We follow the Dr. Gemma Jones model of care giving which concentrates on four different behavioural stages in dementia.

independent, or about your care options, you should
phone your relevant Adult Social Care department to
request an assessment. Contact details can be found
below.
Anyone can ask for an assessment, regardless of their
fi nancial situation. The assessment and advice is free. You
may be asked to make a fi nancial contribution towards
services arranged for you, which could be the full cost of
the service for people who can aff ord to pay.

Directory, which covers a large area, three councils are
responsible for community care services:
• Dorset County Council (Adult and Community Services),
• Borough of Poole (Adult Social Care), and
• Bournemouth Borough Council (Adult Social Care).
Throughout the text, for ease of use, the services are
described as coming from ‘Adult Social Care’.
Adult Social Care (ASC) assesses need and provides
information, advice and services to the people of Dorset,
Poole and Bournemouth, 24 hours a day, 365 days a year.
ASC supports adults of working age and older people who
have physical disabilities, learning disabilities, mental health
issues, a sensory loss or general frailty. The aim is to actively
promote independence and choice and help people to
be self-reliant and live in their own homes for as long as
possible. When this is no longer possible they help people
to choose good quality supported housing or care homes.
ASC will always provide support and advice, even if they
cannot pay for the help you need. They will do their best
to help get you back to living independently.
One in ten people in the UK care for friends or family
members. ASC supports unpaid carers in various ways, for
example by providing training, links to support groups
and assisting carers to get a break from their caring
responsibilities.
This Directory contains information about the services
available to people in Dorset, Poole and Bournemouth.

Your assessment
• a social worker or health professional such as a doctor or
health visitor; or
• any other agency.
If you wish to request an assessment or need more
information, telephone your local Adult Social Care office
(contact details are on page 4). They will arrange for an
interpreter if you do not speak English or if you use sign
language.
What does the process involve?
The assessment of your needs will involve your social
worker talking with you, your carer (if you wish) and other
people who know you well, about different aspects of
your life. These discussions may include your physical
health, your emotional health, how you manage to look
after yourself, what outcomes you are looking for from
the care and support provided and your current family
and support networks. Services may be offered to meet
needs that pose a ‘critical’ or ‘substantial’ risk to your
independence, in line with the Government’s ‘Fair Access
to Care Services’ guidelines. Methods of best meeting your
needs and achieving your outcomes will be discussed and
agreed with you.
Do I have a say in what services I get?
After your needs have been assessed, Adult Social Care
will advise you about services you may be eligible for and/
or a Personal Budget (explained on page 8) to help you
meet your care needs. They will discuss with you:
• the type of help you think you need to meet your
outcomes;
• when you think it is needed;
• who will be providing the assistance;
• how long they will be with you;
• how it will be paid for; and
• your religious and cultural needs.
Support plans are reviewed regularly and will include all
references to any Personal Budget you may be eligible for,
to help pay for your care needs.
Before they can arrange care services for you, Adult Social
Care must assess your needs. Social workers, housing staff
and health professionals may work with you and your
carer to identify what your needs are and how they might
best be met.
The assessment is based on your circumstances and
will be different for each person. A social worker/care
manager will look at ways to best meet your agreed
outcomes and goals.
The best way of meeting your goals will then be
discussed with you, sometimes with an occupational
therapist’s help. If you already have a relative or friend
who is a carer, they can also be assessed if they wish.
Once your needs have been assessed and your desired
outcomes established, Adult Social Care will decide if
you are eligible for their help. To do this, they will use a
framework set by the Department of Health. Your ‘risks
to independence’ and needs will be assessed in four
eligibility bands ranging from ‘low’ to ‘critical’.
A financial assessment will also take place, to determine
how much you can afford to contribute towards the
cost of your care. For details of financial assessments and
contributions, see your Council’s separate factsheets. ‘Paying
for Services in a Care Home’ is available if you live in Dorset.
If you live in Bournemouth the leaflets ‘Paying for Care’ and
‘Choosing and Paying for Care in a Residential or Nursing
Home’ will tell you about charges. Further details are in
Poole’s ‘Charging for Residential Care’ leaflet ‘Contributions
towards Personal Budgets (non-residential social care
services)’ and ‘Paying for a Care Home Placement’ booklet.
Am I eligible for an assessment?
To be eligible for an assessment, you may have one or
more of the following:
• sight or hearing difficulties;
• mental health needs;
• frailty because of old age;
• ongoing care needs when you leave hospital;
• learning disabilities;
• permanent, substantial physical disabilities;
• a long term illness; or
• problems related to substance misuse.
Alternatively, you may provide regular unpaid care for
someone and subsequently be eligible for a carer’s
assessment.
Who can refer you for an assessment?
You can be referred by:
• yourself or a relative, friend or neighbour;

the way communities, organisations and individuals work
to support people. It recognises that people want better
quality services that are personal to them, and more
control over decisions that aff ect them.
In the traditional system of social care, professionals made
decisions about the support people could have and who
would provide it. As a result, people often had little or
no choice about the support they received. The central
government directive ‘Putting People First’ has changed

