  • Annex 1 ‘Fruit Day’ 2008

    Mobilizing Support from Schools

    Aims/Objectives

    The purpose of this event is to encourage home-school cooperation in all local schools by responding the call to organise a campus fruit party on 17 April 2008 or a day close to it so that teachers and students can enjoy fruit together casually. In addition, schools can mobilize their students to carry out more creative and caring activities for encouraging their peers or helping members of the community to increase fruit consumption. Details

    2. The event is co-organised by the Education Bureau, the Department of Health (DH), the Committee on Home-School Co-operation and other key stakeholders with the support from Federations of Parent-Teacher Associations in all districts in Hong Kong. All pre-primary, primary and secondary schools are invited to participate in the programme. 3. The DH will provide relevant health education materials such as online resources and ‘Fruit Party’ banner to all participating schools for use on the ‘Fruit Day’. ‘Fruit Diary Cards’ will be provided to students of pre-primary and primary schools to foster daily consumption of fruit in an interesting way. 4. Schools interested to take part in this meaningful event are requested to reply between 14 January 2008 and 5 p.m., 31 March 2008. Pre-primary schools should complete the attached form and fax it to 3540 4153. Primary and Secondary schools should make online registration at http://school.eatsmart.gov.hk. A commendation trophy will be awarded to each participating school. Please refer to the relevant briefs attached for details:

    ‘Fruit Day’ 2008 Mobilizing Support From Schools

    Latest researches have shown that the risk of major killer diseases such as heart diseases, stroke, diabetes and certain kinds of cancer could be reduced by adequate consumption of fruit and vegetables. However, inadequate fruit consumption among our students as revealed by surveys is worthy of attention.

    Our department introduced the [email protected] Campaign in collaboration with a number of organisations during the last school year. An important initiative, the ‘Fruit Day’ 2007 event for all primary school students, received zealous support from the school community. To mobilize greater support for healthy eating, the ‘Fruit Day’ event will be organized again and extended to secondary schools and pre-primary institutions in this school year.

    The purpose of this event is to encourage home-school cooperation for creating an environment and atmosphere that is conducive to eating fruits every day. We would like to call on your support to organize a campus fruit party on 17 April 2008 involving teachers and students so that they can enjoy fruits together in an enjoyable atmosphere. In addition, schools can mobilize their students to carry out creative and caring activities by encouraging their peers or helping members of the community to increase fruit consumption. Teaching and reference materials for this purpose are available on the thematic website (http://school.eatsmart.gov.hk).

    We cordially invite your school to join “Fruit Day” 2008.     [email protected]’ Campaign ‘Fruit Day’ 2008 -- A Brief Introduction of the Activities for Pre-Primary School Students

    Organizers and Supporters The event is co-organized by Department of Health, Education Bureau, Radio Television Hong Kong, Hong Kong Education City, Home-School Co-operation Committee, Hong Kong College of Cardiology and World Cancer Research Fund (Hong Kong), with the support from Food and Environmental Hygiene Department and Federations of Parent-Teacher Associations in all districts in Hong Kong. Background Latest studies have confirmed the essential role of fruit and vegetables in healthy eating and their benefit in reducing the risk of common killer diseases (such as heart diseases, stroke and certain kinds of cancer). To maintain a balanced diet, we recommend that children between 3 and 6 years old should have one serving of fruit every day. Purpose The event aims to encourage all teachers and students in participating schools to eat fruit together on that day to demonstrate their commitment of sustaining the habit. The ‘Fruit Diary Card’, which records the fruit eating habit in an ongoing manner, helps to sustain the habit by making it more enjoyable and rewarding. Items and Dates of Events Schools which have registered for the event may join one or more of the following activities - Activity Date or Period Particulars (1) Organize ‘Fruit Party’ in school campus on ‘Fruit Day’

    Participating schools may provide a compact disc with sidelights (photographs or descriptions of activities) of ‘Fruit Day’ activities by 30th April 2008 for the Department of Health to upload to the EatSmart website. Remarks - 1. Each school may provide four photos

  • Details of Registration and Participation 1. From 14 January to 5 p.m. on 31 March 2008, interested schools may send the

    registration form by fax to the Department of Health.. 2. Name, estimated number of participants and period of proposed activity shall be submitted

    with registration. 3. List of participating schools and name of activities will be shown online. Trophies will be

    presented to participating schools for commendation. Participation Details 1. One ‘Fruit Day’ banner, two posters and one set of ‘Fruit Diary Cards’ for each student (with

    one additional sheet of fruit stickers for each of the juniors) will be provided by the organisers. Should there be need for extra copies, schools may call 2572 1476 to make their requests, or they may design and produce their own version for the purpose.

