Generic name:Vitamin B-Complex with vitamin CBrand name:Vaneular tablet
Classification:Dietary supplement
Dosage:1 tablet
Frequency:Two times a day
Route:Per Orem
Date Ordered:June 16, 2010
Recommended dose: 27 years old-
Mild cases &
for
maintenance 1
-2 tab daily
Therapeutic level:
conditions eg malabsorption syndrome; radiation sickness; malnutrition or deficiency diseases; severe infections.
Transient diarrhea
Itching- and urticarcia
Anaphylactic shock
Increased uric acid levels, increased urinary calcium
Patient education: Instruct patient strict compliance of taking multivitamins and report side-effects

Generic name:
L-ornithine-L-
aspartate
Brand name:Hepa-Merz sachet
Classification:Cholelitholytics & Hepatic Protectors
Dosage:1 sachet dissolve with ½ glass of water
Frequency:Two times a day
Route:Per orem
Date Ordered:June 26,2010
Recommended dose: 27 years old-
1-2 sachets of Hepa-Merz 1-2 times daily
Therapeutic level:
Acute and
chronic liver
disease eg, liver
cirrhosis, fatty
liver, hepatitis
L-ornithine-L-aspartate stimulates ammonia detoxification by enhancing urea synthesis in the urea cycle. Additionally, it eliminates extrahepatic ammonia from the tissues.
Severe renal impairment
Monitor laboratory results such as liver function test, and
serum creatinine level exceeding 3 mg/100 mL
Dissolve content of the sachet with plenty of liquids
Indication/contraindication
Mechanism of action Rationale for patient
Side effects / adverse effects
Nursing considerations
Generic name:Ursodeoxycholic acidBrand name:Ursofalk
Classification:Cholelitholytics & Hepatic Protectors
Dosage:250 mg/cap
Frequency:Three times a day
Route:Per orem
Date Ordered:June 16,2010
Recommended dose: 27 years old- 300 mg b.i.d.
Therapeutic level:
Treatment of cholestatic liver disease.
Supress hepatic synthesis , biliary secretion and intestinal reabsorbing of cholesterol.
CNS: Anxiety, fatigue, depression
CV: chest pain, hypertension, peripheral edema.
Gallstone calcification
Administer with food intake
Be aware of too much intake of drug, diarrhea may occur
Urge patient to notify the physician immediately for signs of acute cholecytitis
Indication/contraindication

Generic name:
Brand name:Essentiale Forte
Classification:Cholelitholytics & Hepatic Protectors
Dosage:1 capsule
Frequency:Three times a day
Route:Per orem
Date Ordered:June 16,2010
Recommended dose: 27 years old-
1-2 forte caps 3 times
a day for the
first 2 months
Therapeutic level:
Acute and Chronic Hepatitis.
Liver Damaged by
Toxins
It activates metabolic
function and supports
the energy balance of
the liver. It restores
enzyme functions and
promotes detoxification
of the liver. Neutral fats
and cholesterol are
transformed into
transportable forms and
led to their physiological
oxidation. Liver cell
regeneration is
stimulated and the bile
is stabilized.
Indication/contraindication
Mechanism of action Rationale for patient
Side effects / adverse effects
Nursing considerations
Generic name:LactuloseBrand name:Duphalac
Classification:Ammonia reducer
Dosage:30 ml
Frequency:Three times a day
Route:Per Orem
Date Ordered:June 16,2010
Recommended dose: 27 years old-
20 to 30 g (30 to 45 ml) P.O. t.i.d. or q.i.d.
Therapeutic level:
Prevention of hepatic encephalopathy
It makes the intestinal contents more acidic than blood. This prevent ammonia diffusion from the intestine into the blood, as occur in hepatic encephalopathy. The trapped ammonia ions and by lactulose laxative effect is expelled by the feces.
Gi: Abdominal cramping and distention, diarrhea and flatulence
Hypokalemia, hypovolemia, hypernatremia.
Monitor blood ammonia level, electrolytes level and signs of dehydration
Dilute the medication with water or take it with food
Inform patient to report any adverse reaction of the medication to the physician
Indication/contraindication
Mechanism of action Rationale for patient
Side effects / adverse effects
Nursing considerations
Generic name:Potassium ChlorideBrand name:Kalium
Classification:Electrolytes
Dosage:
Frequency:For ever 6 hours
Route:
Date Ordered:June 16,2010
Recommended dose: 27 years old-
.
Therapeutic level:
Hypokalemia
Acts as the major cation in intracellular fluid, activating many enzymatic reactions that are essential for physiologic process, including nerve impulse transmission and cardiac and skeletal muscle contraction.Given to patient with drugs that causes hypokalemia such as lactulose
Acute dehydration
Addison’s disease
Hypersensitivityto potassium salt
Renal impairment
Arrhythmias
Hyperkalemia
Rash Ulceration Nasea and
vomiting
Administer oral potassium with or immediately after meals
Mix potassium chloride for oral solution in cold water, orange juice and stir it for 1 minute before administering
Regularly assess for signs of hypokalemia
Monitor for the serum creatinine, and urinary outputHealth education: teach ways on how to take potassium medication and take drugs after
meals
