The premises of a provider of Enhanced Service where the measured service is being used toprovide one service to its patrons (Voice Messaging) and flat rate for another (TelephoneAnswering Service);

    b. Payphone Exchange Access Service;

    c. Residence customers subscribing to flat rate residence offerings for purposes of operatingBulletin Board Systems and message or measured rate service is the customer's primaryservice.

    d. Reserve Line Service

    3. Transmitting Messages

    The Company does not transmit messages but offers the use of its facilities for communicationsbetween its customers.

    4. Use of Lines of Other Companies

    When suitable arrangements can be made, lines of other telephone companies may be used inestablishing connections to points not reached by the Company's lines. In establishingconnections with the lines of other companies, the Company is not responsible or liable for anyaction of the Connecting Company.

    5. Unauthorized Attachments or Connections

    No equipment, accessory, apparatus, circuit or device shall be attached to or connected withthe facilities furnished by the Company, whether mechanically, electrically, inductively, acousticallyor otherwise, except as provided in this Part, and as authorized by the Federal CommunicationsCommission. In case any such unauthorized attachment or connection is made, the Companyshall have the right to remove or disconnect the same or to suspend the service during thecontinuance of said attachment or connection or to terminate the service.

    6. Except as otherwise provided in this Part, nothing herein shall be construed to permit the use of adevice to interconnect any line or channel of the Company with any other communication line orchannel of the Company or of any other person.

    7. Temporary Dual Service

    Any application of Dual Local Exchange Service is limited to instances involving the relocation ofnonparty line service within a serving office area (and within the same switching entity) where thetelephone number assignment and grade of service remain unchanged. It is intended, where thecapability exists, to facilitate an occasional customer short-term need for continuing service at theold location beyond its re-establishment date at the new location.

    Temporary Dual Service is a simple, nondesigned, "bridged" arrangement that can result in certainfunctional, operational and/or transmission reductions. Provided these limitations are understoodby the customer and found acceptable by both customer and Company, dual service provisioningwill not extend beyond a 30 consecutive calendar day period.

    Under such arrangements, full rates and charges for each service location is applicable.

    Service needs beyond the scope of Temporary Dual Service must be provided under standardguidebook provisions.

    F. PAYMENT FOR SERVICE

    1. Payment for Service

    a. Residence Service

    The customer is responsible for payment of all charges for services furnished the customer,authorized users, and any other charges for which the customer has agreed to be responsible,including but not limited to charges for services originated or charges accepted at thecustomer's telephone for exchange service; intrastate or interstate long distance servicecharges billed by the Company; any FCC-approved end user charge; any charges transferredto the customer's account from terminated accounts billed to the same customer; and anycharges pursuant to Paragraph D., preceding, where the customer has executed a Contract ofGuaranty. A customer shall have at least 21 days from the rendition of a bill to pay thecharges stated.

    Effective February 1, 2009, residential customers will have a Late Payment Charge of $5.50plus 1% of the unpaid balance applied to accounts which have charges greater than $5.00carried over to the next monthly bill. When the balance carried over is in dispute, the LatePayment Charge may be applied and adjusted later, if appropriate, based on the resolution ofthe claim.

    If the customer notifies the Company before the next bill is generated, the Company willexempt the disputed portion of the bill from the Late Payment Charge until the claim isresolved. If the claim is resolved in favor of the Company, the Late Payment Charge will beapplied if appropriate.

    Residential customers may have a Convenience Fee apply in each instance when thesubscriber requests a Company Representative's assistance in processing a payment. Thesubscriber will be informed by the Company Representative of the charge prior to processingthe subscriber's payment. The charge will not apply to payments made via the automatedpayment option of the Company's interactive voice response (IVR) systems, payments madeonline via the Company's website, or payments made by mail../1/.

    Convenience Fee (per payment request) $5.00

    /1/ The fee will not apply when automated payment systems are unavailable due to systemproblems.

    Upon nonpayment of any undisputed, delinquent charge due the Company, the Companymay, after a written notice has been furnished to the customer, without incurring any liability,forthwith discontinue the furnishing of said service../1/ This written notice shall be sent ordelivered to the customer at least ten (10) days prior to the date of the proposeddiscontinuance. Basic local telecommunications service may not be discontinued for customernon-payment of a delinquent charge for other than basic local telecommunications services.Basic local telecommunications service shall not be discontinued on a day when the offices ofthe Company are not available to facilitate reconnection of service or on a day immediatelypreceding such day.

    The Company may place global toll blocking and eliminate any optional, non-basic callingfeatures and functions for customer nonpayment of delinquent charges for other than basiclocal telecommunications service.

    At least 24 hours preceding a discontinuance, the Company shall make reasonable efforts tocontact the customer to advise him of the proposed discontinuance and what steps must betaken to avoid it.

    The Company shall postpone a discontinuance for a time not in excess of 21 days if thetelephone is necessary to obtain emergency medical assistance for a person who is a memberof the household where the service is provided and where such a person is under the care of aphysician. Any person who alleges such emergency shall, if requested, provide the Companywith reasonable evidence of such necessity. Any applicant or one for whom an application ismade, owing the Company for service furnished under a former contract, shall pay or makesatisfactory arrangements for paying any bill outstanding and unpaid for such service beforeany additional service will be furnished.

    When some or all of a Customer's services have been discontinued in accordance with thethis Guidebook, and have been terminated through the completion of a Company serviceorder, the Customer may be held responsible for fees associated with collection efforts,including attorneys' fees.

    /1/ Pursuant to Paragraph 390 of the FCC’s Final Report and Order in Case No. CC 96-45, aLifeline customer’s local service shall not be disconnected for non-payment of toll charges.
