December 13, 2017 Project No: 170081 Malon Haris Dove Academy of Detroit 20001 Wexford Street Detroit, Michigan 48234 Re: AHERA Periodic Surveillance Report Dear Mrs. Haris: The following AHERA Periodic Surveillance Report was completed for Dove Academy of Detroit which is located at 20001 Wexford Street, Detroit, Michigan 48234. Re-Inspection activities were limited to the known or assumed asbestos containing materials identified in the most recent three-year re-inspection report. During the 12/01/2017 periodic surveillance, NAS found the following conditions to be true of the ACM materials identified in the most recent 3 year report: 1950 Building All materials were found to be in good condition, maintain in accordance with the Management Plan and test any assumed material prior to disturbance. 1963 Building All materials were found to be in good condition, maintain in accordance with the Management Plan and test any assumed material prior to disturbance. A copy of the AHERA Periodic Surveillance Form (Form F-4) and a copy of our material check list is attached to this report.