    2. Would schools please issue a notice to parents (Reference may be made to the template at http://school.eatsmart.gov.hk ) to inform them about the participation of this territory-wide ‘Fruit Day’ event where all teachers and students at school will eat fruit together on that day and parents are also encouraged to have fruit together with their children on the same day.

    3. It is recommended to use the ‘Fruit Diary Cards’ on the school days within the period of 4-8 weeks immediately following ‘Fruit Day’ for encouraging students to keep eating fruit every day, so as to build up a good practice. Schools may encourage parents to help children mark their records after eating fruit, with fruit stickers for juniors and drawings of fruit on the cards for the seniors. School heads may present certificates of commendation (downloadable at http://school.eatsmart.gov.hk ) to participating children after the ‘Fruit Diary Cards’ activity. Schools may also prepare their own prizes or design their own ways of award for encouraging purpose.

    4. School may take photographs of the activities for upload to http://school.eatsmart.gov.hk . Arrangement about ‘Fruit Day’

    Schools may arrange a refreshment break for teachers and children to enjoy fresh fruit together.

    Issue a notice to parents and promote ‘Fruit Day’ to children and encourage parents to eat fruit together with their children on the day.

    Expectation from Participation Schools 1. Issue a notice to parents (Reference may be made to the template at http://

    school.eatsmart.gov.hk) informing them that teachers and students in the school will eat fruit together on the “Fruit Day”.

    2. Encourage students to use the ‘Fruit Diary Cards’ for recording their daily fruit consumption in the month following ‘Fruit Day’. Schools may arrange parents, teachers or students’ representatives to stamp on the cards against students’ claim of eating fruit. Head of school may present the certificate of commendation designed by the organisers (downloadable at http:// school.eatsmart.gov.hk) to those students who have got the specified number of stamp marks. Schools may also prepare their own prizes or design their own ways of award.

    Arrangement for Fruit 1. Schools could take the initiative to discuss with suppliers for arranging the provision of fresh

    fruit on ‘Fruit Day’; and / or 2. Issue a notice to parents and remind them to prepare fruit for their children on the “Fruit Day”;

    The Updating of “Data Bank of Hong Kong High Achieving Students”

    Aims / Objectives

    The purpose of this annex is to invite schools to provide information of their students who have won awards in the international or national competitions/events.

Seeing that a number of Hong Kong students have received awards in major international and national competitions, the Education Bureau (EDB) established a “Data Bank of Hong Kong High Achieving Students” in 2004. In addition, a “Young Achievers’ Gallery”, situated at the Podium, West Block of the EDB Kowloon Tong Education Services Centre, has been launched since December 2007 to showcase the achievements of these outstanding students and to inspire others to follow their footsteps. In order to facilitate the updating of the Data Bank, an online programme has been developed for schools to provide the EDB with the latest information of the award winning students. 3. Schools may visit the EDB web site (http://www.edb.gov.hk/nomination) any time to update information of school and award winning students. The login ID and default password are set to be the 12-digit SCRN given to each individual school by the EDB. Schools are advised to change the password after the first login. Please follow the online instructions to update information. After the updating, schools are requested to print out the “Individual Award Record Form cum Consent Letter”, fill in the necessary information and return the completed form together with the photocopies of the related supporting documents of individual awards by post to the Gifted Education Section, Curriculum Development Institute, EDB at Room E328, 3/F, East Block, EDB Kowloon Tong Education Services Centre, 19 Suffolk Road, Kowloon Tong.     “One Country, Two Systems” Historical Study Tours

    Aims/Objectives

    The“One Country, Two Systems” Historical Study Tours aim to enhance students’ understanding of the current developments of our motherland and their national identity through the experience gained in the tours and the interaction with Mainland students.

    Details 2. The Historical Study Tours are sponsored by the Education Bureau and organised by the National Education Centre. The following two routes are specially designed for both primary (P.4 – P.6) and secondary (S.1 – S.3) school students:

    Sino-British Historical Study Tour; China Reform Study Tour.

    3. The study tours will be held in March 2008. Heads of schools are invited to nominate not more than 40 students to participate in one of the study tours, and corresponding number of group leader(s) to accompany the students to facilitate their learning. Please refer to Appendix 4a for details. 4.     簡介 上述活動由教育局贊助、國民教育中心舉辦、中央人民政府駐香港特別行政區聯絡辦公室

    教育科技部協辦,將於 2008 年 3 月在深圳和中英街(為期一天),以及廣州(為期兩天)舉行。對象為高小(小四至小六)和初中(中一至中三)學生,校長和教師亦可隨團參加。