Personalising Adult Social Care
A Personal Budget is an amount of money to pay for your
social care support if you are recognised as needing long-
term support from Adult Social Care after an assessment
of your situation. It allows you to have more choice and
control over your life because it lets you make decisions
about the support that you need. It asks how you want
to live your life, what you want to achieve and the type of
support you need to do this.
There’s also a fi nancial assessment to work out how
much you need to pay towards your Personal Budget.
Adult Social Care also checks that you are getting the
benefi ts you are entitled to.
A Personal Budget, for example, can be used to pay for
Direct Payments are fi nancial payments made to you, if
you have been assessed as eligible to receive support
from Adult Social Care. Instead of councils arranging or
providing services directly, you can use this money to
purchase support that you consider most appropriate to
meet your assessed social care needs.
Direct Payments give you more fl exibility and choice,
by enabling you to buy support for yourself that better
suits your individual needs.
If you receive a Direct Payment, you are in control -
you can decide how your support needs will be met, by
whom and at what time.
You can employ your own support staff , which means
they will report directly to you. Or, you can have a contract
with a local agency, in which case the agency will be
accountable to you, not to Adult Social Care.

The amount should be enough to meet your assessed
needs. The assessment will look at things like how much
help is needed to carry out practical tasks for daily living.
This money can be managed by you or Adult Social
Care and is not classed as income - it’s paid to meet your
needs, and having a Personal Budget will not aff ect your
benefi ts.
A Personal Budget can also be managed by someone
you nominate to act on your behalf or you can ask the
council to look after your budget and use it to buy and
arrange support. You can also choose to use a mixture of
these methods.
If you are a carer you may also be eligible to receive
Direct Payments to help you in your caring role or to have
some time for yourself. A Direct Payment can be used for
all or part of your care or support package.
Who can have Direct Payments?
Adult Social Care can make a Direct Payment to most
people who need support, including:
• older people who have been assessed as needing adult
social care services;
• disabled people (including people with mental health
needs) aged 16 and over, and those with short as well
as longer term needs;
• carers; and
• parents of disabled children, for children’s services.
If you are assessed as being eligible to receive support and
you want to consider having a Direct Payment, you must be:
• over 16;
• able to legally consent to receiving the payment from
Adult Social Care; and
• able to manage the money to meet your needs. This may
be on your own or with someone’s help, as long as you
are in control of how the money is spent.Jurassic Coast

• Home care - help with personal care such as washing
and dressing. See page 18 for more information.
• Supported housing or extra supported housing -
provided by your district or borough council. See page 32.
• Respite care - residential care or care provided in your
own home to give you or your carer a break. Further
details are on page 29.
To decide what sort of help you need, and what kind of
outcomes you are looking for, Adult Social Care will work
with you to assess your needs. They will then need to
agree a support plan for you. This sets out how you can
meet your needs and what help can be provided to help
you manage at home.
It is likely that Adult Social Care will ask you to make a
financial contribution towards the services you receive,
based on your ability to pay. You should be given clear
information about what you can expect to pay.
Support plans may change if your needs change,
leading to another assessment, which is often called a
‘review’. Assessments are explained on page 7.
‘Adult Social Care’ is the name given to services which aim
to help people live at home independently with a care
support package. Services might include:
• Reablement - help to build up strength, re-learn skills
and get ready to cope with day-to-day living at home
after an illness, accident or stay in hospital. See page 10.
• Personal Budgets - a flexible way of providing social
care services which enables people eligible for social
care to have money to choose and buy services, that suit
their lifestyle - see page 8 for further information.
• Direct Payments - this scheme allows you to buy in the
care you need for daily independent living - more
information is on page 8.
• Community meals - usually supplied frozen to reheat
yourself.
• Day services - available in care homes or community
centres. For more information, see page 29.
• Housing adaptations - Adult Social Care can provide
information about equipment and adaptations to make
it easier for you to continue to live in your own home –
see page 14 for more details.

What can Direct Payments be used for?
The money is for you to arrange support to meet your
eligible needs. You may choose to employ people directly,
to buy services from an independent or voluntary sector
provider or purchase equipment to help you live more
independently. Direct Payments enable you to choose
and plan your support in more flexible ways that fit in
with your life.
It is important that your plans meet your assessed
needs in the best way for you. As long as the money is
used lawfully and your eligible needs are met, there can
be a lot of flexibility about how you spend the money. For
this reason there is not a list of things the money can be
used for.
Most Direct Payments are made to meet regular
ongoing support needs. However they can also be made
as a single payment, for example, to purchase equipment
or a short respite break.
If you prefer, Adult Social Care can arrange services to
meet some of your needs directly, they can offer you a
combination of a Direct Payment and services that they
provide or buy from another provider on your behalf.

