SUMMARY The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensee’s performance by conducting an integrated inspection at Duane Arnold Energy Center, Unit 1 in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRC’s program for overseeing the safe operation of commercial nuclear power reactors. PLANT STATUS Duane Arnold operated at or near rated thermal power for the entire inspection period. INSPECTION SCOPES

Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, “Light-Water Reactor Inspection Program - Operations Phase.” The inspectors performed plant status activities described in IMC 2515 Appendix D, “Plant Status” and conducted routine reviews using IP 71152, “Problem Identification and Resolution.” The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards. INSPECTION RESULTS

Observation: Integrated Control Room Annunciator Issues 71152 The licensee documented numerous control room annunciator issues beginning in May 2019 for the 1C05B panel alarms. The initial issue was a failure of the annunciator horn to silence. As part of the trouble shooting, all the alarm cards on 1C05B were removed and re-seated. Subsequent issues included failure of some 1C05B alarms to annunciate when tested and 1C05B alarm power supply fuses blowing. These were all addressed as single issues and corrected individually. Early in August 2016, the licensee wrote a condition report to address the aggregate 1C05B annunciator issue. The licensee determined that the alarm cards were not fully seated during the trouble shooting for failure of the annunciator horn to silence in May 2019. After re-seating the alarm cards, 1C05B annunciator issues ceased.

Observation: Corrective Action Program for Reactor Water Cleanup Pumps Seal Leaks

71152

The inspectors reviewed the licensee's problem identification and resolution activities associated with the reactor water coolant pump seal leaks over the past six months. The licensee’s corrective action program documented alternating and sequential mechanical seal failures on both pumps. The maintenance efforts resulted in a mixed success as ‘A’ pump was returned to service with no leakage. The ‘B’ pump was returned to service with nearly 300 drops per minute. This leakage, however, was well below the technical specifications limit. The inspectors noted that continuous monitoring of reactor coolant conductivity as well as providing continuous purification of recirculation flow has been maintained during this inspection period.

EXIT MEETINGS AND DEBRIEFS The inspectors confirmed that proprietary information was controlled to protect from public disclosure.

• On October 3, 2019, the inspectors presented the integrated inspection results to Mr. D. Curtland, Director of Site Operations, and other members of the licensee staff.

• On August 15, 2019, the inspectors presented the Radiation Protection Baseline Inspection results to Mr. M. Davis, Licensing Manager, and other members of the licensee staff.
