1.0 INTRODUCTION

In August 2008, the United States Army Corps of Engineers (USACE) Omaha District issued a modification to Task Order #0001 under Shaw Environmental, Inc.’s (Shaw’s) Louisville Multiple Award Remediation Contract (LMARC) (W912QR-04-D-0027) for the handling and disposal of 139 igniter tubes discovered at RVAAP 08 (Load Line 1) at the Ravenna Army Ammunition Plant (RVAAP) in Ravenna, Ohio (Figure 1-1 and Figure 1-2). The igniter tubes were uncovered adjacent to the southwest corner of Building CB-13B at Load Line 1 during remediation activities associated with the remediation of soils and dry sediments at RVAAP 08-11 (Load Lines 1-4) performed at the RVAAP between 2007 and 2008 by Shaw under its Omaha Fixed-Price Remediation with Insurance (FPRI) contract (DACA45-03-R-0026). In addition, Shaw was also asked to perform a brief search of available records to determine if the igniter tubes could be further identified to allow for an evaluation of it potential contents. This Site-Specific Work Plan was developed to support the off-site disposal of igniter tubes encountered as part of remediation activities under the FPRI and was prepared in accordance with Shaw’s Project Coordination Plan (PCP) (Shaw 2006a), Quality Assurance Management Plan (QAMP) Addendum (Shaw 2006b), Safety Health and Emergency Response Plan (SHERP) Addendum (Shaw 2006c) prepared for Shaw’s FPRI task order. This document summarizes the discovery of the igniter tubes and provides procedures and controls necessary for the packaging, handling, and offsite disposal of the 139 igniter tubes.

2.0 IGNITER TUBE DISCOVERY AND DESCRIPTION

2.1 IGNITER TUBE DISCOVERY

The igniter tubes were uncovered adjacent to the southwest corner of Building CB-13B at Load Line 1 during remediation activities associated with the remediation of soils and dry Sediments at RVAAP 08-11 (Load Lines 1-4) performed at the RVAAP between 2007 and 2008 by Shaw under its Omaha FPRI contract. After informing both the Army and the RVAAP facility manager of the discovery, the igniter tubes were segregated from contaminated soils by trained Shaw Munitions and Explosives of Concern (MEC) specialists. Based on a field inspection of the igniter tubes by Shaw’s MEC personnel and RVAAP installation personnel, the igniter tubes were not considered an explosive concern. The igniter tubes appeared to be a subpart of a larger firing mechanism. Figure 2-1 provides a photograph of one of the igniter tubes encountered at the site.

The igniter tubes were subsequently placed into two 5-gallon pails with secure covers. The 5-gallon pails were transferred and placed into a secured bunker, Magazine 7C4 (Figure 1-2) at the

2.2 IGNITER TUBE DESCRIPTION

As shown in Figure 2-1, the igniter tubes are composed of metal, are tubular in shape and are approximately 4-5 inches in length. The diameter of the igniter tubes are approximately ¾ of an inch. The tube portion of the piece is perforated on two sides and each end of the tube is capped. The majority of the tubes encountered also have a clip attached to it at about the three quarter point along the length of the tube. A visual inspection of the igniter tube was not able to definitively determine the contents or if any residual propellant material was present. A part number was also not evident on any of the igniter tubes inspected.

2.3 TECHNICAL LITERATURE REVIEW

As required, Shaw MEC and technical personnel performed a technical literature search of available documents in an attempt to more accurately determine the type and composition of the igniter tubes discovered. After reviewing numerous unexploded ordnance (UXO) and MEC books and catalogues, Shaw was unable to find a complete match to the materials encountered. The igniter tubes appear to be part of a larger firing assembly; however, with no part number, the actual composition and content could not be positively identified. The igniter tubes are suspected to contain a powder type propellant and are not considered to represent an explosive hazard.

3.0 PACKAGING, HANDLING AND SHIPPING OF WASTE

The objective of this scope of work is to safely and legally package 139 igniter tubes and transport for off-site disposal. The following section will present the procedures associated with the safe handling and packaging of the waste prior to and during off-site disposal activities. Tasks to support this objective include the following:

• Provide properly trained MEC personnel and awareness training for all personnel handling the igniter tubes

• Package the igniter tubes in 5-gallon pails containing the igniter tubes with water to ensure the tubes are properly inert prior to transport

• Characterize and profile the igniter tubes for offsite disposal

• Transfer the 5-gallon pails containing the inert igniter tubes into the approved vendor’s transport vehicle for approved off-site disposal

Per USACE agreement prior to preparation of this site-specific work plan, an Explosive Safety Submission (ESS) work plan will not be required for the performance of this work.