Reablement and rehabilitation
Reablement is often described as ‘helping people to do
for themselves rather than doing it to or for them’. It is
short term, intensive support that enables people to
optimise their independence.
Dorset County Council’s reablement service provides
practical and emotional assistance to people who are at
risk of not being able to remain independently in their
own home because of frailty, disability or illness. The
service is provided by the Community Support Service.
The aim of reablement is to maximise independence,
choice and quality of life, and reduce the need for support
in the future.
Borough of Poole is looking to commence a new
reablement service in the near future. This will be a
short-term service designed to help people who have
had a crisis (like a fall or spell in hospital) to regain the
confidence and skills to live independently, so they can
help themselves and continue to stay in their own home.
If you live in Bournemouth, are aged 65 or over, and
qualify for adult social care, reablement may be one of the
full assessment of the person’s needs. The social worker
will help people with information, advice and support
about appropriate community services which can include
information about care homes, home care, benefits
information and day opportunities. They will also signpost
people to other services who may be able to help.
Bournemouth residents who are being discharged from
Royal Bournemouth or Christchurch hospitals following
a stroke are supported by the Stroke Coordination Team.
They offer practical support to help people return to
living as independently as possible. This could involve
signposting to services for people who need ongoing
care, helping people to join clubs and groups or
assistance with finding work or volunteering. You can
contact them on 01202 705440 or email: stroketeam@
bournemouth.gov.uk.
Poole residents being discharged from Poole Hospital
following a stroke are also supported by a Stroke
Coordination service. This service provides information,
support and signposting to ensure that people have the
information they need to make choices which enable
them to live as independently as possible after a stroke.
The Stroke Coordination service can also refer clients to
the care management team and psychological support
services if additional care support is required, based on
identified need.
Dorset residents being discharged from Dorset County
Hospital (Dorchester), Royal Bournemouth, Christchurch
or Poole hospitals are also supported through a Stroke
Coordination service. This service provides advice and
information, support and signposting to help people to
become as independent as possible.
Adult Social Care and the NHS work very closely together
to ensure that hospital patients are discharged safely
and are able to return to home life as quickly as possible.
Social care staff are available to help with information and
advice when you are ready to leave hospital.
No matter which hospital you are in, you can
request an assessment of your needs to help you stay
independent and confident when you return home. The
assessment involves asking you about your needs and
what is required to help you live safely and independently
at home. There will also be an explanation of any charges
that may be made.
A relative, carer, or a member of the hospital staff may
also approach the hospital social care team and ask them
to carry out an assessment, but this will not go ahead
unless you agree.
If you already have a social care service in place Adult
Social Care will make an assessment of any other needs
you may have after your stay in hospital.
Carers should be offered an assessment and may also
need advice and support. The hospital social care team
is available to discuss any issues with them and also give
them details of carers’ support groups.
Adult Social Care can also introduce you to local
voluntary organisations who can help with a variety of
services such as laundry and domestic cleaning, or sitting
services.
Borough of Poole also works with people who are
being discharged from hospital who are not eligible
for financial assistance from Adult Social Care services,
sometimes known as self-funding clients.

services you are offered. Reablement can support you to
become independent after a fall or illness, or if you get
to the stage when you feel you cannot manage without
support. Staff work with you for a short time to regain
skills and confidence to achieve manageable goals. They
offer advice and information, make sure you have the
right equipment, suggest daily living aids that might help
you, and let you know about other sources of support.
The aim of rehabilitation is to maintain or increase your
independence and allow you to live in your own home for
as long as possible. You will be supported with the provision
of short-term care support and/or therapeutic intervention
for example physiotherapy or occupational therapy.
The assessment is carried out by multi-disciplinary
teams of health and in some areas, social care staff. (These
are joint teams with reablement services and social care
staff.) The intermediate care services will promote your
health and well-being and can help to prevent falls at
home. The service also aims to reduce admission to
hospital by providing a rapid response to you at home.
In addition, the service can enable you to be discharged
home from hospital earlier by providing care support and
intensive rehabilitation in your own home. The service
NHS continuing healthcare is provided solely by the
NHS if you have extensive healthcare needs. This is free
of charge, wherever it is delivered and can be provided
in any setting. In your own home the NHS funds all care
required to meet your assessed needs. In a care home, the
NHS makes a contract with the home and pays the full
fees for your accommodation as well as all your care.
You can ask for an assessment by health and social care
Occupational therapists can help you lead a more
active and independent life using a variety of methods,
including the use of adaptive equipment. They can assess
your home for hazards and point out things that might
contribute to falls. They also advise carers on techniques
for moving and handling the person they care for.
Adult Social Care may be able to help with specialists.
Visiting you at home, an occupational therapist will
be able to suggest adaptations that will improve your
mobility. Perhaps handrails would be useful? Would a
ramp up to your front door help if you are in a wheelchair?

long-term residential and nursing care.
A discharge or a care plan for any future care needs
will be discussed with you and you will be given details
on how you can receive help to set up on-going care
arrangements. The service may last for a number of weeks
and during this assessment period, intermediate care is
free of charge. When this is completed a care plan for your
future care needs will be discussed and you will be given
details on how you can receive help to set up on going
care arrangements.

