Documents required for applying to MMD Chennai for revalidation is as follows: 1. Photocopy of COC- 2 Copies 2. Photocopy of GOC & STCW endorsement - 2 copies 3. Photocopy of INDOS Certificate- 2 copies 4. Photocopy of Passport (in colour) - 2 copies (Front and back side) 5. Photocopy of CDC (relevant Pages) - 2 copies 6. 35mm x 35mm Colour Photographs- 5 numbers. 7. Original Medical Fitness certificate as per ILO Standards by DGS Approved Doctor. 8. Photocopy of all advance course certificates - 2 copies 9. Photocopy of the Revalidation Course - 2 copies 10. Service Letter from the company/Local agent (with RPSL No.) showing a proof of having sailed for at least 12 months in previous 5 years as per the format NAME OF VESSEL OFFICAL NO. TYPE OF VESSEL KWT GRT NRT COSTAL/F.G RANK FROM TO The name in the COC should be as same as in the Passport / CDC and if it is different an Affidavit has to be obtained from the Notary Public on a Non-Judicial Stamp Paper of Rs.20/- confirming that the name in the COC as well as in the passport/CDC are one and the same. This is also applicable to the candidates having 2 different place of birth in the CDC and Passport. Note : All photocopies need to be self attested for submission at MMD Chennai Submission of COC Application at Chennai MMD: HIMT is glad to inform that we have been assisting the candidates undergoing the course for submitting the completed application form to MMD, Chennai on behalf of the candidates. In the interest of the candidate, HIMT has also been rendering following additional value added services for those candidates who wish to seek assistance from HIMT for applying their completed application form to MMD Chennai for Revalidating the existing COC/ Receiving New Revalidated COC(Hard cover) after completing the course:- The MMD fees for revalidation of COC is Rs 5000/- . Kindly note that this services are extended to the candidates only to assist them in submission of application forms and HIMT will not be responsible for rejection of application forms or further change in rules (after submission of the application form to MMD). It would be the sole responsibility of the candidate to liaise with MMD further for revalidating their COC / obtaining the New Hard cover. HIMT, in no way would be responsible for any delay/rejection of the candidates COC. As per the existing norms it may take about 1- 2 weeks for revalidation of COC at Chennai MMD depending upon the availability of the Surveyor. Note: All applications that are collected prior 0900 hrs of Friday would be submitted to MMD, Chennai for revalidation on subsequent Monday, Subject to minimum of 10 applications. An e-mail would be sent within 48 hrs of submission to MMD, along with scanned copy of the receipt as a proof of submission to MMD. Submit in cash counter MMD fees, application form along with documents could be submitted in a envelope for nautical side and in plastic folders for engineers or as required by MMD. Documents once submitted to the cash counter would not be returned for filling or attaching any additional documents under any circumstances.
