Attitudes of senior registrars in geriatric medicine toresearchJ. Taylor, J.B. MacDonald and D.J. Stott'Geriatric Unit, Gartnavel General Hospital and' University Department ofGeriatric Medicine,Southern General Hospital, Glasgow, UK

To determine the attitudes of senior registrars in geriatricmedicine to research, a postal survey was conducted usinga role of senior registrar posts provided by the BritishGeriatrics Society. The questionnaire was a modifiedversion of that produced by Gaylard' and includedsections recording personal characteristics, type of postand unit, previous research experience, numbers ofpublications, supervision of research and intention topursue research interests at consultant level. The ques-tionnaires were returned and analysed anonymously.The response rate of 92/145 (64%) was comparable

with other postal surveys."2 Approximately half of thosewho replied were in geriatric medicine posts and half injoint geriatric medicine and general medicine positions.At the time of reply: 40% were working in district generalhospitals, 49% in teaching hospitals and 11% in univer-sity departments. More than half (53%) had previouslyheld pure research posts. Only 30/92 of those who repliedhad any formal training in research and, of these, 27 hadpreviously held pure research posts. The median numberof publications for the whole group was six (range 0- 38).The type of post or unit in which a senior registrar wasworking had no effect on the number of publications.However, those who had previously held a research posthad a significantly higher number of publications thanthose who had not (median 10 compared to median fourrespectively, Wilcoxon rank sum test P<0.0001). Thiswould suggest that the training, techniques andexperience gained in research posts may be very success-fully applied to research in geriatric medicine.The majority of senior registrars (92%) were actively

involved in research. Most were able to seek advice on aproject from a senior member of staff. The majority ofsenior registrars felt that publications on the curriculumvitae (CV) were very important when applying for seniorregistrar (75%) or consultant posts (80%). However, 72(78%) said that they would still attempt to publish even ifit was not necessary for CV enhancement and 87% still

hoped to pursue their research interests at consultantlevel. This suggests that there is a genuine interest inresearch in geriatric medicine and that career advance-ment is not the sole motivation for producing publica-tions.

Finally, the main problem encountered by seniorregistrars in attempting to carry out research projects wasthat of inadequate time. Equal difficulty occurred indistrict general hospitals, teaching hospitals and inuniversity departments. The mean number of sessionsdevoted to research was 1.6 a week (range 0-4 sessions)which is below the JCHMT (Joint Committee on HigherMedical Training) guidelines of two sessions per week forresearch and personal study.3 An even more worryingfinding was that 16% of senior registrars who replied hadno specific time allocated for research whatsoever.This survey has shown that there is a high level of

research activity amongst senior registrars in training ingeriatric medicine, despite inadequate provision ofresearch time in many posts. Informal advice on researchmatters is readily available from supervising consultantsin geriatric medicine; however, formal training in researchmethods is largely restricted to those in pure researchposts. This is likely to adversely affect the quality ofresearch performed in geriatric medicine.

Senior registrars who have previously held a pureresearch post are by far the most productive in terms ofpublications; therefore they are likely to have an advan-tage as numbers of publications are often a major factordetermining promotion. Surprisingly, the type of post orunit in which the senior registrars worked did not appearto influence research activity. It is encouraging that a veryrespectable level of research activity is maintained bybusy district general hospitals. However, it is likely thatthe large clinical commitment for doctors in teachinghospitals and academic departments restricts theirresearch activities.

Senior registrars in geriatric medicine appear to begenuinely interested in research, as most would continuetheir involvement even if it was not required for careeradvancement. With modest additional training inresearch techniques, and provision of adequate time forresearch, as already recommended by the JCHMT, thecurrent enthusiasm and commitment to research ingeriatric medicine could, be harnessed and directed tomuch greater effect.

Conference Report

National Association of Clinical Tutors (NACT)Summer Meeting at BangorMay 15-16, 1992

The morning session was devoted to Perceptions ofTraining and was chaired by Dr Peter Wilkinson, theChairman of the NACT. The first speaker was ProfessorIan Houston, Dean ofPostgraduate Medicine, Universityof Manchester, who looked at the preservation of educa-tional training standards in the face of new workingarrangements. He entitled his talk under the headingSWOT-Strengths, Weaknesses, Opportunities andThreats, and under each of these headings he dealt withthe finances involved, organization, time, communica-tion, morale, and quality. He was of the opinion that thenew working arrangements made for a more coherenteducational approach and certainly a more representa-tional attitude. There were at last dedicated staff anddedicated funds which he thought would improve theoverall picture.He was followed by Dr Ilfra Goldberg the Deputy