If your needs change, regular reviews will be carried out
and your care and/or funding arrangements will change
to meet them. Contact the Dorset Clinical Commissioning
Group (CCG) for more information (see page 51 for
contact details of the relevant office for your region).
Major alterations such as fitting a downstairs bathroom
or widening doorways can be paid for with a Disabled
Facilities Grant (DFG).
An occupational therapist will talk you through any
new equipment you may be given, as well as help you
apply for funding for major adaptations, if you need them.
An assessment of your needs will determine whether an
occupational therapist will be the best person to help and
what charges (if any) you can expect to pay.

charge. Equipment provided can range from a Lifeline
unit and pendant only, through to a Lifeline unit and a
whole range of additional sensors. An individual’s ability
to pay the monitoring charge (£4.15 per week) will be
based on a financial assessment.

Dorset
The local authorities, NHS Dorset and hospitals in Dorset
have collaborated to provide an integrated supply service,
with Adult Social Care currently supplying 9,000 items
of home equipment and adaptations to people with

is being trialled, aimed at providing greater choice and
more buying power to the end user.

Assistive technology and community alarms
Assistive technology or telecare services help vulnerable
people, particularly older people, to live in their own
homes with greater safety and independence.
They use a range of technologies that raise alarms to
get help for someone 24 hours a day and so provide help,
security and peace of mind. Special equipment can be
linked to your telephone system so you can get help quickly
if you need it, even if you are unable to speak. For some of
the services you do not even have to press a button to call
for help as they work on sensors that detect movements like
falls, gas or carbon monoxide escapes, or floods.

In order to enable people to live in their own home for
as long as possible, a range of home care services are
available. Help is on off er from day-to-day tasks such
as cleaning, shopping and food preparation/cooking,
through to personal care that helps with tasks like
dressing, bathing, toileting and prompting the taking
of medication. Carers need to be properly trained,
particularly in moving and handling and the use of hoists
for some tasks. Before you make any decisions regarding
the provision of care, be sure to contact several providers
and ask for a ‘Service User’s Guide’ and their charges and a
draft contract between you and the care provider.
Daily care/domestic support
Carers can call in on a daily basis to assist with any of
the tasks described above. Depending on the level of
help required their visits can be just half an hour or up
to several hours. Generally, visits are available from 7am
until 10pm. Some people will need multiple visits per day.
Night services can also be provided. The hourly rate for
these types of services depend on the services required,
the time of day and the location. Rural areas may present
particular diffi culties and a travel charge will probably be
made in addition to the normal hourly rate.
Live-in care
24 hour live-in care can accommodate people with a
very high dependency on a permanent basis. It can also
provide respite breaks for regular unpaid carers and short-
term support following hospital discharge.
In some cases it’s preferable and more economic to
have a carer actually living in the home. This can be for a
short period e.g. a week, or on an ongoing basis. Typical
charges for this service depend on the amount of care
and the particular skills required. Live-in care is also
available to people with permanent physical or mental
disabilities who require long-term ongoing care.
All home care providers are regulated and inspected
by the Care Quality Commission who publish inspection
reports on its website www.cqc.org.uk. When
considering a service it’s always a good idea to check their
report. See page 46 for further details.

• What experience does the agency have in your particular
field of need? Can they supply references for you to
check?
• Home care agencies providing personal care, must
be registered and inspected by the Care Quality
Commission. Ask to see a copy of their registration
certificate.
• How long has the agency been operating?
• How many carers would the agency assign to care for
you and would you see the same one every day? If not,
how does the staff rota operate and what happens if
your carer goes on holiday or is sick, will you be notified
in advance that a different carer will be attending?
• How can you contact the agency in an emergency or
outside office hours?
• How hard or easy would it be to make a complaint and
how are things then put right?
• If this is a private contract ask for a copy of the agency’s
contract terms and read these carefully with someone
you trust. Get as much help as you can going over it and
ask any questions you may have before signing
anything. The Citizens Advice Bureau or Age UK could
help with this.
These questions may be useful when considering using the services of a home care (domiciliary) provider to help you
build up a picture of how your care needs will be met in your own home.
People arranging and funding their own care either privately or through a Direct Payment or their Personal Budget are
able to choose their own worker or care agency. Lists of local home care providers begin on page 20.
• You should expect your personal preferences, dignity
and privacy to be respected. Discuss this with the
agency and ask how the most suitable carers for your
particular care are chosen. Can you talk to them before
deciding?
• Carers should be fully trained or be in ongoing training.
Ask the agency about their policies on this.
• You will have a care plan drawn up by the agency which
the carers will work to. Ask how often this plan will be
reviewed by the agency.
• Carers must be checked with the Disclosure and Barring
Service (DBS) and have a criminal records disclosure –
make sure this is the case.
• If your care needs do not meet Adult Social Care’s
eligibility criteria and have been arranged privately you
should check carefully the fee rates charged and exactly
what the payment you make covers.

