Clinical Innovations LADDERS (Learning and Developmental Disabilities Evaluation and Rehabilitation Services) began in l981 at the Youville Hospital in Cambridge, Massachusetts as a small multidisciplinary clinic for children with developmental disabilities, primarily learning disabilities. However, as the basic science research of autism began to become more widely publicized (primarily resulting from the publication of our brain anatomy paper published in 1985 demonstrating that autism was a neurodevelopmental disorder of the brain and was therefore not caused by bad parenting), the patient population rapidly shifted toward those with autism and related disorders. The program rapidly grew and in l995 moved to larger quarters under Braintree Hospital and then in l997, came under the administration of the Massachusetts General Hospital for Children where it remains to the present time, having since been renamed the Lurie Center. The program now serves children, adolescents and adults with a variety of developmental disabilities with the large majority of the patient population carrying a diagnosis on the Autism Spectrum Disorder (ASD). The program is not only a diagnostic clinic but continues to provide treatment, advocacy and support services throughout the life span. Services include Neurology. Psychiatry, Gastroenterology, Psychology, social work, speech and language, occupational and physical therapy, special education, and family resource assistance. Currently, the program serves children, adolescents and adults primarily from throughout the New England area, eastern New York and northern New Jersey with many additional patients coming from Canada, Europe, South and Central America and China.

In 2003, The LADDERS model was adopted by the Northwest Autism Foundation in Portland, Oregon and subsequently became the Autism Treatment Network (ATN) which included a total of five academically based sites throughout the United States. Since its beginning, the ATN has rapidly expanded and now includes 17 multidisciplinary clinical sites throughout the United States and Canada supported by Autism Speaks. The eventual goal is to further expand the number of similar sites throughout the United States so that ASD individuals will have ready access to comprehensive medical and therapeutic care throughout their lifetime and close to where they live. Through the availability of this interdisciplinary model of care, the LADDERS (Lurie Center) has been able to draw attention to the fact that many ASD individuals have associated medical disorders that often have an atypical clinical presentation and which therefore can be missed and go untreated, especially in those patients who are non-verbal. Through the LADDERS model, it has become apparent that when these disorders are identified and treated, behaviors improve and patients are more able to participate in and benefit from services provided, thus improving quality of life for the patient and his/her family.

Thus the development of the LADDERS model has significantly and positively impacted

on how ASD individuals receive health care and other associated services, both locally, nationally and internationally and has made a substantial impact on the quality of life and developmental outcomes for those affected with this disorder.

Report of Technological and Other Scientific Innovations

With the initial description of the microscopic anatomy in the brain of an autistic adult in 1985, it

became apparent that the use of whole brain serial section (creating well over 4000 sections of an entire brain) could be used to study what would otherwise be subtle changes in brain development and histopathology when utilized in comparison with identically processed age and sex matched control brains using a comparison microscope, whereby identical sections from each brain could be studied side by side in the same filed of view. Using this approach, a number studies have since been conducted and published in other disorders such as Rett syndrome, disorders in which the histopathological abnormalities might have been missed without a direct comparison with normal brain tissue.

More recently, we have developed new techniques for stereological computerized cell counting in

our laboratory, which has been become more efficient and accurate as we continue to study our findings in the autistic cerebellum. A paper describing these innovative approaches has been published in 2014.

Narrative Report

• As a pediatric neurologist and research investigator who has been a pioneer in the study and treatment of autism for the past twenty- five years, I am dedicated to clinical and research advances in this field. This dedication is exemplified in my establishment and development of The Autism Research Foundation (TARF), The Autism Research Consortium (TARC), LADDERS (Learning and Developmental Disabilities and Rehabilitation Services) now known as the Lurie Center, and the Autism Treatment Network (ATN). In 1984, my research partner and I opened the door to future scientific study when we were the first investigators to identify neuro-anatomical abnormalities in the limbic system and cerebellum of people with autism. Spurred by these first scientific observations that confirmed the biologic basis for autism, I founded TARF in 1990. TARF is devoted to the study of the neuro-pathological basis for autism and has been the global leader in such research. TARF researchers are widely published and disseminate their knowledge pertaining to the pathology of the brain in autism, behavioral disorders, and the relationship between autism and memory to colleagues and the general public via their writings and lectures throughout the world. In 1996, I founded The Autism Research Consortium. I invited a small group of elite investigators – some who were directly involved in autism research, others who worked in related fields – to come together to form a “think tank” to determine the most fruitful course for future studies. TARC is a venue that provides a multidisciplinary, integrated approach to optimize the scientific use of the valuable and limited brain tissue available for the study of autism.

My research work has been is mirrored in the clinical care setting I have developed at the LADDERS Program, now known as the Lurie Center, which I founded in 1981. This program provides comprehensive diagnostic and therapeutic services for children, adolescents, and adults with autism and associated neurological disorders. I have recruited top practitioners in the field to provide a true team approach to the cohesive evaluation and treatment of each patient. I have established a comprehensive evaluation and treatment protocol which includes major provisions for a follow-up plan based on the individual’s need for medical, educational, and rehabilitative services. Currently, this program will render medical and therapeutic intervention through approximately 10,000 patient visits. My mission since 2005 has been to establish centers nationwide (The Autism Treatment Network) based on this model of care which will create a “gold standard” of medical treatment that can be made broadly available to physicians, researchers, parents, policy makers and others dedicated to enhancing the care of individuals with autism spectrum disorders (ASD). I have combined the knowledge gained from my progressive research with my expertise in the diagnosis and treatment of autism and associated neurological disorders to the betterment of those living with these disabilities and will work to continue this mission.
