MANAGEMENT EXPERIENCE

I’m the current ENT Department LRI lead clinician for Infection Prevention IP and the lead clinician for department Coding. I’m a current attendance to the ENT department Consultant meeting and M&M department meeting in a monthly base. The meetings involve all aspects of the department and hospital management for financial, academic, teaching, patient safety, training and development.

PROCEDURES & JOB DETAILS

Surgical, Clinical & Teaching Experience at LRI, Consultant ENT Surgeon post since 5 th

January 2004

17 The job consisted of five clinical sessions, three operating sessions, one administration session and one teaching session. The on-call rota is one week in seven.

The post gave me the chance to teach medical students the introductory course once a week for the first three years. I am currently an examiner of middle and final year medical students, and teach Semester 4 SSC in the medical school at Leicester University.

I trained a number of SpRs and two Staff Grade colleagues, who are now consultants or on their way to applying for consultant posts.

Noses: My main interest is post-traumatic deformed and dysfunctional noses and revision rhinoseptoplasty. In the course of the job, I did 480 aesthetic and functional rhinoplasty +/- septoplasty with or without augmented grafts; the cartilage grafts were autogenous and harvested from the septum, the conchae of the ears and the ribs.

 Ears: I performed around 143 cases of major ear surgery including one stage mastoidectomy, reconstruction of the mastoid cavity, ear canal and ossiculoplasty. I am pleased with the per meatal per hole technique I used for 141 of myringoplasty as it left no visible scars. The results of my 44 stapedotomy cases matched the RCS standard requirements which are deemed necessary for continuing to practise the procedure.

Sinuses: A 149 cases of FESS for CRSwNP & CRSsNP, and 43 endoscopic endonasal polypectomies.

Head & Neck: 15 parotidectomies, 13 submandibular gland resections, removal of 5 branchial and 3 thyroglossal cysts (using Sistrunck technique). I currently operate on all benign vocal cord lesions microlaryngoscopically with the help of a laser and a microdebrider in accordance with the nature of the lesion. I currently perform bronchoscopy for removal of aspirated F.B in children as part of my on-call duties.

 Skull-Base Surgery: I successfully repaired spontaneous CSF rhinorrhoea endoscopically on a middle-aged lady and a post-traumatic young boy. I endoscopically drilled the floor of the frontal sinus on two ladies and a gentleman (Modified Lothrop/ Draff 2&3).

I currently perform endoscopic ligation of the sphenopalatine artery whenever it is indicated, and manage orbital sepsis in line with the stage of the disease (Chandler Classification). I removed large extended left ethmoidomaxillary sinuses with inverted papilloma with infratemporal fossa extension from a gentleman in his late 50s using the degloving midfacial approach with the assistance of a microscope and electric drill. The follow-ups over 3 years proved no recurrence.

Paediatric ENT:      I currently have one paediatric ENT clinical session a week and have paediatric surgical cases on my operating lists. My clinical judgement and cochlear implant referral are based on NICE and SIGN guidelines. I follow the National Hearing Screening Program (NHSP) policy. I removed a deeply extended and frequently infected nasal dermoid cyst on a 2 year-old child. The operation required an open rhinoplasty approach and microscopic dissection of the skull base extension; the follow-ups over six years proved no recurrence. I give post-adenotonsillectomy bleeding the highest priority and manage all cases appropriately.

I am involved in medico-legal reporting and presently report on an average of 30 cases per month.

18Surgical, Clinical &Teaching Experience on Syrian Professor Post, Oct 95-Dec 03 & Aug11

Teaching: the post involved teaching under- and postgraduate trainees clinical and surgical skills. It involved lecturing at medical school and the Al Moasat University Hospital. It also involved regular on-the-job training in surgical and clinical skills of postgraduates during my outpatient and theatre sessions. I am a member of faculty of the Damascus Medical School and am a current examiner of undergraduate students in year four and in their final ENT viva exams. I am an examiner and research body sponsor of the ENT Postgraduate Master Degree thesis and final award.

Head & Neck: I operated on more than 90 patients with different stages of H&N cancer; for each one, I performed either comprehensive or non- comprehensive neck dissection surgery. 56 patients underwent total & partial laryngectomies, and 31 underwent parotidectomy.  There were also a number of cases of thyroglossal cysts and branchial cysts. I used local and pedicle vascular flaps for reconstruction. I’m well experienced in taking the pectorals major myocutanous flap. I operated on an infant with a congenital cystic hygroma and severe upper airway embarrassment.

Ear Surgery: I performed 88 mastoidectomies, 109 tympanoplasties, 32 stapedotomies, and 56 reconstructions of middle ear ossicles.  

Skull Base Surgery: I established a skull base surgery unit with the help and support of my neurosurgeon colleagues in 1997. Our prime surgery was lateral infratemporal fossa (Fish approach), in which I operated on advanced glomus jugularis tumours. All operations were video recorded and two postgraduate trainees were awarded MSc degrees for their theses about 12 recorded cases. I successfully removed recurrent and radiotherapy resistant juvenile angiofibroma on two young males. The procedures took place using the degloving mid-facial approach.

In the period between 2011 and 2013, I focused on endoscopic endonasal ventral skull surgery, At the Alasad University Hospital, Neurosurgery Department, I dealt with five cases of revision hypophysectomy, one CSF leak repair and one young patient with extensive craniopharyngeoma.

I operated on three ladies with extensive glomus jugularis using the lateral approach at the same department.

I successfully operated on two new-born babies with unusual middle skull base obstructed teratoma who were referred from the paediatric university hospital and who underwent complete microscopic resection.

Rhinoplasty and Endoscopic Sinus Surgery: I operated on 192 patients at the university hospital and I did more than double this number of patients in the private sector. I did pan and functional endoscopic sinus surgery on more than 148 patients. I presented the results of my 28 cases of endoscopic repair of CSF rhinorrhoea at the Franco-Arab Congress of ENT Head & Neck Surgery 12 to 14 Nov 1999 with no clinical evidence of recurrence in 27 out of 28 over a period of 6 to 12 month post-operative follow-up.  

19REWARDING ACTIVITIES

1. On air Radio Monticarlo questioning about facial plastics from 5pm until 6pm on UAE time on Thursday the 27th April 2017.

2. Over a period of 13 years, I satisfactorily carried out more than 480 cases of functional and aesthetic rhinoseptoplasty at LRI hospital.

3. At the FRCS ceremony at RCS London in 1995, I was acknowledged as one of the top three candidates across the country in the DLO, RCS final exam. At that time the DLO, RCS was the only ENT degree in UK sponsored by RCS of England. Applicants had to complete three years’ training as a registrar or two years as a senior registrar before being eligible to apply.

4. I was a regular consultant on Syrian National and International Satellite Broadcasting TV Channels and other media including newspapers and radio stations. I provided advice to the public exclusively on ENT and other medical and educational matters.