East Boro Housing Trust’s Handy Van Repair Service
provides a friendly, professional repair service, providing
practical, useful help around the home.
If you are aged 60 or over, own or rent your home in
Bournemouth or Poole and receive benefit support, eg
pension credit or attendance allowance you are eligible
for the service.
Day services are provided in a variety of locations, ranging
from residential and nursing homes to community and
specialist centres. The services aim to help people live
as independently as possible. If you have a learning or
physical disability or if you are an older person, Adult
Social Care will help to support you in finding activities
which will help promote independence.
Unpaid carers sometimes need a break and this can be
provided in several ways: you may wish to go into a care
home yourself for a week or two; or arrange home-based
respite care where another carer moves into your home;
or family-based respite care where you move into another
family’s home for a period. The best option for you will
depend on your needs, the urgency of the situation and
If you look after, help or support a person who is ill, frail
or disabled who cannot manage without support and do
not receive pay to do this then you are a carer. The person
might be a child with a disability, an adult member of
your family, or a friend. You do not need to be living with
the person. You do not have to be giving physical care.

There is no charge for labour, no membership fees and
no hidden costs. All you pay for is the cost of materials
needed for your repair as the Handy Van Repair Service
is funded by the Borough of Poole and Bournemouth
Borough Council.
To be eligible for day services Adult Social Care will
have to assess your needs. This means they will have a
discussion with you to agree with you the best way of
organising support. They will consider your needs and
decide with you what services might be suitable for you.

Whilst looking after another person is mostly very
satisfying, it can also leave you isolated, tired, resentful or
angry. Caring can aff ect your personal life, your health,
leisure time and fi nances. As a carer you need to think
about things like:
• the type of help that you need to continue caring;
• whether you are willing or able to carry on caring;
• planning for the future; and
• how to meet your own needs.

Whether you care for someone for a few hours a week or
24 hours a day, you can access local support and services
that can assist you in your role as a carer.
You can keep up-to-date with the latest news and
national developments, fi nd out about carers’ rights and
benefi ts and search for events and activities in your local
area. The Carers Hub has been developed by the Dorset
Carers Partnership (DCP) that works to give local carers
a voice. The Carers Hub also includes information on

Poole and Bournemouth
The Carers Information Service for people caring for
someone living in Bournemouth and Poole keeps families
and friends informed of helpful news and events. Regular
events enable carers to meet up and enjoy an activity,
some of these with the person they support. Carers will
receive a free welcome pack when they join, copies of the
‘Home from Home’ off ers a day sitting/respite service
in the ‘Home from Home’ host’s own home. Each host
undertakes training, has an enhanced Disclosure and
Barring Scheme police check and their home has been
inspected to make sure it reaches appropriate health and
safety standards. The service is managed by East Boro
Housing Trust.
As well as needing a break, carers worry about what
would happen to the person they support if the carer is
unwell, or has an accident. Carers in Crisis is an emergency
back-up scheme, available to all carers of adults over 18,
that has been set up to give carers peace of mind by
alerting people to keep the person being cared for safe.

Throughout the year there are beach huts available and a
luxury holiday lodge that carers can apply to stay in.
There is a short break sitting service in both Poole and
Bournemouth that runs in a similar way to the service
in Dorset and is available to unpaid carers of cared
for people who are over 18. It is a fl exible service that
provides a care worker from a local approved agency to
stay with the cared for person. The service in both areas
is subsidised by the local authorities, but they are all run
slightly diff erently.

Direct includes:
• details of assessments, benefits, Direct Payments,
Personal Budgets and time off;
• help to maintain, leave or return to employment;
• help in education or training;
• advice on how carers maintain or improve their social and
emotional well-being and their physical and mental health;

provide a substantial level of care. This may lead to a

Dorset POPP is primarily a partnership between Dorset
County Council, Dorset Clinical Commissioning Group,
voluntary organisations and older people.
It is a complex and ambitious person-centred
programme, led by the needs, desires, and aspirations of
Dorset’s older people.
The vision of Dorset POPP is ‘to build supportive
communities to enable older people to remain living in
SAIL offers residents of Dorset free support, services or

in their own home. SAIL is a Dorset-wide partnership
between local authorities, Dorset Fire and Rescue Service,

Most of the schemes available to rent in Poole are
provided by Borough of Poole or local housing
associations. Allocation of these properties is usually
through the Council’s Housing Register. You will need
to speak to Housing and Community Services on

East Dorset Housing Association has a range of housing
specially designed to meet the needs of older people
consisting of flats, a communal lounge, a laundry and
gardens.
Schemes are especially suitable for very frail older
people with flats and bungalows built with wheelchair
access and level deck showers. All schemes have a visiting
sheltered housing officer or scheme manager. There are
Extra Care housing is a form of housing, similar to
sheltered schemes, with self-contained accommodation
together with some communal facilities. In an Extra
Care scheme, instead of low level support traditionally
provided by wardens in sheltered schemes, higher levels
of care and support are provided on site for people with
assessed needs by a designated team of care workers,

Extra Care allows people to retain their own tenancy
and have care services delivered ensuring that they
are able to remain safely in their own self-contained
accommodation.

A new Extra Care housing scheme has been developed in
Christchurch that will help older and disabled people to
live independently. The scheme at Bure House, has been
devised by East Dorset Housing Association in partnership
with Christchurch Borough and Dorset County councils.

