1 Dr. R Raw selected professional statistics and facts 2004 – 2016 (7 th September 2016) (Data mostly extracted from Dr. Raw’s institutional professional CV) He visited the Department of Anesthesia at the University of Iowa, USA, from South Africa as an invited Visiting Professor in 2003. The then Departmental Chief Dr. DL Brown offered him a job because of Dr. Raw’s special knowledge and experience in regional anesthesia. Dr. Raw took up the job in August 2004, and worked then Departmental Chief Dr. Michael Todd. Dr. Raw was awarded a special medical license (SP0163) to practice medicine by the Iowa Board of Medicine. Dr. Raw in the twelve years since appointment to September 2016 has performed the following professional work. PROFESSIONAL TEACHING; He made presentations for the department’s education of anesthesia trainees. He also made presentations at the department’s series of CME meetings held within the state and internationally in Mexico. His number of presentations exceeded that of all other departmental faculty members in this period, totaling the number of 481 lectures, workshops or PBLDs. National invited presentations made at meetings not associated with the university numbered 23 lectures, workshops or PBLDs. International invited presentations took him on 23 trips to 8 countries. The countries were Italy [2x], Sri Lanka [2 x], Great Britain, Kenya, Mexico [3 x], South Africa [9x], Jordan [2x], and the United Arab Emirates [3 x]. He made a total of 95 presentations. PROFESSIONAL PRODUCTIVITY He obtained publication of 14 articles, book chapters, and letters some of which he was the first author. At national and international conferences he presented 3 educational videos and 4 posters. On the internet he posted 12 advanced edited teaching videos which currently have about 400 000 viewings from around the world. The YouTube site is called vidRASCI. PROFESSIONAL SERVICE; He was primarily a clinical anesthesiologist working in all fields except pediatric surgery on patients under five years old, and on open heart surgery patients. His most prominent area of work was in orthopedic surgery. All his regular reviews consistently used the written phrase to describe him as being “a master clinician”. In the department of anesthesia he served during parts of his employment time on the following committees, or was responsible as; o Director of the regional anesthesia fellowship. o Director of orthopedic anesthesia. o Resident selection group. This involved interviewing applicants and also selecting new residents. o Member of the Departmental Review Group (DRG) reviewing and voting on the promotion of other faculty members. o Anesthesia Work Room Committee, reviewing potential new equipment and advising on equipment changes and purchases. o Director of the Regional Anesthesia Study Center of Iowa (RASCI).

2 In between 2007 when Dr. Raw took over this responsibility, and 2015 when the

series stopped he had convened 40 meetings. Most were two day meetings. One was for veterinary regional anesthesiologists and two were for pediatric regional anesthesiologists.

Doctor Raw converted and re-conceptualized this meeting from being its former trade financed marketing tool for Arrow company nerve catheters using old land-mark and electrostimulation techniques, to a modern self-financing meeting teaching ultrasound guided peripheral nerve blocks.

This was the most financially successful anesthesia department CME meeting ever. This meeting was in the top three most financially successful meeting of this medical school exceeded only some years by one or two meetings enjoying big trade sponsorships.

The workshop included the longest running anesthetized-pig workshop in the world for delegates to get hands-on experience performing live nerve blocks on this bio-responsive training model.

The workshop included the world’s only regular live teaching demonstrations with needles inserted into volunteers.

The workshop was the world’s most advanced workshop in its use of audiovisual aids. The ultrasound image was projected onto a large screen allowing for large groups of delegates to get good views of the demonstration. Simultaneously a close-up view of the teacher’s hands performing the nerve block was filmed from a remote controlled direct overhead HD camera and projected onto a second large screen. These videos were all recorded and preserved for later use as teaching videos, on the website called vidRASCI. In addition a 4K front view camera recorded the overall frontal view as seen by delegates for use in producing the teaching videos.

The live human-volunteer-with-needles-insertion portion of the workshop has been taken internationally to Sri Lanka, Jordan and South Africa.

Approximately 450 nerve blocks were demonstrated on 120 volunteers, at this institution, with no complications and no lasting symptoms.

Dr. Raw introduced ultrasound guided regional anesthesia to the University of Iowa. He personally bought the first ultrasound machine for the anesthesia department’s use for this purpose. He helped pioneer the techniques of ultrasound guided regional anesthesia.

The list of nerve blocks that Dr. Raw most commonly practiced and taught clinically follows. Dr. Raw practiced all of (i) electrostimulation needle guided techniques, (2) land-mark needle directed techniques, and (3) ultrasound image needle guided nerve blocks. For most of those he taught both single shot techniques and perineural catheter placement techniques. The common nerve block list includes the following; superficial cervical nerve plexus block, cervical paravertebral plexus block (per Boezaart), Winnie interscalene brachial plexus block, ultrasound guided posterior approach interscalene block, supraclavicular plexus nerve block, suprascapular nerve block, axillary plexus nerve block, pectoral nerve block, intercostobrachial nerve block (proximal ultrasound guided, or distal land mark guided), elbow block, wrist block, Bier’s block, femoral nerve block, obturator nerve block, lateral cutaneous nerve of thigh block, adductor canal block, distal trans-sartorius saphenous nerve block, sciatic nerve (in popliteal fossa, trans-gluteal on GT-IT line, parasacral, mid-

3 thigh lateral/anterior/medial), ankle block, psoas compartment block, epidural block (caudal, lumbar, thoracic), spinal block), rectus sheath block, and TAP block. Records show he performed in a range of 30 to 50 nerve blocks (excluding counting neuraxial blocks) per month. Records show he performed 160 nerve blocks in one exceptional “maximum” month.

For the Medical School of the University of Iowa he served on the following university committees at different times;

o The Conflict of Interest Committee. He advised on the ethics of the financial relationships between trade companies and educational faculty who lectured. Teaching materials were reviewed for potential conflicts of interest.

o The Continuing Education Committee (CME). He advised on the awarding of CME credits to educational meeting applications.

o The Executive Committee. He reviewed applications and voted on all professional medical faculty appointments and promotions.