Dean of Postgraduate Medical Studies, University ofManchester, who talked on the attitudes towards theeducation of pre-registration house officers. She thoughtthat there was potential for conflict here and that financialconstraints would play a very important part in educa-tion. She explained how the University did theirassessment visits and how their in-depth assessmentquestionnaires were made up.The next speaker was Dr Dianne Kelly, a general

practictioner from Kirkintilloch in Scotland, who dealtwith the adequacy of vocational training for generalpractice. In the survey which she had carried out it wasvery apparent that there was a very poor quality ofeducational time for senior house officers (SHOs) inhospital and that there was a deep conflict betweeneducational needs and service requirements. She felt thatbefore any SHO had been accepted for, or had decided toaccept a post in the hospital, that guidelines by consultantand SHO should be laid down at the beginning.

Professor Tom Hayes, Director and Dean of Post-graduate Studies at the University of Wales College ofMedicine, Cardiff, gave a short paper on the educationalexperience of Welsh SHOs. He personally was becomingmore and more depressed by the business administrationof the ward round and when he started his survey he wasamazed to find that so many consultants were unaware ofany difficulties in the system.

His was the most illuminating lecture of the morningsession and it showed that there was a lesson to be learntnot only by consultants but by clinical tutors. By andlarge he found that SHOs learnt from unstructuredteaching and were now asking for more formal teaching.However, it was a sad reflection on the whole SHO careerstructure that by far the most important thing to the SHOwas a good reference. He discussed the role of the collegetutor but it was quite clear from his survey that SHOsthought that consultants and clinical tutors should beresponsible for career advice. His survey showed thatpostgraduate medical education was poor in 12 out of 18hospitals and that there was an inappropriate variation

from hospital to hospital. In the teaching hospitals theSHOs worked longer hours, they carried out moreinappropriate duties and they had less satisfaction withtheir job. The non-teaching hospital SHO had lessinappropriate duties and more satisfaction. In general thecareer advice was poor and there was an exceptionallypoor feedback on performance of their duties. He stressedthat it would be much better for Regional and Post-graduate Deans to be responsible for the educationalexperience of SHOs.The next hour was devoted to a small group session

when the educational needs of pre-registration houseofficers, do-it-yourself and vocational training schemeswere discussed.The afternoon session was given over to the place of

research in training and was chaired by Dr GordonJackson, Hon. Secretary of the NACT. The first speakerwas Professor Miles Irving, Professor of Surgery, Univer-sity of Manchester, who spoke on the place of research intraining to be a surgeon. He thought that exposure toresearch is essential in surgical training and that today'ssurgeon has to deal with a torrent ofnew innovative ideasand that research should be built into basic surgicaltraining. He discussed the arguments for and againstdifferent timings of research but he thought that a periodof well-planned, supervised research played a vital role inthe training of a surgeon and he emphasized that researchprogrammes should be inspected regularly.Mr Frank Arnold, a Manchester surgeon, took a

contrary view, suggesting that critical evaluation ofresearch was much more important than actually doingthe research.Two consultant physicians, Dr G. Neale from Cam-

bridge and Dr R. Fisken from Northallerton, bothconsidered the role of research in training to be aphysician. They felt that it had an important place intraining but to be worthwhile it should be of high qualityand it should not simply duplicate what is already known.It should certainly encourage critical thinking and judge-ment. They felt that research and publications hadbecome unduly prominent in the process of consultantselection and that other selection criteria now neededevaluation. These might include relevant skills, servicework and management training.The next morning, Dr Neville Hodges, Postgraduate

Organizer at Bangor, chaired the morning session whichwas devoted to arrangements for funding postgraduateeducation.Dr Hodges apologized that despite searching the length

and breadth of the country he could find no RegionalManager willing to come forward to talk on the manage-ment approach to setting funding for postgraduatemedical education.Dr T.J. Bayley, the Postgraduate Dean of the Univer-

sity of Liverpool, spoke on problems with financingpostgraduate education. He saw the problem as fourfold:conforming to the reforms; working with the appren-ticeship model; service underfunding as a whole; and,most importantly, historical budgets. He discussed allthese problems in full and offered as solutions: monitor-ing education and audit in NHS Trusts; overcoming

'historicity'; removing the consultant study leave fromSPLIT; and working out strict educational strategies.