Delphis Court offers 45 flats in a mixture of one and
two bedrooms, every resident will have their own self-
contained flat with their own front door. Responsive
support is available 24 hours a day and where appropriate,
safety devices can be installed within flats to support

Bournemouth
Self-contained apartments with extra care and support on
site are located in several developments in the town. High
standards of accommodation complement communal
facilities where you can relax and meet other residents.
Specialist care staff are available to provide services
to meet assessed needs. The latest technology is also
available to ensure your safety and security.
Bournemouth Borough Council works in partnership
with housing associations, the NHS, local doctors and
pharmacies to support your wellbeing. To find out more

types of property available to buy can vary considerably.

In early 2011 an Extra Care housing facility opened
in Poole - Delphis Court has been developed as part
of the new Poole Quarter developments and provides
a purpose-built facility for people who need a higher
level of care and support and want to maintain their

a waiting list and you must fully explain your housing

you want to rent privately, look in your local newspaper

• Buy your own home – Can you afford it? Are you able
to live on your own? You could think about ‘part buying’
and ‘part renting’ a place from a housing association.
• Move into a care home – If you want to leave home but
are not ready to live alone, care homes can meet your
personal needs and offer appropriate support. Adult
Social Care and Health will contribute if you are eligible
and if they agree the care home meets your needs. If
your chosen home costs more than usual, you may get
some help but you must pay the rest.
There are a range of care homes catering for younger
adults with learning difficulties, physical or sensory
impairments or mental health issues. There is also a range
of housing options you could explore.
If you are thinking about a care home placement, seek
advice from social and health care professionals about
your needs. Does the home provide opportunities to
learn and maintain skills, and opportunities to move on
into living in the community? What are the home’s links
with specialist staff in the community services? Use the
care homes checklist on page 41 when visiting potential

be fully accessible to disabled people. However, if you
choose an older home, there are a number of adaptations
that could make it easier for you to use. These include
wider doorways; a downstairs toilet; bathroom equipment
including handrails, a walk-in bath, electronic bath seat;
kitchen adaptations including lower working surfaces;
lower level light switches; easy-to-use controls for heating;
a stairlift; security intercom at the front door and ramp
access to the garden. You should also think about the

Some forms of dementia are not permanent and can be
caused by a range of emotional, physiological and physical
ailments, which bring on dementia-like symptoms. It is
important that any diagnosis eliminates the possibility of
a reversible dementia before a medical prognosis is made.
Following a diagnosis of dementia, your GP may refer you
to a specialist like a consultant in old age psychiatry, a
community psychiatric nurse or an occupational therapist
with a special interest in dementia care.
Support in the community
As it can be best for the person with dementia to stay in
their own home for as long as possible, a range of adult
social care services is available to facilitate this. For example,
they may provide access to: home care (discussed on page
18); meals in the home; sitting services (which provide a
break for any carer); advice on equipment and adaptations
(covered on page 12); day care centres (see page 29) and
respite care (explained on page 29).
These services can be provided directly by registered
providers to those who are not eligible for Adult Social Care
fi nancial help, however Adult Social Care will still be willing
to off er an assessment of need. You may have a choice of
which agency you receive your care from whether or not
Adult Social Care assists with the funding of your service.
People are entitled to receive an assessment of their care
needs and if eligible Adult Social Care may fund services to
meet these care needs or provide a Direct Payment so that
people can arrange their own care services in the way that
best suits them.

across Bournemouth, Poole and Dorset to implement the
requirements of the Dementia Strategy and to ensure
that people living with dementia have access to good
quality care services. The Dementia Strategy identifies how
crucial it is for people who may be experiencing the early
symptoms of dementia to be able to access good quality
advice and support and receive a diagnosis as to whether
in fact they have a dementia illness.
Alternatively people may want to initially access one of
the memory clinics or Dementia Cafés that are run by the
Alzheimer’s Society, details of these can be obtained from
the Alzheimer’s Society national helpline on 0300 222
1122 or the local office on 01202 716393. Information
about services provided by the Alzheimer’s Society is also

Memory cafés are, first and foremost, a setting in which
people with memory loss can share fun and laughter
with their care partners and friends in a setting free from
awkwardness and stigma. They are also a setting where
accurate information about resources is available and
relationships with persons in similar circumstances are
formed. They are a vehicle for bringing dementia ‘out of
the closet’ and into the heart of the community.

own home, or in hospital if you’ve been ill, or in a care home.
You will be fully involved in planning your care needs.
If a care home providing nursing care is the best
solution for you, the social worker will give you
information to help you find a home which meets your
care requirements. The cost of the nursing care part of
your fees is paid by the NHS to the home directly: the
current amount is £109.79 per week. You can contact the
Nurse Care Management Teams for your area at your local
Clinical Commissioning Group listed on page 51.

Differentiating one home from another can be difficult,
but the quality and quantity of staff training is especially
important, along with the turnover of staff, as continuity
of relationships in this type of home is critical.
The use of medication is a highly emotive subject and
any antipsychotic medication must be very carefully
prescribed. A fine balance exists between ‘acting in the
best interests of the individual’ by prescribing medicine
and the benefits of ‘normalisation’ where time and energy
are spent maintaining familiar, comfortable surroundings
which help reduce stress and possibly avoid the need for
certain medication.