3.1 PERSONNEL AND SAFETY AND AWARENESS TRAINING

3.1.1 MEC Personnel

Personnel and work standards will be in compliance with Department of Defense Explosive Safety Board (DDESB) TP-18 “Minimum Qualifications for UXO Technicians and Personnel”. For this project, the Shaw will utilize a trained MEC Supervisor (MEC Tech II) to oversee packaging of the igniter tubes and ensure the containers are properly handled and loaded onto the contracted disposal vendor’s vehicle. Names and data base numbers of these qualified personnel will be provided prior to mobilization.

The Shaw MEC Supervisor is responsible for ensuring that personnel performing MEC tasks during this project have the required qualifications. The MEC Supervisor oversees and coordinates all MEC work activities. The MEC Supervisor is the final authority for Shaw’s entire MEC field activities performed in support of this project by Shaw personnel or its subcontractors. The MEC Supervisor will document all MEC activities in his log book and on the Field Activity Daily Log (FADL) form included in Appendix A.

3.1.2 Tailgate Safety Meetings

The Shaw MEC Supervisor will conduct a tailgate safety meeting for all personnel prior to commencing packaging and transfer of the igniter tubes. The topics discussed at the tailgate safety meeting will include health and safety considerations for the day’s activities and necessary personal protective equipment (PPE). Attendance records and meeting notes will be maintained with the project files.

3.1.3 Health and Safety

3.1.3.1 Safety Health and Emergency Response Plan Addendum

Shaw will prepare a brief addendum to its SHERP (Shaw 2004) to address the handling and offsite disposal of the igniter tubes prior to the commencement of on site activities. The document will be an addendum to Shaw’s existing SHERP for work performed at Load Lines 1-4.

3.1.3.2 Execution of Work

The Shaw personnel performing work at RVAAP will follow the approved SHERP (Shaw 2004) the associated SHERP Addendum (Shaw 2006c), and the above referenced task specific addendum documents. The anticipated level of PPE is Level D with the following exceptions:

• Hard hats will not be worn unless an overhead hazard exists, (such as when using earthmoving machinery)

• Non-steeled toed protective boots will be worn instead of steel-toed boots

Both of these modifications comply with the requirements of the U.S. Army Engineering and Support Center Basic Safety Concepts and Considerations for MEC Response Action Operations, EP 385-1-95a, included in Appendix B. If the hard hat must be worn by personnel, it should be securely attached to the wearer; otherwise, PPE worn by personnel will be in accordance with the USACE Safety and Health Requirements Manual, EM 385-1-1 (USACE 2003).

3.1.3.3 General Safety Concerns and Procedures

General safety concerns will be addressed by implementing the following procedures before and during MEC operations:

• Only qualified personnel will perform MEC procedures

• Personnel who will be handling MEC items will not wear inner or outer garments having static electricity-generating characteristics nor carry fire or spark producing devices into the site

• All personnel have the right and duty to stop work when conditions are unsafe, and to assist in correcting these conditions

3.1.3.4 Visitors

Authorized visitors will be briefed on the hazards present at that location by the Shaw MEC Supervisor. Visitors will be responsible for compliance with the requirements specified in this section, the SHERP (Shaw 2004), the SHERP Addendum (Shaw 2006c), and the site specific addendum referenced above.

3.2 PROFILING AND PACKAGING OF WASTE

Since the igniter tubes could not be positively identified during the literature search, the material will be packaged to ensure it remains inert during the handling and off-site disposal activities. To ensure the material will remain inert, the existing buckets in which the igniter tubes are currently packaged will be filled with water to create a fluid, non sparking environment around the igniter tubes. The buckets will subsequently be sealed to prevent leaking and properly labeled for disposal in accordance with applicable federal, state and local regulations.

Prior to packaging the waste, Shaw will prepare and submit required waste profiling documents. Once completed, the draft profiling documents will be submitted to the Army for review, approval and signature. The completed waste profiling documents will be forwarded to the identified disposal facility for acceptance. The contents of the containers will be identified so as consistent with the profiling and acceptance criteria received from the accepting disposal facility. It is expected this material will not carry an explosives waste coding.