Unfortunately Dr F.J. Woodroffe, the Vice Chairmanof the NACT, could not stay for Saturday morning andhis paper on 'Contracts up and down the UK' waspresented by Dr Jackson. He described three types ofcontracts that were being arranged throughout thecountry: 1. the situation in some regions where they feelthey don't need contracts; 2. the contract between theDean, the Unit and the Clinical Tutor; and 3. the straightcontract between the Dean and the Clinical Tutor.

In the second type of contract, the Unit provides thebanking and personal management and the third type ofcontract is purely a reporting relationship. He went on todescribe the contracts which were going on in the variousregions and the various anomalies which exist on eitherside of the Pennines.Dr Janet Grant of the Joint Centre for Educational

Research and Development in Medicine gave a shortlecture on 'What place for tele-conferencing? An evalua-tion of the first year'. She went on to describe interactivesatellite television, her evaluation study and then she gavea short demonstration and explained to the audience theimplications for future development. The response of the

audience was mixed; some delegates thought that it hadbeen successful in their postgraduate centres, othersthought that it should be given a try, and some thoughtthat it was of little educational benefit and that thecontent was too minimal.Dr Gifford Batstone, the 1991 Wyeth North American

Travelling Fellow, concluded the academic programmewith a lecture entitled 'When you come to a fork in theroad - take it'. He described his travels in Boston,Toronto, and North Carolina and the insights he gainedinto medical education whilst there. He came away fromthis visit to America much wiser and much more educa-tionally alert than when he left these shores last year. Thiswas a most enjoyable and illuminating lecture.

Bangor was a charming and picturesque venue for thisexcellent NACT Meeting. The small number attendingthe Conference suggests that clinical tutors, like theirjunior staff, are also required to work under considerablepressure.

Book Reviews

Diagnostic Picture Tests in Endocrinology, edited by D.C.Evered and R. Hall. Pp. 128, illustrations. Wolfe,London, 1991. Paperback, £7.95.

This is a lovely little book: cheap, attractive, fun to dipinto and educational. A total of 215 illustrations (nearlyall are colour photographs) have been taken from theWolfe Colour Atlas ofEndocrinology and presented with ashort list of questions. The main value of the book is thebreadth of the clinical conditions covered, which enablesstudents to benefit at a glance from the experience whichtakes a lifetime for an expert to acquire.The authors state in the preface that they have tried to

deal largely with common endocrine problems, butfortunately they have failed. Doctors see commonproblems in everyday clinical practice, and the advantageof a compendium like this is the illustration of things theyare never likely to see but about which they need toknow.One of the key points is the high quality of the

photographs, but other aspects of layout are equallyimportant. The bold numbering of the questions,combined with the quality of the binding, makes iteminently easy to flick to the back and look up theanswers.

It is always possible to find fault, ofcourse, and potentialpurchasers should not be detracted from what follows.Experts would quibble with some answers (e.g. thatneurofibromatosis was not associated with medullarycarcinoma ofthe thyroid), but the world would not be thesame if they didn't. They would also note occasionalmistakes (e.g. hyperglycaemia for hypercalcaemia inQuestion 2). They would also fret about the ECG thatshowed small complexes and U waves, in whichhypothyroidism was given as a correct answer whileConn's syndrome was incorrect. Finally there are veryoccasional questions phrased in the 'not uncommonlyseen in' format, but they are the exception rather than therule.

This is a good little book. It is well worth buying.P.M. Fisher

As in the first edition, the authors deliberately eschewproviding a formal definition of clinical epidemiology. Ithas something to do with the application of the principlesof population epidemiology and biostatistics to the careof the individual patient.The structure of the book reflects the three major

aspects of clinical practice, namely diagnosis, manage-ment and keeping up to date. It applies epidemiologicalprinciples in a critical analysis of how to improve theaccuracy and efficiency of diagnosis and prognosis mak-ing, how to select diagnostic tests and interpret diagnosticdata, how to select the best therapy and aid patient com-pliance and how to assess response. The third section isconcerned with self audit of performance, the best use ofinformation sources, and continuing medical education.The major changes in this edition are in the chapters ondiagnostic tests, the interpretation of confidence limits,understanding utilities and do it yourself N-of- 1 trials.There is also a timely new chapter on how to assessreviews and meta analyses.

Putting science into the art of medicine - it entails adegree of numeracy and there are equations - might seema dull business but not a bit of it. The authors' enthusiasm,nay passion, for their brief - this overflows into anynumber of footnotes - combined with an informal andlighthearted approach should inspire the reader who app-reciates the potential here for improving his or her clinicalperformance. Clinical Epidemiology is wholeheartedlyrecommended and should be mandatory reading forthose who teach medicine to imbue important principlesat a stage where they will have the greatest long-termeffects.