Care homes offering personal care only
If you are reasonably active, but would like greater
security and care, subject to a needs assessment, a care

Deferred payment agreements
After the twelve-week property disregard period, Adult
Social Care may be able to help you while your property
is being sold. In these circumstances they will charge any
financial help they give against the value of your home.
This means they will take back the amount they have paid
once your property has been sold. However, they may
limit how much they will pay.
Did you know you can choose a care home outside the
area you currently live in? You may want to be closer to
friends, family members or you may want to relocate to
another part of the country.
If your care home place is state-funded, your local
authority is still responsible for your care fees, even if you
choose a care home in another area.
The home you choose must be suitable for your
assessed needs and comply with the terms and conditions
set by the authority. The fees that your local authority
will pay may vary. They may offer you the fees you would
receive if you remained where you are, or they may offer
you the fees that the local authority would pay in your
chosen area. You must seek further advice before making
your decision.
If you’re self-funded, obviously you have freedom of
choice, however it is important that you have considered
the fees the home charges and whether the home will
accept the local authority rates if or when your savings
fall below the capital limit. If the home is unable to meet
the local authority rate the possibility of moving to an
alternative home increases substantially.

important to seek financial and legal advice about the
various options you may have before committing yourself.
Attendance Allowance
This is a non-means-tested, non-taxable benefit from
the DWP. It’s paid at the lower rate of £53.00 a week if
you need care by day or night and at the higher rate
of £79.15 a week if you need care during the day and
night. Everyone who needs care can, and should, claim
Attendance Allowance. If you are paying the full cost of
residential care, with or without nursing care, you will be
entitled to the higher rate.
NHS Nursing Care Contribution
Whether you are a temporary or permanent resident,
if you live in a care home that provides nursing care
you may be entitled to NHS Nursing Care Contribution
towards the cost. It is not means-tested and is currently
£109.79 a week. This is paid directly to the home.

What if you run out of money?

advance that this is going to happen. They will then be
able to help with your care fees (provided your care needs
assessment has shown that you need residential care).
They will carry out a financial assessment to confirm the
date from which your funds fell below £23,250, and the
amount they, and you, will pay towards your care.
If the home you have chosen charges more than
the local authority will pay, you must find someone to
pay the difference – a ‘top-up’ payment. Whoever does
this, whether a family member or a charity, they should
understand that the cost could increase, and that they will
have to pay the top-up for as long as necessary. The local
authority will not meet the additional cost even if you have
lived in the home for a number of years. If you cannot find
someone to help you meet the extra cost, you may have to

about how a top up to your care home fees might be

If you move to a care home where a third party
payment is required, the person who will make the
payment on your behalf must sign a contract with Adult
Social Care, before the contract with the home is signed.
In doing so they must confirm that they have the financial
means to make the third party payments (including any
future increases) for the whole time you will live at the
care home.
If you are already a resident in a care home, and no
third party agreement was required at the time you
became a resident, the home may seek to introduce a
third party payment at a later date. Also if a change to
your arrangements is made at your request or with your
agreement, for example you move to a superior room,
then an additional third party payment can become due.

Some care homes and care homes with nursing charge
fees that are higher than the maximum amount Adult
Social Care can contribute. If they are contributing
towards your care home fees and you choose to move
into a home which charges a higher fee, the difference
between the two amounts is paid by a third party, usually
a relative. Third party payments are sometimes referred to
as ‘top-up’ payments.
Before you sign any contract, the home should give
you written details of all the charges it intends to make in
its ‘Statement of Purpose’ document. If there is anything
that is not clear or which you do not understand, you
should ask for advice.
As the name suggests, this payment must be made by
someone other than yourself or Adult Social Care. It can
be a relative or friend, but it cannot be a ‘liable relative’ i.e.
someone whose financial situation has been taken into
account when your financial assessment was carried out
by Adult Social Care.
The general rule is that you cannot use your own
money to fund a third party payment.
However in certain specific circumstances you may make
a third party payment from your own funds. These are:

Care fees can increase
It is usual for the fees charged by care homes to increase
each year. If you are not paying your own fees, the
amount that Adult Social Care can contribute may not
increase at the same rate as the home fees increase. Any
shortfall in fees must be met by a third party (top-up)
payment – discussed on page 45. The third party element
of the fee can only be increased once in any financial
year and as a guide, the amount should never increase
by more than 25% a year. You should receive at least one
month’s notice that the third party payment is to increase.
The person who makes the payment for you must sign a
new contract with Adult Social Care if the amount of the
third party payment increases.
If you are already living in a care home, plan ahead and
find out what help you will get from Adult Social Care
when your savings fall below the £23,250 limit, explained
on page 43.