    現邀請全港中、小學校申請。由於名額有限,所以每所學校的學生參加名額最多為 40 人

    (有意申請的學校必須按照 1 比 10 的師生比例委派校長/教師/家長擔任隨團導師(師生

    比例計算方式以 10 人為單位,如學生人數不足 10 人,亦作 10 人計。例如:學生人數為

    23 人,便作 30 人計算,學校須委派 3名隨團導師)。獲取錄的學員須於旅程後呈交學習報

    告。「中英街歷史文化之旅」將舉辦 4 團,而「中國改革開放成就研習之旅」將舉辦 2 團。

    每團人數為 160 名學生和 16 位隨團導師。舉辦日期如下:

    及民生狀況,達致了解國情的目的;

    3. 讓學生協助教師在校內推行國情教育。

    特色 為了促進學生的學習,整個旅程包含豐富的學習元素,包括透過與內地學生和市民的交

    流,深入認識內地與香港生活文化的差異;透過參觀文物古蹟,追溯中國文化根源,加強

    學生對國民身份的認同。完成整個學習之旅的校長、教師及學生可獲頒發證書。

    資助 獲取錄參加「中英街歷史文化之旅」的學生需支付每人港幣 140 元;而參加「中國改革開

    放成就研習之旅」的學生則需支付港幣 240 元,包括研習費、食宿、交通、團體旅遊保險

    及入場費等。隨團導師費用全免。不足之數由教育局及國民教育中心資助。

    參觀深圳求水山風景區。區內根據山形走勢,按比例修建的「長城」,其城墻、烽火台

    等與北京八達嶺長城相似,氣勢雄偉。沿「長城」拾級而上,可一路領略風景,山嶺

    俯瞰「南國明珠」,別有情趣。

    參觀深圳市少年宮,了解深圳青少年的課外活動情況。其他內容包括能源天地、科普

    王國、美麗家園、生命探索等。

    “4.23 World Book Day in 2008” Creative Competition on “Sports and I”

    Aims/Objectives

    The theme of the “4‧23 World Book Day in 2008” Creative Competition, organised by the Hong Kong Public Libraries, is “Sports and I”. The competition aims to promote reading and writing. It also promotes the universal and cultural values conveyed by the Olympic Games, which is to give one’s best and to strive for personal excellence while at the same time promote “Unity, Friendship, Progress, Harmony, Participation and Dream”.

    2. All local day schools are invited to nominate their students (Primary 1 – Secondary 7) to participate in the event. The competition consists of 7 categories, namely, Junior Primary, Senior Primary (Chinese), Senior Primary (English), Junior Secondary (Chinese), Junior Secondary (English), Senior Secondary (Chinese) and Senior Secondary (English). Each school can nominate up to 5 entries for each category. For details, please refer to Appendix 5a. 3.     Entries of Junior Primary Category should be designed on A3 size (297 x 420mm) drawing paper.

    Entries of other categories should be computer-printed or clearly written in black ballpoint pen on A4 size paper.

    The photo or picture for the entries of Senior Secondary categories should be stuck on

    Aims/Objectives The aim of this Annex is to inform primary schools of the “Library Cards for All School Children” scheme and the arrangement of bulk application for library cards of the Hong Kong Public Libraries or using Smart Identity Cards as identity cards allowed for library purposes for primary 1 to 6 pupils. Details 2. ‘Life-long learning’ is the main thrust of our curriculum reform. Libraries play a pivotal role in the promotion of life-long learning. Pupils should fully utilize services and facilities provided by the school libraries as well as the Hong Kong Public Libraries. 3. The Education Bureau and the Leisure and Cultural Services Department jointly organised the “Library Cards for All School Children” scheme to encourage pupils to make full use of services provided by the Hong Kong Public Libraries. Schools should encourage their pupils to apply for Hong Kong Public Libraries library cards or using Smart Identity Cards as identity cards allowed for library purposes. 4. Ways to apply for Hong Kong Public Libraries library cards or using Smart Identity Cards as identity cards allowed for library purposes:

    (a) Direct Application Pupils may visit any public libraries with their parents to apply for and

    receive library cards / library card numbers for Smart Identity Cards directly. Applicants and their parents should present supporting documents showing their residential addresses and personal identifications.

    (b) Bulk Application through Schools Pupils may also apply through bulk application at schools. Library cards /

    library card numbers of Smart Identity Cards will be issued in about two months. Please refer to Appendix 6a for details.

  • Appendix 6a “Library Cards for All School Children” Scheme

    (Jointly organised by the Education Bureau and the Leisure and Cultural Services Department)

    Objective

    The Hong Kong Public Libraries of the Leisure and Cultural Services Department provides public library services to meet the community's need for knowledge, information and research, to support life-long learning, continuous education and profitable use of leisure time, and to promote local literary arts. The “Library Cards for All School Children” scheme is jointly organised by the Leisure and Cultural Services Department Hong Kong Public Libraries and the Education Bureau. It aims to encourage primary school pupils to use public library resources for life.