3.3 OFFSITE DISPOSAL

3.3.1 Disposal Documentation and Notifications

Prior to offsite disposal, Shaw will prepare the proper offsite disposal and transportation documents for RVAAP review and signature as Generator. Shaw’s onsite coordinator will meet with the RVAAP Installation Manager to go over the documents prior to signature to ensure there are no other issues related to disposal.

Shaw will coordinate with RVAAP to schedule a date/time to dispose of collected propellant and will make notification to the Ohio Environmental Protection Agency (Ohio EPA) prior to off-site disposal in accordance with the MEC Notification Procedures in Appendix C.

3.3.2 Material Loading, Shipment, and Disposal

Once the approval to accept the waste has been received from the accepting facility, the material will be shipped off site for disposal at the accepting facility. Shaw has identified Heritage Environmental Services in East Liverpool, OH as the designated facility to accept the igniter tubes. The facility is properly licensed and permitted to accept and handle the waste. Once at the facility, the material will be destroyed via incineration. The facility will provide proof of destruction and proper disposal of the waste for the project file.

During loading operations, a minimum safe separation distance of 200 feet will be established as an exclusion zone (EZ) to protect individual operating units in the event of an accidental detonation while packaging and transfer operations are underway. The minimum of 200 feet is per the EM 1110-1-4009 Engineering and Design Ordnance and Explosives Response, June 23, 2000, USACE, and Memorandum on the Determination of Appropriate Safety Distances on Ordnance and Explosives Project Sites. The 200-foot minimum distance will be used since the probability of encountering MEC in this area is low. In the unlikely event that an MEC item is discovered that has a larger Minimum Separation Distance (MSD) then the EZ safe separation

distance will be re-evaluated and re-established based on the Material Potentially Presenting an Explosive Hazard (MPPEH) found or suspected. Essential personnel are generally considered the Shaw MEC supervisor and the disposal contractor personnel.

Shaw on site personnel will maintain the established EZ perimeter to preclude non-essential personnel from entering the EZ. Vehicle entrances and roadways to the magazine EZ will be blocked to protect errant or non-authorized vehicles from entering the work area during the MEC removal activities.

All work will be performed in accordance with established RVAAP site plans and requirements.

Munitions and Explosives of Concern (MEC) at the Ravenna Army Ammunition Plant (RVAAP) - Notification Procedures

Paragraph 9(a) of the Director’s Final Findings and Orders (journalized June 10, 2004) allows for the following exemption: “The requirement to obtain a hazardous waste facility installation and operation permit, as required by ORC 3734.02 (E), for the storage and treatment (destruction) of MEC (excluding bulk storage of munitions and chemical and biological warfare materiel) at OD#2, and for the in-place treatment (destruction) of MEC (excluding bulk storage of munitions and chemical and biological warfare materiel) discovered at the RVAAP that can not be safely transported to OD#2, provided, however, that Respondent shall comply with all applicable requirements of ORC chapter 3734 and OAC chapters 3745-50 through 3745-68, including but not limited to the hazardous waste requirements set forth at Appendix E.” In the absence of obtaining emergency permits, the following is the type of information that should be provided to Ohio EPA Northeast District Office (NEDO), Division of Emergency and Remedial Response (DERR) [attn: Eileen Mohr] and Division of Hazardous Waste Management (DHWM)[attn: Greg Orr]. The information is divided into categories: prior to destruction of the MEC and subsequent to the destruction of the MEC. Prior to the MEC destruction, notification can be made via either letter or email. Subsequent to detonation, the information can be transmitted in a written summary report after each detonation event or at the conclusion of clearance activities at a particular Area of Concern (AOC).] Information to be provided prior to Blow in Place (BIP) or Detonation at OD#2: 1. Point of Contact (POC) 2. POC’s phone number(s) and fax number 3. Location/date/time/person discovering the MEC 4. Description of MEC to be blown: including type and quantity 5. Proposed destruction location: either at OD#2 or BIP 6. Proposed method of destruction 7. Proposed methods to mitigate/abate potential contamination 8. Preparedness and prevention 9. Notifications to be made Information to be provided subsequent to BIP or Detonation at OD#2: 1. POC 2. POC’s phone number(s) and fax number 3. Description of MEC blown: including type and quantity 4.