When considering care services, it’s always a good idea
to check a service’s inspection report on the Care Quality
Commission’s comprehensive website: www.cqc.org.uk
Providers are required under essential standards of quality
and safety to have a simple and easy to use complaints
procedure that they will be happy to give you. If you are
concerned about the care that you, a friend or a relative is
receiving, it makes sense to speak to the manager before
you take any further action. The problem may be resolved
quite easily once they are made aware of it.
However, if you need to make a formal complaint, you
should initially contact the registered owners of the service.
They have a duty to respond to any complaints made.
If Adult Social Care have arranged and funded your care,
another option is to complain to your social worker/care
manager or the Complaints Manager in your local council:
All care providers must be registered under a system
introduced by the Health and Social Care Act 2008, which
brings adult social care, independent healthcare and the
NHS under a single set of essential standards of quality
and safety for the first time.
The Care Quality Commission (CQC) is the independent
regulator of health and social care in England. They regulate
care provided through a range of methods including:
• registering providers to ensure they are meeting the
essential standards of quality and safety;
• monitoring how providers comply with the standards;
• promoting improvement in services;
• using enforcement powers where necessary;
• publishing up to date information about the compliance
of adult social care providers with essential standards of
safety and quality.
All organisations need to know how they are performing.
They are happy to receive your feedback on their service
whether they are compliments or complaints. Feel free to
tell them what you think and your comments can be used
constructively to improve the service.
If you do need to make a complaint you should feel
able to complain about any aspect of your care that
affects your happiness or comfort. This could be about
the way you are treated by a staff member or the quality
of the food you are served. You can also make comments
and suggestions about possible improvements to your
surroundings and the services provided.
Making a complaint should not be difficult for you.

Healthwatch is the new independent consumer
champion created to gather and represent the views of
the public. Healthwatch plays a role at both national and

and people who use services are taken into account
Three Dorset organisations - Help & Care, Citizens Advice
in Dorset and Dorset Race Equality Council - have joined
forces and will work to ensure the health and well-being of
Dorset’s residents is championed across the county
Local Healthwatch has taken on the work of the Local
Involvement Networks (LINks) and also represents the
views of people who use services, carers and the public on
the Health and Wellbeing boards set up by local authorities,
provides information and signposting about local health
and care services and reports concerns about the quality
of healthcare to Healthwatch England, which can then
recommend that the Care Quality Commission take action.

A vulnerable adult is any person aged 18 years or over
• who is or may be in need of community services
because of mental or other illness, disability or age, and
• who is, or may be, unable to take care of themselves
against significant harm or serious exploitation.
Abuse is a violation of an individual’s human and civil
rights and may consist of a single act or repeated actions.
It may be physical, sexual, financial, psychological or
institutional, or may relate to neglect or discrimination.

Good nursing homes continue to provide a safe,
comfortable, caring environment for anyone requiring
nursing – whether long-term or for short-stay respite,
convalescence, or rehabilitation.
The nursing home of today is a far cry from the
outdated perception of latter years. Today’s nursing home
combines an ambience of the client’s personal home,
with quality furnishings and surroundings, together
with a warm caring atmosphere backed up by qualified,
professional nursing staff. A total package of care is
provided, designed exclusively for the individual, which
encompasses social, medical and welfare needs.
It may be that you are making the choice, or more likely
nowadays, a family member or close friend is looking on
your behalf.
There is one level of comfort that can go a long way to
removing some of the concerns of placement – choosing
a ‘blue cross’ home.
What should you expect from a quality ‘blue cross’ home?
• 24 hour nursing services provided by a team led by a
qualified nurse.

social workers who have undertaken further specialist
training and who know a great deal about the types of
concern that arise.

It would be nice to think that your retirement and old
age will be worry free. It can be. Advance thought and
preparation can pay dividends for you and your loved ones
at a time when you may be less able to cope by yourself.
Some people are lucky enough never to need help with
their financial or personal arrangements. For many,
though, health issues or general slowing down in old age
mean that decisions and action on day to day matters
(which seem routine when you were younger and fitter)
need to be taken by someone else on your behalf.
If you take no steps to make arrangements yourself
then the only course of action may be the appointment
of a Deputy through the Court of Protection. The Court
will oversee the actions of your Deputy and complicated
work is necessary to comply with the Court’s
requirements. A level of formality is needed which can
seem harsh and intrusive, not to mention expensive.
The alternative is for you to make decisions yourself
ahead of the difficult times which you may face in the
future in the form of a Lasting Power of Attorney (LPA)
for either your financial or welfare affairs or both. An
LPA enables you to appoint people of your choice to
handle your affairs and gives your personal authority to
them to act with greater ease on your behalf.
It is important to note that if you have already made
an Enduring Power of Attorney it will probably still be
effective, but it would be sensible to take advice on
whether a welfare LPA would suit your future needs.

Deciding to ask people to come into your home to care for you or a loved-one can seem daunting. However, there are a few straightforward questions you can ask to help decide on a care provider.Ask how carers are recruited and trained. They should be subject to a Disclosure and Barring Service Check (formerly known as a CRB check). Carers should also have been trained in moving and handling, to protect both their clients and themselves. If carers do not hold a QCF (Qualifications and Credit Framework – previously the NVQ) award in Health and Social Care, then a good provider will have enrolled them on a course. Professional development opportunities grow a motivated, quality team of carers. You should check how the calls will be scheduled. Will you get care at a time convenient for the company’s existing schedules or will it be given when you need it?
So, if you want to go to bed after watching TV in the evening, that is when the carer should come, not in the afternoon.In just over a year, Right at Home has recruited thirty incredible carers.