    Application Procedures for Bulk Application for Library Cards of the Hong Kong Public Libraries or Using Smart Identity Cards as Identity Cards Allowed for Library Purposes

    1. Collection of Application Forms Teachers-in-charge may contact the Librarians / Assistant Librarians of the

    branch libraries in their districts for collection of application forms (please refer to Appendix 6b for contacts of the branch libraries) or download the application form from the web site of Hong Kong Public Libraries (http://www.hkpl.gov.hk/tc_chi/aboutus/aboutus_forms/files/libcard_app.pdf and http://www.hkpl.gov.hk/tc_chi/aboutus/aboutus_forms/files/lcs694.pdf). Pupils may apply for “library cards” or “using Smart Identity Cards as identity cards allowed for library purposes”. Those applying for the latter must possess valid Smart Identity Cards. For application of library cards, please refer to the application form at Appendix 6c(i). For application of using Smart Identity Cards as identity cards allowed for library purposes, please refer to the application form at Appendix 6c(ii).

    2. Return of Duly Completed Application Forms

    Procedures for collecting application forms:

    i. separate application forms of “library card” from the application of “using Smart Identity Cards as identity cards allowed for library purposes”* .

    ii. check the application forms for the following:

    a. If the pupils have provided all necessary information on the application forms;

    b. If the pupils have signed the application forms; and c. If the part ‘Guarantee’ is duly completed and have included signatures of

    their parents/guardians.

    iii. stamp all verified application forms with school chop. iv. return the completed application forms, reply slip(s) and name list(s) of pupils* (to

    be grouped separately under “library card” and “using Smart Identity Cards as identity cards allowed for library purposes”) to the Librarians / Assistant Librarians of the branch libraries in their districts.

    (Appendix6c(i) and Appendix 6c(ii)) for reference. 3. Issue of Library Cards / Library Card Numbers for Smart Identity Cards Library cards / library card numbers for Smart Identity Cards# will be issued in about 2

    months after the application. Teachers-in-charge will be informed of the arrangement for the collection of library cards / library card numbers for Smart Identity Cards.

    4. Deadline The deadline of bulk application is 30 June 2008.

    #Successful applicants will be issued with a “library card number” to access telephone renewal and online services (including

    borrower record enquiry, renewal and reservation of library materials) of the Hong Kong Public Libraries. When borrowing

    or renewing library materials at library counters or self-charging terminals, Smart Identity Cards should be used to complete

    the procedure.

  • SUMMARY OF THE AMENDED LIBRARIES REGULATION

    (GOVERNING THE USE OF THE PUBLIC LIBRARIES)

    (The complete set of Libraries Regulation may be consulted on request at any of the Hong Kong Public Libraries)

    1. Lending library materials may be borrowed for a period of 14 days. 2. Where a library material is returned, it may be borrowed by the same borrower for a further

    period of 14 days unless it is required by another borrower. 3. A charge will be made for library materials returned late at the rate of HK$1.50 a day or part of a

    day for each adult library material or HK$0.50 a day or part of a day for each junior library material overdue.

    4. Each registered borrower may borrow up to six items of library materials. 5. A borrower shall not lend or transfer his/her library card or ID card allowed for library purposes

    to any other person. 6. A borrower shall forthwith report to the librarian in writing loss of his/her library card or ID card

    allowed for library purposes. 7. The following fees shall be payable on the issue of replacement library card:

    a. for persons of or above the age of 18 years, HK$25; and b. for persons under the age of 18 years, HK$13.

    8. Holder of ID card allowed for library purposes may apply for the use of his/her replacement smart ID card for library purposes when his/her smart ID card is lost and no fee will be levied on the application.

    9. Unless a library card or ID card allowed for library purposes has been reported lost in writing, the card holder will be held responsible for any library material issued on it.

    10. A fee of HK$2.50 will be charged for the reservation of any library material. 11. When a borrower changes his/her address, he/she should notify the Library immediately. 12. Library cards or ID card allowed for library purposes may be used at any of the Hong Kong

    Public Libraries. 13. Except with the permission of the Librarian or when accompanied by an adult, a person under 12

    years of age shall not enter or use the Adult and Reference Library. 14. A borrower shall, before leaving the Library, ensure that any library material issued to him/her is

    complete and undamaged. 15. A charge will be made for any library material lost or damaged, which will be such sum as the

    Librarian considers necessary to replace the library material or set of library materials which the library material forms a part. In addition, a surcharge of 20% of such sum shall be payable.

    16. A borrower may apply to cancel his/her library card or discontinue the use of his/her smart ID card as ID card allowed for library purposes.