The Postgraduate Medical Journal considers manu-scripts prepared in accordance with the guidelines laiddown by the International Committee of MedicalJournal Editors (Br Med J 1988, 296: 401-405). Allmaterial submitted is assumed to be submitted exclus-ively to the Postgraduate Medical Journal unless thecontrary is stated.Typescripts Two complete copies should be sent tothe Editor, Postgraduate Medical Journal, 6 StAndrew's Place, London NW1 4LB. Papers must betynewritten, double-spaced, on one side of paper not. .ger than A4 (297 mm x 210 mm), with a 5 cmmargin. The first page ofthe typescript should bear thenames of the author(s) and the name and address ofthe laboratory or institution where the work has beencarried out, in addition to the title of the paper. Thefull address and fax number of the principal author towhom proofs will be sent should be given as afootnote, as should any permanent changes ofaddressand/or appointment. A short (running) title of notmore than 45 characters should be given. All pagesshould be numbered including the title page. In linewith many other journals, we do not normally returnrejected manuscripts.The principal author must ensure that any co-authorslisted agree to submission of the typescript. Anywritten or illustrative material which has been or willbe published elsewhere must be duly acknowledgedand accompanied by the written consent of theauthors and publishers concerned.Arrangement Papers should be divided into; (a) Titlepage, (b) Summary, (c) Introduction, (d) Materialsand methods, (e) Results, (1) Discussion, (g) Ack-nowledgements, (h) References, (i) Tables, () Figuresand captions. The summary should not exceed 250words and should state concisely what was done, themain findings and how the work was interpreted. In allsections of the paper numbered paragraphs should beavoided if at all possible.Style Abbreviations and symbols must be standardand SI units used throughout. Acronyms should beused sparingly and must be fully explained when firstused. Whenever possible drugs should be given theirapproved generic name. Where a proprietary (brand)name must be used, it should begin with a capitalletter. Statistical analyses must explain the methodsused. The use of footnotes is not permitted. Singlequotation marks should be used and words to beitalicized should be underlined. The Concise OxfordEnglish Dictionary is used as a reference for all spellingand hyphenation.References should follow the Vancouver format. Inthe text, they should appear as numbers starting at 1.At the end of the paper they should be listed (double-sp aced) in numerical order corresponding to the orderofcitation in the text. All authors should be quoted forpapers with up to six authors; for papers with morethan six authors, the first three only should be quotedfollowed by et al. Abbreviations for titles of medicalperiodicals should conform to those used in the latest

Figures In the text Arabic numbers should be usedand all illustrations should be specifically referred to inthe text, e.g. (Figure 2). All illustrations should besubmitted at about l{ times the intended final size andshould be numbered as figures whether they arephotographs, representational drawings or line diag-rams and graphs.Photographs and photomicrographs should beunmounted glossy prints and should not be retouched,and should be chosen to exclude technical artefacts.Magnification is best indicated by a line representing adefined length included within the photographs. Areasof key interest and/or critical reproduction should beindicated on a flimsy overlay attached to the photo-graph or on a photocopy. All annotations and letter-ing should be indicated in the same way, andpreferably not included on the original print. Clearlycontrasted and focused prints are essential for ade-quate reproduction.Line diagrams and graphs should be on separate sheets;they must be drawn with black Indian ink on whitepaper, or supplied as photographic prints of suchoriginals. Lettering on figures should be minimal andmust not duplicate the legend. The use of symbolsshould be consistent within papers, and explanationsof symbols should be included in the caption, not onthe figure.A photocopy of all illustrations should be submitted.Tables These should be as few as possible and shouldpresent only essential data. They should be typewrit-ten on separate sheets, have a title or caption, andgiven Roman numbers.Proofs Two marked copies of the proofs will be sentto the principal author which should be read carefullyfor errors. One corrected copy must be returned to theeditor within 3 days. Major alterations to the textcannot be accepted.Copyright assignment The principal author mustcomplete and return to the Publisher the CopyrightAssignment Form enclosed with the proofs.Offprints A copy of the journal will be sent to theprincipal author. Offprints may be ordered on theform accompanying proofs. The charges are neces-sarily higher if orders are received after the issue hasgone to press.Drug side effects Manuscripts reporting adverse drugreactions should be accompanied by evidence showingthat the reaction has been reported on a 'yellow card'or to the appropriate drug licensing authority, and tothe drug manufacturer.
